PURPOSE: To report a case of successful treatment with infliximab for Behçet disease (BD) during pregnancy.
DESIGN: A case report.
METHODS: A 30-year-old woman was diagnosed with BD at 12 weeks of pregnancy. Additional symptoms occurred, and infliximab at a dose of 5 mg/kg body weight was initiated at 18 weeks and repeated.
RESULTS: All symptoms improved, and a 3950-g infant was delivered uneventfully without any abnormality at 39 weeks. A few weeks after delivery, uveitis and systemic symptoms relapsed. Infliximab was reinitiated and all symptoms were resolved.

Infliximab, a novel chimeric anti-tumor necrosis factor-alpha (TNF-alpha) monoclonal antibody, has been increasingly used in the treatment of pyoderma gangrenosum. However, an established dosing regimen is lacking in the published literature. A variety of dosing regimens have been suggested, including a treatment schedule similar to that of psoriasis. The authors report a case of rapid response to infliximab in a patient with pyoderma gangrenosum associated with inflammatory bowel disease utilizing a dosing regimen similar to that used for psoriasis.

A 4-month-old female infant presented with widespread pyoderma gangrenosum associated with stridor, presumed secondary to tracheal involvement. No underlying cause was revealed despite extensive investigation. Treatment with immunosuppressive agents only partially suppressed disease activity. Complete resolution followed treatment with infliximab. We review this rare condition in infants and discuss treatment with infliximab not previously described in this age group.

BACKGROUND: Pustular psoriasis is an uncommon form of psoriasis that often affects areas of the hands and feet. It typically manifests as small pustules that develop within erythematous areas of the palms and soles. Infliximab, a tumor necrosis factor inhibitor, can be used to treat pustular psoriasis. Tumor necrosis factor (TNF)-α inhibitors are increasingly being used to treat rheumatoid arthritis. Infliximab (INF) is a TNF-α inhibitor that is usually used in combination with methotrexate (MTX). Interstitial lung disease (ILD) during combination therapy has been attributed to MTX rather than INF. However, INF-associated ILD without combination with MTX has recently been reported. We describe herein a case of severe ILD secondary to INF without MTX therapy.
