An unprecedented enantioselective and general olefin isomerization was realized via biomimetic proton transfer catalysis with a new chiral organic catalyst. A broad range of mono- and disubstituted β,γ-unsaturated butenolides were transformed into the corresponding chiral α,β-unsaturated butenolides in high enantioselectivity and yield in the presence of as low as 0.5 mol % catalyst. Mechanistic studies have revealed the protonation as the rate-determining step.

The density dependence of diiodomethane photoinduced isomerization in supercritical (sc) CO2, CHF3, and C2H6 was investigated by transient absorption spectroscopy, covering a fluid density range from 0.7 to 2.5 (in reduced units). We present the case of a 1-month-old infant with right atrial isomerism and complex functionally univentricular heart disease, in whom totally anomalous pulmonary venous return drained below the diaphragm via two separate vertical veins. One of the descending vertical veins drained the entire right lung and the left upper lobe, while the other drained the remainder of the left lung. BACKGROUND: Heterotaxy has been demonstrated to reduce survival. There are several different subgroups of patients, however, and no single study has had a large number of patients and analyzed survival across the different subgroups such as patients born in different eras, patients with right and left isomerism, and patients with biventricular or functionally univentricular hearts. LI is a subset of the heterotaxy syndrome and a rare birth defect that involves the heart and other organs. It can be combined with extracardiac abnormalities, especially BA. CHD can be associated with LI in up to 15% of cases, although it is rare in BA. Pediatric LT for a child with ESLD due to BA combined with LI and CHD is a challenging issue for a transplant surgeon. Herein, we report a successful split LT on a three-yr-old boy with LI who survived after a Fontan procedure due to single ventricle, but who suffered from HPS associated with BA.

An intraatrial reentrant circuit was identified using an electroanatomical mapping system and evaluation of postpacing intervals in a patient with isomerism of the left atrial appendages and atrioventricular septal defect. Intraatrial reentrant tachycardia was eliminated on the basis of our interpretation of the reentry circuit being dependent on a new anatomical obstacle consisting of a right-sided atrioventricular annulus and atrial septation patch. We must consider the possibility of arrhythmogenic obstacles changing, as a patient grows, long after congenital heart disease surgery.

This case report presents a low-birth-weight neonate who received primary stent implantation as a long-term palliative intervention for obstructive mixed-type total anomalous pulmonary venous connection (TAPVC) without common pulmonary venous chamber associated with right atrial isomerism, which was considered difficult to surgically repair in the neonatal period. Stent redilation with balloon catheters was repeated for in-stent stenosis from neointimal proliferation, resulting in successful TAPVC repair with cavopulmonary connection at 17 months of age.

We report successful primary surgical repair of transposition of the great arteries (TGA) and complete atrioventricular septal defect (AVSD) associated with left isomerism {I,L,L} without pulmonary obstruction in a 1-month-old infant. The operative procedure consisted of an arterial switch and a two-patch repair of the AVSD. To the best of our knowledge, this is the first report of successful primary surgical repair of TGA and AVSD with left isomerism. Primary anatomic correction of this defect combination is feasible in patients with left isomerism.

We describe an autopsied specimen, from an infant who died in 1967, in which concordant ventriculoarterial connections coexisted with parallel arterial trunks, usually described as "anatomically corrected malposition." Based on their shape, the extent of the pectinate muscles, and the absence of a coronary sinus, we diagnosed isomeric right atrial appendages. Despite this, the arrangement of the remaining thoracic and abdominal organs, as well as the patterns of venous drainage, was almost completely mirror imaged.
