Reflections: neurology and the humanities. Odd harvest.

Brain donation is an unusual pursuit. It is an aspect of medical research that the lay public may not necessarily know much about. In 2003, to learn more about the underlying pathophysiology of essential tremor (ET), we established the Essential Tremor Brain Repository (ETCBR) at Columbia University, a research effort whose purpose was to enhance understanding of the ET brain (NIH R01 NS042859, Elan D. Louis, principal investigator). Now, at a 10-year point in this endeavor, we pause for a moment to reflect, distill, and share. Why? From the moment potential brain donors first inquire about th

Frontal lobe neurology and the creative mind.

Concepts from cognitive neuroscience strongly suggest that the prefrontal cortex (PFC) plays a crucial role in the cognitive functions necessary for creative thinking. Functional imaging studies have repeatedly demonstrated the involvement of PFC in creativity tasks. Patient studies have demonstrated that frontal damage due to focal lesions or neurodegenerative diseases are associated with impairments in various creativity tasks. However, against all odds, a series of clinical observations has reported the facilitation of artistic production in patients with neurodegenerative diseases affectin

Neuromarketing and consumer neuroscience: contributions to neurology.

BACKGROUND: 'Neuromarketing' is a term that has often been used in the media in recent years. These public discussions have generally center … rketing scholars have eagerly integrated neuroscientific evidence into their theoretical framework, neurology has only recently started to draw its attention to the results of consumer neuroscience. DISCUSSION: In this paper we address key research topics of consumer neuroscience that we think are of interest for neurologists; namely the reward system, tru …

[Inborn errors of metabolism in adult neurology].

Inborn errors of metabolism (IEM) are caused by mutations in genes coding for enzymes and other proteins involved in cell metabolism. Many IEM can be treated effectively. Although IEM have usually been considered pediatric diseases, they can present at any age, mostly with neurological and psychiatric symptoms, and therefore constitute an integral subspeciality of neurology. However, although they are increasingly being recognized, IEM remain rare, and the care for patients should be optimized in specialized reference centers. Since the number of different diseases is very large, the diagnosti…

Clinical applications of intravenous immunoglobulins in neurology.

Intravenous immunoglobulin (IVIg) is used increasingly in the management of patients with neurological conditions. The efficacy and safety of IVIg treatment in chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) and Guillain-Barré syndrome (GBS) have been established clearly in randomized controlled trials and summarized in Cochrane systematic reviews. However, questions remain regarding the dose, timing and duration of IVIg treatment in both disorders. Reports about successful IVIg treatment in other neurological conditions exist, but its use remains investigational. IVIg has bee

[The use of intravenous immunoglobulins in neurology].

INTRODUCTION: In the past decade, intravenous immunoglobulins (IVIG) have been widely used and their administration has grown throughout the world. The current indications of IVIG in neurological diseases are discussed on the basis of the passed and current trials. Unlike other immuomodulatory agents, IVIG are well tolerated and have very few side effects and a good viral safety. STATE OF ART: There is clinical evidence, based on controlled trials, for the effectiveness of IVIG in Guillain-Barré syndrome, chronic inflammatory demyelinating polyneuropathy and multifocal neuropathy with conduct

Epidemic encephalitis and American neurology, 1919-1940.

Encephalitis lethargica, also known as epidemic encephalitis, emerged as a new infectious disease near the end of the First World War. Bacteriologic, epidemiologic, and clinical investigation produced no clear consensus regarding the nature of the disease, even as several other experimentally demonstrable "encephalitides" appeared on the scene. By 1940, new encephalitis lethargica cases had almost entirely disappeared, and neurologists renamed this once-novel infection as an amorphous syndrome of marginal interest. A variety of forces influencing the fate of encephalitis lethargica's epidemic

[Movement disorders and botulinum toxin in neurology].

Botulinum toxin has been a useful treatment in many movement disorders and more recently in other non-neurological motor dysfunctions for more than 15 years. Here, we review the various indications in neurology, mainly in the field of movement disorders. From 1973 to 2002, we searched the Medline database on this topic. We selected the most useful and relevant papers, with a special interest in dystonia. We summarized the results in the main indications (spasmodic torticollis, bleparospasm, hemifacial spasm) and in other manifestations such as writer's cramp, oromandibular dystonia, tremor, ti…

Origins of the sensory examination in neurology.

Formal testing of sensation as part of the neurological examination followed the improvements in examination techniques as well as advances in neuroscience. By the 1890s, the observation that temperature sense was frequently impaired at the same time that pain was appreciated led to the supposition that the two paths traveled closely. Through the works of Brown-Séquard and Edinger the existence of a crossed afferent tract was verified. The distinction between two sensory pathways was clear by 1898, when van Gehuchten reported a case of syringomyelia and suggested that the pain and temperature

Neurology: an ancient sensory organ in crocodilians.

Crocodilians hunt at night, waiting half-submerged for land-bound prey to disturb the water surface. Here I show that crocodilians have specialized sensory organs on their faces that can detect small disruptions in the surface of the surrounding water, and which are linked to a dedicated, hypertrophied nerve system. Such 'dome' pressure receptors are also evident in fossils from the Jurassic period, indicating that these semi-aquatic predators solved the problem of combining armour with tactile sensitivity many millions of years ago.

The neurology and neuropathology of coeliac disease.

A number of neurological syndromes have been described in association with coeliac disease. These include disorders of the central nervous system encompassing epilepsy, myoclonus, ataxia, internuclear opthalmoplegia, multifocal leukoencephalopathy and dementia. Most of these associated conditions show a poor response to gluten restriction. Peripheral neuropathies, of axonal and demyelinating types, have also been reported and may respond to elimination of gluten from the diet. The mechanism underlying these processes remains obscure but may be immunological or related to trace vitamin deficien

[Epidemiological aspects in occupational neurology. Investigational methodology]].

INTRODUCTION: In the working population, unsatisfactory working conditions (in the broadest sense) may cause effects whose gravity depends mainly on the characteristics of exposure and the general condition of the individual. DEVELOPMENT: These effects may, in practice, be classified as occupational accidents and professional illness (both legal concepts), illnesses related to work, diminished quality of life and effects on offspring. Agents as varied as head injuries and metals are known to cause from loss of memory or behavior disorders to chronic toxic encephalopathy or Parkinsonism. There

[New developments and perspectives of intensive neurology].

In the last few years considerable advances have been made in the diagnosis and treatment of cerebrovascular diseases, neurotrauma, and infections or inflammatory diseases of the CNS. Although thrombolysis of medial cerebral artery infarction and craniotomy with dural replacement plastic in space-occupying medial cerebral artery infarction only concern a minority of "patients with acute cerebral infarction", these treatment strategies have now been accepted as justifiable methods in the acute management of vascular ischaemic cerebral events. In addition to invasive and non-invasive monitoring

Hospital-acquired morbidity on a neurology service.

Clinical services must monitor hospital-acquired morbidity, but what rates are expected specifically for neurology inpatients is not evident from published studies. We studied prospectively 1317 consecutive admissions to a neurology service in a university-affiliated city hospital from 1987 to 1990 and recorded all nosocomial infections, nosocomial pneumonia, and decubitus ulcers of stage III or IV. Over the 3-year period, 6.8% of patients had > or = 1 nosocomial infection (and almost half of these had a nosocomial bloodstream infection); 3.1% had > or = 1 case of nosocomial pneumonia; 1.2% de…

[Telemetric electromyography in kinesiological investigations in neurology].

Telemetrical electromyographical investigations were performed in 23 healthy subjects and 33 patients with various diseases of the extrapyramidal system. The comparison of our findings leads to electromyographic criteria in the differential diagnosis of these disorders. As the majority of previous investigations with this method centered on questions concerning functional anatomy, a number of modifications were introduced to apply telemetrical methods to neurological problems. The following pathological findings were elicited by comparison of normal subjects and patients with extrapyramidal di
