Trends and Technologies in Rehabilitation of Foot Drop: A Systematic Review.

INTRODUCTION: Foot Drop (FD) is a condition, which is very commonly found in post-stoke patients; however it can also be seen in patients with multiple sclerosis, and cerebral palsy. It is a sign of neuromuscular damage caused by the weakness of the muscles. There are various approaches of FD's rehabilitation, such as physiotherapy, surgery, and the use of technological devices. Recently, researchers have worked on developing various technologies to enhance assisting and rehabilitation of FD. AREAS COVERED: This review analyzes different types of technologies available for FD. This include de

Modeling the walking patterns of Reciprocating Gait Orthosis users with a novel Lower Limb Paralysis Simulator.

A mechanical Lower Limb Paralysis Simulator (LLPS) was developed for able-bodied persons to model the gait of Reciprocating Gait Orthosis (RGO) users. The purpose of this study was to determine if able-bodied subjects ambulating with the LLPS exhibited gait characteristics typical of RGO users. Five able-bodied persons were trained to ambulate with the LLPS and underwent a motion gait analysis. LLPS users were found to exhibit gait patterns that were characteristic of RGO-assisted gait.

Control and implementation of a powered lower limb orthosis to aid walking in paraplegic individuals.

This paper describes a powered lower-limb orthosis that is intended to provide gait assistance to spinal cord injured (SCI) individuals by providing assistive torques at both hip and knee joints, along with a user interface and control structure that enables control of the powered orthosis via upper-body influence. The orthosis and control structure was experimentally implemented on a paraplegic subject (T10 complete) in order to provide a preliminary characterization of its capability to provide basic walking. Data and video is presented from these initial trials, which indicates that the ort

[The Ponseti method of treatment of congenital clubfoot--first experiences].

PURPOSE OF THE STUDY: To provide a detailed description of the Ponseti method and report the first results of its use, including factors that played a role. MATERIAL AND METHODS: In the 2005-2007 period, 91 patients with idiopathic rigid clubfoot (133 feet) were treated by the Ponseti method. The group comprised 62 boys and 29 girls. In most patients the Ponseti method was used as primary treatment, or by 3 months of age when previous treatment failed. In five children this treatment was started between the 3rd and 8th months of age. The result were evaluated by the criteria described by Rich

Rehabilitative treatment in flexible flatfoot: a perspective cohort study.

Paediatric valgus flexible flatfoot is a common childhood paramorphism. Its treatment options consist of rehabilitation, corrective footwear and, if necessary, surgical intervention. The aim of our study was to compare a group of children who followed a rehabilitative programme versus a historical group of children who had been treated with insoles and orthopaedic footwear. Over a 2 year period (1995-1997), 300 children (mean age was 3.4-184 male, 116 female) with bilateral flexible flatfoot (600 feet) were recruited and underwent a rehabilitative programme for a mean period of 2.75 years. The

Neuromuscular issues in cavovarus foot.

Differential muscle weakness can cause a cavus foot deformity. Presenting complaints in the hindfoot may include ankle instability, secondary arthritis, or peroneal tendonitis. Presenting complaints in the forefoot may include stress fractures, callus formation over the lateral border of the foot, claw toes, first ray overload, and metatarsalgia. More general presenting complaints can include a drop-foot gait, decreased walking tolerance, and difficulty with shoe or orthotic fitting. To surgically correct the foot shape, soft tissue contractures need to be released, bone deformity corrected, a

[Acquired adult flatfoot deformity: a pragmatic approach].

Acquired adult flatfoot deformity is a commonly encountered entity in orthopaedic practice. It results mainly from posterior tibial tendon (PTT) dysfunction, is more common in women and frequently associated with obesity. The four severity stages are based on dysfunction of the PTT, on the reducibility of the deformity, and on the condition of the hindfoot joints. Conservative treatment is functionally efficient in the early stages and may also contribute to analgesia in advanced stages. The goal of surgery is to restore a functional mobility in the early stages and to achieve rigid stabilizat

Cranial remodeling devices: treatment of deformational plagiocephaly and postsurgical applications.

Since the first cranial remodeling devices were introduced in 1979, both their design and availability have continued to evolve. Today, these devices are used to treat deformational plagiocephaly (plagiocephaly, brachycephaly, and dolichocephaly) and are used as adjuncts to surgery for craniosynostosis. In deformational plagiocephaly, the goal is to improve cranial symmetry and return the cranium to a more normal proportion. Postoperatively, these devices are used to provide stabilization and to enhance surgical outcomes. Numerous clinical studies have demonstrated the safety and efficacy of t

[Effects of taping therapy on the deformed angle of the foot and pain in hallux valgus patients].

PURPOSE: This study was to examine the effects of Taping therapy on the deformed angle of the foot and pain in hallux valgus patients. METHOD: The subjects were 24 feet from 15 patients who were diagnosed withhallus valgus at the orthopedic department of K University Hospital in Seoul. Taping therapy was conducted 15 times overall during a four-week period. Data was analyzed using descriptive statistics and t-test. RESULT: The deformed angle of the foot of the hallus valgus patients significantly improved from 21.95 (4.38) to 18.75 (4.80) after Taping therapy. Pain significantly decreased fr

Pavlik: the man and his method.

Because of the high rate of avascular necrosis, Pavlik developed his harness and method of treating developmental dysplasia of the hip in infants. This historical review highlights Pavlik's career from the time he worked with Frejka and then later in his own clinic in Olomouc, Czech Republic (1938). In the 1950s, he wrote five articles on the results of his harness use in developmental dysplasia of the hip. Subsequently, Pavlik's harness has become the orthosis of choice worldwide for treatment of infants with hip dysplasia because of the principles he espoused.

Pedorthic considerations in the management of the injured worker.

The pedorthist works from a physician's prescription, which includes a diagnosis, modality, and desired goals. The pedorthist's and the physician's combined efforts result in the patient's foot, shoe, shoe modifications, and foot orthoses working together as one unit.

Effective treatment by orthosis of dislocation of the patella associated with Down's syndrome. A report of case.

The authors reported a case of recurrent dislocation of the patella associated with Down's syndrome. As for treatment, it is important to consider the levels of intelligence and daily activity, severe obesity, and certain internal medical conditions associated with this disease. Primary orthotic treatment is recommended for subluxation or reducible dislocation of the patella.

Prospective study of congenital dislocation of the hip.

A prospective study involving 62 newborns diagnosed as having subluxable or dislocatable hips has established the efficacy of treating such cases from birth. Patients were treated first with a simple abduction splint, leading from a plastic abduction brace to the Pavlik harness and finally, a hip spica. Routine clinical examination showed this to be necessary until hip stability was achieved. This approach, together with continuing, detailed, clinical examinations, produced a 100% success rate with no iatrogenic complications.

[The consideration of footforms for a better judgement and explanation of footcomplains (author's transl)].

Differentiating individual foot types has decisive advantages in diagnosing and treating fallen arches and splayfoot. The basic methods of classifying different foot types are discussed and the necessity of such classifying is shown. Seemingly different interpretations of foot malformations are thereby explicable. Reference is made to the problem of therapeutic failures resulting from non-observance of the interaction between the arch-support and the shoes worn. The positive effects of indicated alterations in heel height are demonstrated.
