Evaluating complications of local anesthesia administration and reversal with phentolamine mesylate in a portable pediatric dental clinic.

This study sought to identify and quantify complications with local anesthetic administration and reversal on consecutive patients seen for comprehensive dental care in a school-based, portable dental clinic, and includes data on the patients seen by the participating portable dental providers. In 923 dental visits where local anesthetic was administered, a standardized form was used to gain further information and identify any complications; this was accompanied by a questionnaire for the student's teacher, in order to quantify the student's distraction and disruption ratings following the de

Anesthesia management in pediatric patients with laryngeal papillomatosis undergoing suspension laryngoscopic surgery and a review of the literature.

OBJECTIVE: The excision of laryngeal papillomas poses a great challenge for both the anesthesiologist and the surgeon. The narrowness of the airways and the great variability of the pathological lesions necessitate close collaboration between the surgical and anesthesia teams to provide optimal operating conditions and ensure adequate ventilation and oxygenation. Our aim was to explore perioperative anesthesia management in pediatric patients during the excision of laryngeal papillomas with a suspension laryngoscope. METHODS: Fifty-eight pediatric patients suffering from laryngeal papillomas

Safety of Local Anesthesia in Pediatric Dermatologic Surgery: Our Experience and a Concise Review of the Existing Literature.

INTRODUCTION: Pediatric dermatologic surgery is important for both diagnosis and treatment, including excision of suspected cutaneous neoplasms. While general anesthesia and deep sedation are well studied in this setting, evidence on the safety of local anesthesia is more limited. OBJECTIVES: To evaluate the safety of local anesthesia in pediatric dermatologic surgery and compare our findings with a concise review of the literature. METHODS: We conducted a single-center retrospective study of 678 children who underwent skin biopsy or excision with histopathologic diagnosis at the Pediatric D

Reducing General Anesthesia for Lumbar Punctures in Pediatric Patients With Leukemia and Lymphoma, a Quality Improvement Project.

PURPOSE: Curative therapy for most children with leukemia and lymphoma includes multiple lumbar punctures (LPs) for the administration of intrathecal chemotherapy, commonly facilitated with deep sedation or general anesthesia (GA). There is ongoing concern about the potential for neurocognitive deficits induced by GA in the developing brain. The objective of this quality improvement (QI) project was to implement and evaluate a process whereby children received the level of sedation they need for their LPs and not GA by default. METHODS: We included children at our center aged 5-18 years who r

Relationship between decrement time and recovery time in pediatric total intravenous anesthesia with propofol and remifentanil.

BACKGROUND: Propofol target-controlled infusion is now used in many countries for the administration of total intravenous anesthesia (TIVA) and uses population pharmacokinetic data incorporated into the target-controlled infusion pump to administer a dose which will achieve a target concentration selected by the practitioner. Modern target-controlled infusion pumps can include an estimate of how long it will take for the plasma propofol concentration to fall to a value programmed into the pump, once the propofol infusion is stopped. This is known as decrement time. Many users consider decremen

Improving Identification of Pediatric Emergence Delirium in the Post Anesthesia Care Unit: A Quality Improvement Opportunity.

OBJECTIVE: This article offers a recommendation on how the Pediatric Assessment Emergence Delirium Scale (PAEDS) could be implemented in the post anesthesia care unit (PACU) to improve the assessment and treatment of pediatric emergence delirium (PED). BACKGROUND: PED is an anticipated complication in the PACU characterized by mental confusion, irritability, disorientation, inconsolable crying, and prolonged postanesthetic recovery time. Although it is a short-lived phenomenon, it increases the risk for traumatic injuries and may lead to a decrease in overall parent satisfaction with their ch

Epidural Anesthesia and Continuous Epidural Analgesia in a Pediatric Patient With Pelizaeus-Merzbacher Disease: A Case Report.

Pelizaeus-Merzbacher disease (PMD) is a genetic leukodystrophy, which is a progressive and degenerative central nervous system abnormality caused by dysmyelination. Because the incidence of PMD is extremely low, only a few case reports have been published regarding its anesthetic management. In particular, epidural anesthesia has only been reported in one case of general anesthesia combined with caudal anesthesia. We performed general anesthesia combined with epidural anesthesia for the soft-tissue release surgery for bilateral hip subluxation in a six-year-old male patient diagnosed with PMD.

Caudal dexmedetomidine in pediatric caudal anesthesia: A systematic review and meta-analysis of randomized controlled trials.

OBJECTIVES: To evaluate the efficacy and safety of caudal dexmedetomidine in pediatric caudal anesthesia (CA). METHODS: We searched PubMed, Embased, and Cochrane Library (from inception to June 2019) for eligible studies. The primary outcomes were the time to first analgesia, time of postoperative eye opening, intraoperative endtidal sevoflurane concentration, and postoperative sedation score. We calculated pooled risk ratios (RR) and 95% CIs using random- or fixed-effects models. RESULTS: Thirteen trials involving 793 patients were found. Meta-analysis showed that the time to first rescue p

Supraclavicular approach for subclavian vein catheterization in pediatric anesthesia: The reborn of an ancient technique with the ultrasound's assistance.

The insertion of a central venous catheter is a procedure frequently performed in paediatric anaesthesia. The most frequent approach for this procedure is the internal jugular vein. However, some specific factors in children, such as the short neck and the easy collapse of this vein, which is accentuated in situations such as hypovolaemia and changes in the respiratory pattern, can lead to greater difficulty. The supraclavicular approach of the subclavian vein was described more than 50 years ago. However, it is not often considered nowadays, even after the introduction of ultrasound, and desp

Acute masseter dystonia in a pediatric patient receiving aripiprazole and methylphenidate following induction of general anesthesia.

An 11-year-old male receiving aripiprazole, methylphenidate, and clonidine developed acute masseter dystonia inhibiting tracheal intubation after induction of general anesthesia with propofol and rocuronium. Following emergence, he had trismus and jaw discomfort. Psychiatry consultation suspected an acute dystonic reaction, so diphenhydramine was administered intravenously which resolved symptoms. We suspect chronic aripiprazole and methylphenidate usage combined with propofol administration in the short-term absence of methylphenidate made this patient susceptible to dystonic reactions.

Radiation treatment for the right naris in a pediatric anesthesia patient using an adaptive oral airway technique.

Radiation therapy for pediatric patients often includes the use of intravenous anesthesia with supplemental oxygen delivered via the nasal cannula. Here, we describe the use of an adaptive anesthesia technique for electron irradiation of the right naris in a preschool-aged patient treated under anesthesia. The need for an intranasal bolus plug precluded the use of standard oxygen supplementation. This novel technique required the multidisciplinary expertise of anesthesiologists, radiation therapists, medical dosimetrists, medical physicists, and radiation oncologists to ensure a safe and repro

[Efficiency of pre-emptive analgesia using ketoprofen in a program of combined regional anesthesia in pediatric orthopedics].

It is difficult to make an objective assessment of postoperative pain in children so anesthetic methods must ensure prolonged effective analgesia. The study was undertaken to determine the efficiency and duration of analgesia when using ketoprofen in a combined regional anesthesia (CRA) program in children aged 13 to 15 years who had correctable supracondylar osteotomy of the hip. Preference is given to preventive parenteral administration of ketoprofen. Comparative assessment of intra- and postoperative monitored variables and needs for postoperative analgesia in the traditional CRA (n = 10)

[Induction of general anesthesia with ketamine for short surgical interventions in pediatric patients with chronic hepatic pathologies].

Serum levels of bilirubin and of other four enzymes known to be elevated in liver diseases were measured preoperatively and controlled on the first and fourth postoperative day in 14 children with chronic liver disease and anesthetised for liver biopsy using Ketamine (7 patients, group A) or other drugs (group B). Serum levels of gamma GT, Ph. Alk, direct and indirect bilirubin show neither significant variations nor differences between the two groups of patients. SGOT and SGPT basal levels were similar in the two groups and were rather two-fold the normal values. At the first postoperative da

Respiratory management of pediatric patient with bronchiolitis obliterans syndrome during general anesthesia surgery: a case report.

BACKGROUND: Bronchiolitis obliterans syndrome (BOS) is a rare but severe noninfectious pulmonary complication that typically arises in the context of chronic graft-versus-host disease (cGVHD) following allogeneic hematopoietic stem cell transplantation (HSCT). Characterized by progressive small airflow obstruction and irreversible airflow limitation, it poses significant challenges in managing general anesthesia, especially in pediatric patients. There is currently no established consensus or clinical research on the optimal anesthetic approach for such cases, making this report noteworthy. C
