Treatment of depressive disorders in primary care--protocol of a multiple treatment systematic review of randomized controlled trials.

BACKGROUND: Several systematic reviews have summarized the evidence for specific treatments of primary care patients suffering from depression. However, it is not possible to answer the question how the available treatment options compare with each other as review methods differ. We aim to systematically review and compare the available evidence for the effectiveness of pharmacological, psychological, and combined treatments for patients with depressive disorders in primary care. METHODS/DESIGN: To be included, studies have to be randomized trials comparing antidepressant medication (tricycli

Defining the bellwether procedures and processes for global trauma care: an international Delphi study.

BACKGROUND: The complexity of delivering trauma care makes the assessment of its provision challenging. The identification of bellwether procedures has previously been successful in the evaluation of global surgical care; however, any equivalent in assessing trauma care is currently lacking. Through a Delphi process, we aimed to produce the bellwether procedures and processes for global trauma care. METHODS: A global Delphi process was undertaken with healthcare professionals and academics involved in trauma care from across the world. A list of potential procedures and processes was identifi

Understanding and treating panic disorder in the primary care setting.

According to studies, the median prevalence of panic disorder in the primary care setting is 4%. Rates are higher among certain patient populations, such as those with cardiac (20% to 50%) or gastrointestinal presentations (28% to 40%). Consequently, patients with panic disorder are high utilizers of medical services and are heavily represented among patients classified as high health care utilizers, compared with other psychiatric or non-psychiatric groups. Despite its frequency in the primary care setting, panic disorder is significantly under-recognized by medical providers. Corresponding w

The Massachusetts General Hospital Vascular Center: emergence of a multidisciplinary program designed to improve quality outcomes for patients.

The prevalence of cardiovascular disease in the United States is extensive and growing rapidly. Contributing factors include the aging population, tobacco abuse, obesity, and diabetes, all of which increase the prevalence of cardiovascular diseases. Treatment paradigms for cardiovascular disease have also been rapidly evolving; minimally invasive, catheter-based approaches are increasingly used as first-line therapy. Physicians from a variety of specialties have responded to this growing patient population and treatment paradigm shift by enhancing their training and expanding their practices t

The national free delivery and caesarean policy in Senegal: evaluating process and outcomes.

This article presents the results of an evaluation of the free delivery and caesarean policy (FDCP) in Senegal. The policy was introduced into five poor regions in 2005 and in 2006 was extended at regional hospital level to all regions apart from the capital (Dakar). The evaluation was carried out in 2006-7. There were four research components, all focused on selected facilities and districts within the five FDCP regions: a financial analysis of expenditure on the policy and wider health financing in the five regions and nationally; 54 key informant interviews from national down to facility le

Using Six Sigma to improve once daily gentamicin dosing and therapeutic drug monitoring performance.

BACKGROUND: Safe, effective therapy with the antimicrobial gentamicin requires good practice in dose selection and monitoring of serum levels. Suboptimal therapy occurs with breakdown in the process of drug dosing, serum blood sampling, laboratory processing and level interpretation. Unintentional underdosing may result. This improvement effort aimed to optimise this process in an academic teaching hospital using Six Sigma process improvement methodology. METHODS: A multidisciplinary project team was formed. Process measures considered critical to quality were defined, and baseline practice w

Process evaluation of the randomized trial-"Care management for older people with cognitive impairment during and after hospitalisation in Germany (intersec-CM)".

BACKGROUND: Effective care of people with cognitive impairment and their families depends, among other things, on the cooperation of the professionals involved in their care. An effective dementia care management support model for people with dementia and their families for the outpatient sector has been further developed for ambulatory and hospital settings. The intersec-CM randomized controlled trial (RCT) implemented this intersectoral care management (iCM) model into routine care in three hospitals in Bielefeld and Greifswald (Germany). Care managers (CMs) trained in advance for the study

Outcomes of outpatient management of pediatric burns.

The literature surrounding pediatric burns has focused on inpatient management. The goal of this study is to characterize the population of burned children treated as outpatients and assess outcomes validating this method of burn care. A retrospective review of 953 patients treated the burn clinic and burn unit of a tertiary care center. Patient age, burn etiology, burn characteristics, burn mechanism, and referral pattern were recorded. The type of wound care and incidence of outcomes including subsequent hospital admission, infection, scarring, and surgery served as the primary outcome data.

A systematic review of outcomes of chronic disease self-management interventions.

PURPOSE: To carry out a systematic review of program outcomes used in the evaluation of group-based self-management interventions aimed at people with arthritis and other chronic conditions. METHODS: The systematic search was performed across databases MEDLINE, EMBASE, CINAHL, and PsycINFO. Both between-group and within-group effect sizes (ES) were calculated. Results were interpreted as small (ES ~ 0.2), medium (ES ~ 0.5), or large (ES ~ 0.8) effects. RESULTS: The majority of 18 included trials investigated the effectiveness of arthritis-specific interventions. Across most outcomes, small e

Educational disparities in health behaviors among patients with diabetes: the Translating Research Into Action for Diabetes (TRIAD) Study.

BACKGROUND: Our understanding of social disparities in diabetes-related health behaviors is incomplete. The purpose of this study was to determine if having less education is associated with poorer diabetes-related health behaviors. METHODS: This observational study was based on a cohort of 8,763 survey respondents drawn from ~180,000 patients with diabetes receiving care from 68 provider groups in ten managed care health plans across the United States. Self-reported survey data included individual educational attainment ("education") and five diabetes self-care behaviors among individuals fo

The base for exposure treatment in agoraphobia: some indicators for nurse therapists and community psychiatric nurses.

This study examines, using a randomised control trial, the question of the optimum base for exposure treatment with agoraphobic clients. The results have relevance for community psychiatric nurses and nurse behaviour therapists working with such problems and indeed for other behavioural practitioners. While the results confirm the already extensive evidence for the efficacy of exposure in vivo for agoraphobic clients, the results show that in general there is no difference between outcome of treatment delivered from the patient's own home and the out-patient clinic. There are, however, a small

Employing a Nutrition Nurse in Neonatal Intensive Care Unit Improved Nutrition and Growth Outcomes in Preterm Neonates.

BACKGROUND: Nutrition nurses are clinical nurse specialists with knowledge and experience in nutrition support. We aimed to investigate the nutrition and growth outcomes after employment of a nutrition nurse in our level III neonatal intensive care unit (NICU). METHODS: A retrospective cohort study including preterm neonates < 34 weeks and < 2000 g were conducted. Nutrition and growth outcomes of infants in pre-nutrition-nurse and post-nutrition-nurse periods were compared. Primary outcome was presence of postnatal growth retardation (PGR) at term-equivalent age (body weight < 10 percentile a

Evaluation of a Specialised Counselling Service for Perinatal Bereavement.

OBJECTIVES: We aimed to evaluate the outcomes of Petals: a charitable organisation in Cambridgeshire. Petals provides counselling for women and couples who have suffered perinatal bereavement, or trauma during pregnancy or birth. This paper attempts to evaluate the effect of counseling interventions at this difficult time. METHODS: Outcomes were recorded in 107 patients using the CORE (Clinical Outcomes in Routine Evaluation) system. CORE was developed to assess the effectiveness of psychological therapies. CORE-OM (CORE Outcome Measure) involves a questionnaire that assesses subjective well-

Telephone follow-up and self-performed urine pregnancy testing after early medical abortion: a service evaluation.

INTRODUCTION: Telephone follow-up with a self-performed low-sensitivity urine pregnancy (LSUP) test was introduced at the Royal Infirmary of Edinburgh, Scotland, as an alternative to routine ultrasonography for confirming successful abortion at 2 weeks following early medical abortion (<9 weeks' gestation). Women who screened 'positive' at telephone follow-up on the basis of ongoing pregnancy symptoms, scant bleeding or LSUP test result subsequently attended the clinic for a confirmatory ultrasound. METHODS: A service evaluation was conducted of the first 8 months of telephone follow-up consi

Five-year experience with an early discharge program in well newborns.

PURPOSE: This study was conducted to determine if early postnatal discharge (< or = 48 hrs; EDC) in well newborns had an effect on the rate of hospital readmission within the first week after hospital discharge when compared to infants who remained > 48 hrs after birth (LDC). METHODS: This was a retrospective medical chart review. Infants who were born at Norton Hospital in Louisville, Kentucky, between 1/1/94 and 12/31/98, discharged as well newborns and treated at Kosair Children's Hospital, Louisville, Kentucky, within 7 days of neonatal discharge, were eligible for review. Infection surveillance in home care: device-related incidence rates.

BACKGROUND: Four home health agencies of different sizes, including a rural agency with many branch offices, participated in a surveillance study. METHODS: These agencies used the same definitions for symptomatic urinary tract infections in patients with urinary catheters and for bloodstream infections in patients receiving intravenous therapy. Identical data and methods were used for calculating infection rates, with device-days consistently used as the denominator. Each agency's primary goal was to develop baseline information about its own infection rates and to use these statistics to det

Early discharge: outcomes of primiparas and their infants.

The effects of early hospital discharge on maternal and infant outcomes were studied by interviewing 21 primiparas choosing early discharge and 21 primiparas with hospital stays longer than 24 hours. Other than supervision of infants for jaundice, conditions requiring medical intervention were not linked to time of discharge. No significant differences occurred in intensity of concerns at one week postpartum. The most intense concerns of primiparas, regardless of length of stay, focused on maternal body image, infant care and behavior, and recognition of signs of illness. Women choosing early
