Technical and clinical performance of the thermo-test device "Q-Sense" to assess small fibre function: A head-to-head comparison with the "Thermal Sensory Analyzer" TSA in diabetic patients and healthy volunteers.

BACKGROUND: Thermo-test devices are rarely used outside specialized pain centres because of high acquisition costs. Recently, a new, portable device ("Q-Sense") was introduced, which is less expensive but has reduced cooling capacity (20°C). We assessed the reliability/validity of the "Q-Sense" by comparing it with the Thermal Sensory Analyzer (TSA). METHODS: Using a phantom-skin model, the physical characteristics of both devices were compared. The clinical performance was assessed in a multicentre study by performing Quantitative Sensory Testing (QST) in 121 healthy volunteers and 83 diabet

Persistent increased threshold of electrical stimulation selective to motor nerve in multifocal motor neuropathy.

In multifocal motor neuropathy (MMN) the threshold of electrical stimulation showed a persistent, marked increase for the motor nerve which decreased after treatment with intravenous immunoglobulin or oral cyclophosphamide; whereas, the threshold was normal for the sensory nerve. This discrepancy of the thresholds for motor and sensory nerves indicates that the increased threshold for motor nerve is not caused by change in perineural capacitance, such as subperi- and endoneural edema or perineural thickening. Inching studies showed that the site of the elevated motor nerve threshold was closel

Detection of feigned tactile sensory loss using a forced-choice test of tactile discrimination and other measures of tactile sensation.

OBJECTIVE: Intentional exaggeration of disability is a risk in work injuries but is hard to reliably detect clinically. This study examined the accuracy of tactile sensory threshold and forced-choice discrimination measures in detecting feigned sensory loss. METHODS: Participants (n = 80) were randomly assigned to one of four sensory loss groups: (1) none; (2) partial; (3) full; or (4) feigned. Sensory data were collected for the upper extremities. RESULTS: Tactile thresholds greater than 0.5 g, discriminability less than 0.50, or forced-choice scores less than 90% were associated with a ver

Large and small fibre type sensory dysfunction in patients with cancer.

Quantitative sensory testing was used to assess the prevalence of sensory dysfunction in patients with cancer, carefully screened for other risk factors for neuropathy. Large fibre type sensory function was evaluated using vibration threshold (VT) determinations while small fibre type sensory function was assessed by thermal threshold (TT) determinations. Mean VT and TT were significantly elevated in the toes but not the fingers of cancer patients. VT elevations in the toes occurred in 31% of cancer patients and in 6% of control subjects. TT elevations in the toes occurred in 43% of cancer pat

Psychophysical correlates in adults with sensory modulation disorder.

PURPOSE: Individuals with sensory modulation disorder (SMD) demonstrate abnormal responses to naturally occurring stimuli in a manner that interferes with daily life activities. This study is the first study applying quantitative sensory testing to characterize the somatosensory sensitivity of adults with SMD. METHOD: One hundred and fifty one adults (68 males and 83 females) were tested comparing 91 SMD to 60 SMD-free, control individuals. Group placement (SMD vs. SMD-free) was determined using the Sensory Responsiveness Questionnaire (SRQ). Sensory detection thresholds for skin warming, coo

Oral pain perception and taste in burning mouth syndrome.

BACKGROUND: Aim of this study was to investigate the effect of idiopathic burning mouth syndrome (BMS) on both, the pain perception within trigeminal lingual nerve distribution and gustatory sensitivity using capsaicin threshold test, and regional taste tests, respectively. METHODS: Pain thresholds for capsaicin were assessed using capsaicin-impregnated filter-paper strips. The strips were placed midline on the tongue tip for whole mouth testing with the mouth closed, and on the left or right edge of the extended anterior tongue for lateralized testing. Measures of gustatory function were obt

Distinct patterns of small-fiber dysfunction in anti-myelin-associated glycoprotein-positive and negative paraproteinemic neuropathies.

OBJECTIVE: IgM paraproteinemic neuropathies are traditionally considered large-fiber disorders, while small-fiber involvement remains poorly investigated. We assessed small-fiber dysfunction using thermal quantitative sensory testing in IgM paraproteinemic neuropathies, comparing anti-myelin-associated glycoprotein-positive and negative patients, and explored relationships with nerve conduction studies, disability, quality of life, and serum neurofilament light chain. METHODS: Thirty-nine patients (24 anti-myelin-associated glycoprotein-positive, 15 negative) and 20 healthy volunteers underwe

Could Sensory Differences Be a Sex-Indifferent Biomarker of Autism? Early Investigation Comparing Tactile Sensitivity Between Autistic Males and Females.

Sensory differences are highly prevalent in autistic individuals. However, few studies have compared their presentation between autistic males and autistic females. We used psychophysics to assess and compare tactile perceptual sensitivity between autistic and non-autistic boys and girls aged between 8 and 12 years of age. While there were sex-differences of amplitude discrimination, frequency discrimination and order judgement thresholds, these sex-differences were not autism-specific. Mean RTs and detection thresholds were elevated in autism but were comparable between the sexes. Tactile sen

Raynaud's phenomenon associated with the use of pneumatically powered surgical instruments.

Five patients developed symptoms of Raynaud's phenomenon and upper extremity paresthesias after 7-32 months of exposure to air-powered surgical instruments used for the harvest of bone for bone banks. Results of cold challenge plethysmography, nerve conduction studies, vibrotactile thresholds, and quantitative sensory testing were as follows: all patients had significant reproducible vasospasm with nondetectable finger systolic blood pressure (FSBP = 0) after local digital cooling; nerve conduction abnormalities included delayed median nerve sensory conduction (< 48 m/s) across 5 of 10 wrists;

Electrochemical Skin Conductance and Quantitative Sensory Testing on Fibromyalgia.

BACKGROUND: An impairment of the peripheral nervous system has been suggested in fibromyalgia (FM). Noninvasive distal electrochemical skin conductance (ESC) has been studied little so far when combined with quantitative sensory testing (QST) in patients with FM. METHODS: This study (clinicaltrials.gov NCT03347669) included 50 female patients with FM and 50 matched healthy volunteers (HVs). ESC (measured in microsiemens [µS] with Sudoscan), as well as psychological, quality of life, sleep, and social characteristics, were assessed in both groups. In a subgroup of 24 patients with FM and 24 HV

Vibrotactile difference thresholds: effects of vibration frequency, vibration magnitude, contact area, and body location.

It has not been established whether the smallest perceptible change in the intensity of vibrotactile stimuli depends on the somatosensory channel mediating the sensation. This study investigated intensity difference thresholds for vibration using contact conditions (different frequencies, magnitudes, contact areas, body locations) selected so that perception would be mediated by more than one psychophysical channel. It was hypothesized that difference thresholds mediated by the non-Pacinian I (NPI) channel and the Pacinian (P) channel would differ. Using two different contactors (1-mm diameter

Rectal mucosal electrosensory testing--evidence for a rectal sensory neuropathy in idiopathic constipation.

Rectal sensation is usually assessed using balloon distention. The authors describe a more precise technique that avoids the variables of balloon dynamics and rectal diameter and compliance. Using both methods, rectal sensation was assessed in 13 healthy control women and 26 women with severe idiopathic constipation. Balloon distention in the rectum revealed an elevated sensory threshold (16.9 +/- 4.4 vs. 30.4 +/- 3.1 ml air, controls vs. patients, P = 0.018) and volume required to elicit a call to stool (61.1 +/- 9.1 vs. 97.5 +/- 6.4, P = 0.003) in subjects with severe constipation. Posterior parietal cortex modulates perceptual decisions depending on psychotic phenotype.

BACKGROUND: Reduced data-gathering and altered sensory precision are associated with psychotic phenotypes in tasks engaging the posterior parietal cortex (PPC). We investigated whether PPC excitability - modulated via 1 Hz repetitive transcranial magnetic stimulation (TMS) - differentially affects these behavioural patterns in high vs. low psychotic phenotypes. Based on prior work, we hypothesised that delusional and hallucinatory traits would moderate TMS effects on sensory precision (proxied by drift rates), while hallucinatory traits would additionally moderate effects on decision threshold

Ulnar and Median Nerve Abnormalities in Carpal Tunnel Syndrome.

BACKGROUND: Nerve compression underlying carpal tunnel syndrome (CTS) results in an increase in the threshold of superficial sensation in the area supplied by the median nerve, which is a mixed nerve dominated by sensory fibres. The distribution of sensory symptoms is strongly dependent on the degree of electrophysiological dysfunction of the median nerve. The association between carpal tunnel syndrome and ulnar nerve entrapment at wrist level is still unclear. Patho-logical processes leading to median neuropathy in CTS may affect ulnar nerve motor and sensory fibers in the Guyon canal. This m

Characterization of evoked tactile sensation in forearm amputees with transcutaneous electrical nerve stimulation.

OBJECTIVE: The goal of this study is to characterize the phenomenon of evoked tactile sensation (ETS) on the stump skin of forearm amputees … voked both in the PFM of the stump skin of amputees and in the normal skin of able-bodied subjects. Sensory thresholds in the normal skin of able-bodied subjects were generally lower than those in the stump skin of forearm amputees, however, both were linearly modulated by current amplitude and pulse width. The variation of the MSPs in the PFM was confined to a smal …
