Sentinel node biopsy before neoadjuvant chemotherapy spares breast cancer patients axillary lymph node dissection.

BACKGROUND: Neoadjuvant chemotherapy in breast cancer patients is a valuable method to determine the efficacy of chemotherapy and potentially downsize the primary tumor, which facilitates breast-conserving therapy. In 18 studies published about sentinel node biopsy after neoadjuvant chemotherapy, the sentinel node was identified in on average 89%, and the false-negative rate was on average 10%. Because of these mediocre results, no author dares to omit axillary clearance just yet. In our institute, sentinel lymph node biopsy is performed before neoadjuvant chemotherapy. The aim of this study w

Follow-up of sentinel node negative breast cancer patients without axillary lymph node dissection.

BACKGROUND AND OBJECTIVES: The purpose of this study was to evaluate the feasibility of sentinel lymph node biopsy in breast cancer patients at our institution and to report the follow-up status of node-negative patients with removal of only the sentinel node. METHODS: A total of 247 breast cancer patients underwent sentinel node (SN) mapping between June of 1996 and September of 2000. The SN was identified by using a combination of vital blue dye and a radiolabeled colloid. RESULTS: A SN was identified in 227 of 247 patients (91.9%). One hundred forty-five were SN negative, 82 were SN posit

Sentinel NOSE: Prospective feasibility study on sentinel lymph node biopsy in bulky nasal vestibule cancer.

OBJECTIVE: Patients with bulky squamous cell carcinoma of the nasal vestibule are at high risk of nodal relapse and have worse prognosis, despite modern neck imaging. More sensitive assessment of the neck may obviate elective treatment and ultimately reduce the risk of nodal relapse and improve outcome. The goal of this study was to evaluate the feasibility, safety and tolerability of sentinel lymph node biopsy (SLNB) in staging of bulky squamous cell nasal vestibule carcinoma (NVC). METHODS: Ten patients with cT1-T2N0 squamous cell NVC with diameter ≥1.5 cm, planned for curative brachytherap

Non-sentinel lymph node metastases in breast cancer patients with a positive sentinel lymph node: validation of five nomograms and development of a new predictive model.

AIMS AND BACKGROUND: Discordance of intraoperative analysis with definitive histology of the sentinel lymph node in breast cancer leads to completion axillary lymph node dissection, which only in 35-50% shows additional nodal metastases. The aim of the study was to identify individual patient risk for non-sentinel lymph node metastases by validating several statistical methods present in the recent literature and by developing a new tool with the final goal of avoiding unnecessary completion axillary lymph node dissection. METHODS: We retrospectively evaluated 593 primary breast cancer patien

Prognostic Value of Non-nodal Regional Metastases in Predicting Sentinel Lymph Node Status in Cutaneous Melanoma: Multicenter Analysis of the Sentinel Lymph Node Working Group Database.

BACKGROUND: Non-nodal regional metastases, including microsatellite lesions, satellites, and in-transit metastases, are an uncommon but aggressive entity in cutaneous melanoma. We evaluated their association with clinicopathological features, sentinel lymph node (SLN) status, and prognosis. PATIENTS AND METHODS: The Sentinel Lymph Node Working Group (SLNWG) database was used to examine the clinicopathological associations of non-nodal regional metastases, their prognostic significance in relation to SLN status, and their impact on clinical outcomes, including in patients with negative SLN sta

Risk of Residual Axillary Lymph Node Macrometastasis in Early Breast Cancer PATIENTS with One Positive Macrometastasis Sentinel Lymph Node.

OBJECTIVE: To investigate the risk factors for residual axillary lymph node macro-metastasis in early-stage breast cancer patients with a single macrometastasis sentinel lymph node (SLN). METHODS: We retrospectively analyzed the clinical data of 119 breast cancer patients diagnosed between January 2018 and September 2023, each with one positive SLN stained with methylene blue, who subsequently underwent axillary lymph node dissection. The patients were divided into two groups based on the total number of SLNs identified: fewer than three and more than three. Fisher's exact test was used for s

Sentinel lymph node biopsy in early-stage oral squamous cell carcinoma: clinical outcomes and optimal lymph node removal thresholds.

BACKGROUND: Sentinel lymph node biopsy (SLNB) is an effective method for managing clinically node-negative (cN0) early-stage oral squamous cell carcinoma (OSCC). This study examines lymphatic drainage patterns and clinical outcomes in pT1/T2 OSCC patients undergoing SLNB while establishing optimal thresholds for SLN identification and lymph node removal. METHODS: Clinicopathological data were analyzed to assess associations between SLN localization and tumor characteristics. Logistic regression identified risk factors for contralateral SLN involvement. Receiver operating characteristic curves

Multiple Lymph Node Basin Drainage in Trunk Melanoma Is Not Associated with Survival of Sentinel Lymph Node-Positive Patients.

OBJECTIVES: This study was aimed at investigating the prognostic role of multiple lymph node basin drainage (MLBD) in patients with positive sentinel lymph node (SLN) biopsy. BACKGROUND: MLBD is frequently observed in patients with trunk melanoma undergoing SLN. The prognostic value of MLBD in SLN-positive patients is still debated. METHODS: Retrospective data from 312 trunk melanoma patients with positive SLN biopsy (1991-2012) at 6 Italian referral centres were gathered in a multicentre database. MLBD was defined at preoperative lymphoscintigraphy. Clinical and pathological data were analy

Comparative morbidity of axillary lymph node dissection and the sentinel lymph node technique: implications for patients with breast cancer.

OBJECTIVE: To assess our long-term complications from complete axillary lymph node dissection (AXLND) in patients with breast cancer. SUMMARY BACKGROUND DATA: Complete AXLND as part of the surgical therapy for breast cancer has come under increased scrutiny due the use of the sentinel lymph node (SLN) biopsy technique to assess the status of the axillary nodes. As the enthusiasm for the SLN technique has increased, our impression has been that the perceived complication rate from AXLND has increased dramatically while the negative aspects of the SLN technique have been underemphasized. Occult breast cancer management with axillary lymph node dissection versus sentinel lymph node biopsy: Case reports and literature review.

BACKGROUND: Breast cancer is the most common cancer worldwide. In patients with occult breast cancer, sentinel lymph node biopsy (SLNB) has been shown to reduce complications, shorter hospital stay, and improved quality of life compared to axillary lymph node dissection (ALND). This study presents two case reports comparing outcomes of ALND versus SLNB in the management of occult breast cancer. CASE 1: A 52-year-old female presented with a left axillary tail mass which showed invasive ductal carcinoma. Underwent left axillary lymph node dissection. She developed postoperative drain seroma. Single-cell profile of tumor and immune cells in primary breast cancer, sentinel lymph node, and metastatic lymph node.

PURPOSE: Little is known about the host-tumor interaction in the lymph-node basin at a single cell level. This study examines single cell sequences in breast cancer nodal metastases of a patient with triple-negative breast cancer. METHODS: The primary breast tumor, sentinel lymph node, an adjacent lymph node with metastatic involvement and a clinically normal-appearing lymph node were collected during surgery. Single-cell sequencing was performed on all four specimens. RESULTS: 14,016 cells were clustered into 6 cell subpopulations. Cancer cells demonstrated the molecular characteristics of

Imaging features of sentinel lymph node mapped by multidetector-row computed tomography lymphography in predicting axillary lymph node metastasis.

INTRODUCTION: Accurately assessing axillary lymph node (ALN) status in breast cancer is vital for clinical decision making and prognosis. The purpose of this study was to evaluate the predictive value of sentinel lymph node (SLN) mapped by multidetector-row computed tomography lymphography (MDCT-LG) for ALN metastasis in breast cancer patients. METHODS: 112 patients with breast cancer who underwent preoperative MDCT-LG examination were included in the study. Long-axis diameter, short-axis diameter, ratio of long-/short-axis and cortical thickness were measured. Logistic regression analysis wa

Primary excision margins, sentinel lymph node biopsy, and completion lymph node dissection in cutaneous melanoma: a clinical practice guideline.

BACKGROUND: For patients who are diagnosed with early-stage cutaneous melanoma, the principal therapy is wide surgical excision of the primary tumour and assessment of lymph nodes. The purpose of the present guideline was to update the 2010 Cancer Care Ontario guideline on wide local excision margins and sentinel lymph node biopsy (slnb), including treatment of the positive sentinel node, for melanomas of the trunk, extremities, and head and neck. METHODS: Using Ovid, the medline and embase electronic databases were systematically searched for systematic reviews and primary literature evaluat

200 Sentinel lymph node biopsies without axillary lymph node dissection -- no axillary recurrences after a 3-year follow-up.

The aim of this study is to evaluate the rate of axillary recurrences in sentinel lymph node (SLN)-negative breast cancer patients after sentinel lymph node biopsy (SLNB) alone without further axillary lymph node dissection (ALND). Between May 1999 and February 2002, 333 consecutive patients with primary invasive breast cancer up to 4 cm and clinically negative axillae were entered into this prospective study. Sentinel lymph nodes were identified using the combined method with blue dye (Patent blue V) and technetium 99m-labelled albumin (Nanocoll). Sentinel lymph nodes were examined by frozen

Pathologic features associated with nonsentinel lymph node metastases in patients with metastatic breast carcinoma in a sentinel lymph node.

BACKGROUND: To the authors' knowledge it has not yet been determined which patients with primary breast carcinoma and an axillary sentinel lymph node (SN) metastasis have additional metastases in nonsentinel lymph nodes. METHODS: Pathologic features of the primary breast carcinoma and its SN metastasis were examined in 194 patients and correlated with the tumor status of the non-SNs in the same axillary basin. Two-level cytokeratin immunohistochemistry was applied to the SNs and to non-SNs of cases that were negative by standard hematoxylin and eosin examination. RESULTS: Lymph node staging

Pathologic review of negative sentinel lymph nodes in melanoma patients with regional recurrence: a clinicopathologic study of 1152 patients undergoing sentinel lymph node biopsy.

A sentinel lymph node (SLN) that is melanoma negative by pathologic examination implies absence of melanoma metastasis to that regional lymph node field. However, a small proportion of patients develop regional node field recurrence after a negative SLN biopsy. In this study, we reviewed the histopathology of negative SLNs from such patients to determine whether occult melanoma cells were present in the SLNs, to characterize the pathologic features of false-negative SLNs, and to provide recommendations for the histopathologic examination of these specimens. Between March 1992 and June 2001, of
