An easier Lichtenstein hernioplasty.

BACKGROUND: The Lichtenstein technique is used worldwide because of its rapid learning curve, good results and low recurrence rates. In this approach, a lateral tie-shaped incision is made on the mesh, involving the spermatic funiculus and thus correcting the deep inguinal ring defect by narrowing it. The lateral tie, however, does not prevent the wrinkling of the upper portion of the mesh that is caused by the arched shape of the inguinal ligament, where the lower edge of the prosthesis is sutured. OBJECTIVE: To describe a variation of the Lichtenstein technique that is easy to perform, main

Outcomes of Retromuscular Porcine Biologic Mesh Repairs Using Transversus Abdominis Release Reconstruction.

BACKGROUND: Optimal mesh reinforcement and operative technique for major abdominal wall reconstructions (AWR) remain debatable. Posterior component separation via transversus abdominis release (TAR) allows for wide sublay mesh reinforcement with durable reconstruction, and has been gaining popularity in recent years. Although biologic mesh has been associated with mixed results, outcomes of AWR with bioprosthetics have not been well elucidated to date. We evaluated our outcomes of TAR reconstructions with retromuscular porcine biologic mesh reinforcement. STUDY DESIGN: Consecutive patients un

Laparoscopic management of infected mesh after laparoscopic inguinal hernia repair.

PURPOSE: We present our experience with 10 patients with infected meshes after laparoscopic inguinal hernia repair in whom we explanted infected meshes laparoscopically. METHODS: On retrospective analysis over 5 years (2007 to 2012), we identified 10 patients (6 TAPP/4 TEP) with localized deep-seated mesh infections in whom infected meshes were explanted laparoscopically. Peritoneum was incised, associated abscesses were drained, meshes were identified, separated, and extracted through 10/12 mm port. RESULTS: Nine patients experienced resolution of symptoms after 3 weeks of surgical interven

Enterocervical Fistula Six Weeks After Laparoscopic Mesh Sacral Colposuspension: A Case Report.

Laparoscopic colposuspension with mesh is a common procedure performed to treat apical pelvic organ prolapse refractory to non-surgical interventions. Mesh complications involving a fistula are an uncommon event that typically occurs months or years later. We report a case complicated by enterocervical fistula that occurred six weeks after mesh placement. This was the case of a 67-year-old female with no surgical history who underwent a laparoscopic supracervical hysterectomy, bilateral salpingo-oopherectomy, laparoscopic colposuspension with mesh, and midurethral sling for treatment of stage

Recurrent vaginocutaneous fistula 9 years after partial removal of a 16-year-old transobturator tape: a case report.

This case report describes an exceptionally late mesh-related complication occurring 16 years after transobturator tape placement for stress urinary incontinence. The patient developed a recurrent vaginocutaneous fistula and abscess 9 years after partial mesh removal, highlighting the long-term risks associated with retained synthetic material. Clinical presentation included persistent purulent discharge and inguinal abscess, with MRI confirming infection of the remaining sling arm and fistulization to the thigh. Complete surgical excision led to full recovery. This case emphasizes the potenti

Three-Dimensional Printed Polycaprolactone Mesh in Pediatric Cranial Vault Remodeling Surgery.

BACKGROUND: The surgical management of craniosynostosis has greatly evolved with improvements in both technology and understanding of the disease process. Some drawbacks remain regarding bone regeneration within the surgical bony gaps. Generally, bony gaps improve in the 12 to 24 months after surgery, but some gaps may remain for longer and cause deformity and/or require additional bony reconstruction. These considerations make tissue-engineered bone very attractive. Novel 3-dimensional printed bioresorbable mesh implants made of Polycaprolactone (PCL) can be used to fill the surgical bony def

Mesh migration into esophagogastric junction after laparoscopic hiatal hernia repair; how to prevent it? A case report.

Although the use of mesh reinforcement during large hiatal hernia repair may reduce the rate of recurrence, various mesh-related complications have been reported. A 65-year-old woman presented with dysphagia. The patient was diagnosed with a large hiatal hernia and treated with laparoscopic fundoplication and Collis gastroplasty with mesh repair. Six months after surgery, the patient presented with dysphagia and vomiting. Esophagogastroduodenoscopy showed migration of mesh material into the esophagogastric junction. We performed a proximal gastrectomy with mesh removal. The patient was dischar

Incidence of tissue-based inguinal hernia repair in Belgium: a web-based national survey.

BACKGROUND: Inguinal hernia repair is one of the most commonly performed procedures worldwide. Several different methods for repair exist, where the preferred surgical procedure for a non-complicated unilateral inguinal hernia is still up for debate. Originally described as a tissue repair, the introduction of prostheses have led many surgeons to move away from this type of repairs. However, recently updated guidelines still recommend non-mesh treatment, in a subgroup of patients, with a preference for the Shouldice technique. Therefore, we set out to map the incidence and knowledge about tiss

Retrospective evaluation of a funnel-shaped mesh for the prevention of parastomal hernias in patients with permanent end colostomy.

BACKGROUND: Parastomal hernia (PSH) is the most common long-term complication after abdominoperineal resection (APR) with permanent end colostomy. Although prophylactic mesh placement has been suggested to reduce the rate of PSH, the optimal mesh type and surgical technique remain unclear. Recently, three-dimensional funnel-shaped meshes have been introduced to stabilize the bowel loop and minimize stoma-related mechanical stress, potentially reducing the incidence of PSH. METHODS: This retrospective cohort study, reported in accordance with STROBE guidelines, included consecutive patients who

Amniotic Membrane Graft in the Management of Complex Vaginal Mesh Erosion.

Vaginal mesh erosion is a devastating complication after pelvic floor mesh surgery and it can be treated conservatively or with surgical revision. However, the management options following a failed primary revision or complex vaginal erosions are very limited. The aim of this study is to describe a novel treatment using an amniotic membrane as an inlay graft for such patients. Eight patients who failed conservative or primary surgical revision were enrolled. The complex erosions included vaginal agglutination, multiple vaginal erosions, recurrent erosions, and mesh cutting through the urethra.
