Diagnosis and treatment for ocular tuberculosis among uveitis specialists: the international perspective.

PURPOSE: To assess the approach of international specialists, who primarily practice in tuberculosis-endemic areas, to ocular tuberculosis (TB). METHODS: International experts from India, Brazil, Taiwan, and more than 10 other countries were surveyed using two clinical cases and general questions. RESULTS: A total of 244 experts were sent a survey about the treatment and diagnosis of ocular tuberculosis; 65 responded (27%), of whom 34 were affiliated with practices in India, while 31 primarily practice at international sites outside of India and North America. The data from this survey were

Lack of consensus in the diagnosis and treatment for ocular tuberculosis among uveitis specialists.

PURPOSE: To assess the approach of specialists to ocular tuberculosis (TB). METHODS: The American Uveitis Society (AUS) Listserv was surveyed using two clinical cases and general questions. RESULTS: Of 196 members, 87 responded (44.4%), of whom 64 were affiliated with practices in North America, while 23 were outside of North America. The survey provided normative data on how physicians evaluate patients with uveitis as well as opinions about ocular TB. Responses varied widely on such issues as (1) the pretest probability that a patient with granulomatous panuveitis had TB uveitis (range 1-7

Degree, duration, and causes of visual impairment in eyes affected with ocular tuberculosis.

BACKGROUND: Ocular tuberculosis (TB) can affect nearly every ocular tissue, leading to a variety of vision-threatening clinical manifestations. The goal of this study is to estimate the degree, duration, and causes of visual impairment in eyes affected by ocular TB. RESULTS: This was a retrospective study of patients diagnosed as ocular TB based on polymerase chain reaction (PCR) for Mycobacterium tuberculosis complex. We applied the World Health Organization definition of visual impairment (VI) to affected eye(s), instead of better-seeing eye. Best-corrected visual acuity (BCVA) of <6/18 and

A case series of symptomatic ocular tuberculosis and the response to anti-tubercular therapy.

OBJECTIVE: To identify the clinical pattern of symptomatic ocular tuberculosis (OTB) and the outcome following antitubercular therapy (ATT) in an endemic setting. DESIGN: A descriptive case series. SETTING: Tuberculosis and ophthalmology clinics, Teaching Hospital, Kandy, Sri Lanka. METHOD: We followed up patients with OTB on standard six-month regimen of ATT, from January 2006 to December 2008. Serial opthalmological assessment were done at the beginning and end of therapy and in each clinic visit. Relevant investigations were performed. Objective improvement of visual acuity was considere

Ocular disorders in adult patients with tuberculosis in a tertiary care hospital in Nigeria.

OBJECTIVES: To determine the ocular disorders in adult tuberculosis (TB) patients in Benin City, Nigeria. METHODS: A prospective study of adult TB patients presenting at the University of Benin Teaching Hospital, Benin City, Nigeria, between March 2006 and October 2006 was undertaken. The patients were interviewed and examined by the authors and the ocular findings recorded. RESULTS: There were 92 patients (45 males and 47 females) with mean age 37.9 years (SD±15.6). Only one (1.1 percent) was HIV positive. Among the ocular findings in patients with TB, 8 patients had monocular blindness tha

[Pediatric ocular tuberculosis: Epidemiological and clinical profile and particular issues regarding treatment].

INTRODUCTION: Tuberculosis remains the third leading cause of infectious disease mortality worldwide. Ocular tuberculosis, though rare, is on the rise, particularly in relation to the HIV/AIDS epidemic. In children, this condition presents a diagnostic challenge due to its clinical polymorphism and the frequent absence of lesions that are accessible for histologic examination. PURPOSE: To study the epidemiological, clinical, and therapeutic profiles of ocular tuberculosis in children through a case series. METHODS: This retrospective study includes 78 cases of ocular tuberculosis in HIV-nega

Long-term close follow-up of chorioretinal lesions in presumed ocular tuberculosis.

PURPOSE: To evaluate the long-term outcome (up to 7 years) of presumed ocular tuberculosis (TB) when the therapeutic decision was based on WHO guidelines. METHODS: Twelve out of 654 new uveitic patients (1998-2004) presented with choroiditis and positive tuberculosis skin test (TST) (skin lesion diameter >15 mm). Therapy was administered according to WHO recommendations after ophthalmic and systemic investigation. The area size of ocular lesions at presentation and after therapy, measured on fluorescein and indocyanine green angiographies, was considered the primary outcome. Relapse of choroi

Chasing shadows: case series of six posterior segment manifestations of ocular tuberculosis.

BACKGROUND: Ocular tuberculosis (OTB) is difficult to confirm microbiologically, so diagnosis often rests on characteristic posterior segment phenotypes supported by systemic immunologic evidence and therapeutic response. This case series from a high-prevalence setting illustrates the heterogeneity of posterior OTB and provides practical diagnostic anchors, treatment approaches, and complication surveillance that are directly applicable to routine uveitis care. CASE DESCRIPTION: Six immunocompetent males (28-46 years) presented with posterior disease entities that included: choroidal granulom

Systemic prime exacerbates the ocular immune response to heat-killed Mycobacterium tuberculosis.

Post-infectious uveitis describes the condition of chronic immune mediated ocular inflammation associated with pathogens such as Mycobacterium tuberculosis (Mtb). Mtb associated post-infectious uveitis can be modeled in mice by intravitreal injection of heat-killed Mtb (HKMtb). To better understand how prior systemic exposure to the pathogen alters the local immune response to Mtb, we used flow cytometry and multiplex ELISAs to compare ocular responses to intravitreal HKMtb in the presence or absence of a systemic "prime" of HKMtb. Priming resulted in exacerbation of local inflammation with si

The Diverse Spectrum of Presumed Ocular Tuberculosis Including Optic Neuropathy in a Tuberculosis-Endemic Region: A Case Series from India.

Ocular tuberculosis (TB) presents with diverse clinical manifestations and remains a diagnostic and therapeutic challenge, particularly in TB-endemic regions. In addition, antitubercular therapy (ATT), especially ethambutol and linezolid, may contribute to treatment-related optic neuropathy and visual morbidity. This case series aimed to describe the clinical manifestations of presumed ocular TB and to assess the burden of ATT-induced optic neuropathy during the study duration. Diagnosis was based on clinical features, tuberculin skin test (Mantoux test) or interferon-gamma release assay (e.g.

Sustained Delivery of Rifampicin Nanoformulation Administration Intravitreally Into Rabbit Eyes for Ocular Tuberculosis.

INTRODUCTION: Diagnosis and treatment of ocular tuberculosis is very challenging. It poses a significant and potential management dilemma after diagnosis as a primary, active, or secondary infection. The higher amounts of orally administered antitubercular drugs are needed to achieve the therapeutic concentration in the eye, which may lead to a higher risk of side effects. However, the intravitreal administration of drugs is not practiced because of the frequent administration of the injections. METHODS: This study was carried out to develop, optimize, and characterize rifampicin-loaded poly

Case Report: Ocular Tissue Diagnosis of Previously Undiagnosed, Extensively Drug-Resistant Pulmonary Tuberculosis.

We describe a patient with concurrent ocular and pulmonary tuberculosis (TB) in whom the diagnosis of extensively drug-resistant TB was made through phenotypic drug-sensitivity testing of an ocular fluid sample after sputum testing yielded incomplete results. Our results are remarkable, because culture-based diagnosis of TB in ocular fluid is unusual. We not only overcame this limitation, but also were able to create a complete drug-sensitivity testing profile from ocular samples, which led to effecting appropriate therapy for the patient.

Nodular posterior scleritis associated with presumed ocular tuberculosis: A multimodal imaging case report.

PURPOSE: To describe a patient with nodular posterior scleritis associated with presumed ocular tuberculosis (TB). OBSERVATIONS: A 25-year-old Caucasian man reported metamorphopsia in the right eye (OD). He had lost the vision in his left eye when he was 15 years old. The visual acuity (VA) levels were 20/20 and light perception in the right and left eyes, respectively. Wide-field color fundus photography, fluorescein and indocyanine green angiography, optical coherence tomography, and ultrasound suggested an inflammatory condition associated with an elevated lesion in the choroid/sclera. The
