Leveraging large scale deep learning models for diagnosis and visual outcome prediction in retinitis pigmentosa.

Retinitis pigmentosa (RP) is an inherited progressive retinal degeneration that shows symptoms of night blindness, visual field loss, declining of vision and eventually, blindness. Currently, gene therapy and retinal prosthesis are available, but the indication for these treatments is limited. In this study, we report on the development of a diagnostic and prognostic model for RP based on large-scale deep learning (DL) models pre-trained with fundus images. The EfficientNetB4 model performed best in diagnosing RP with an AUC of 0.94. The diagnosis of RP with this model is superior in cases wit

Convolutional neural network classifies visual stimuli from cortical response recorded with wide-field imaging in mice.

Objective. The optic nerve is a good location for a visual neuroprosthesis. It can be targeted when a subject cannot receive a retinal prosthesis and it is less invasive than a cortical implant. The effectiveness of an electrical neuroprosthesis depends on the combination of the stimulation parameters which must be optimized, and an optimization strategy might be performing closed-loop stimulation using the evoked cortical response as feedback. However, it is necessary to identify target cortical activation patterns and to associate the cortical activity with the visual stimuli present in the v

Comparison of perioperative analgesia between posterior stabilized and posterior cruciate retaining prostheses following total knee arthroplasty.

BACKGROUND: Posterior-stabilized (PS) and posterior cruciate retaining (CR) prostheses constitute the most widely used knee prostheses. This study aimed to determine the effects of PS and CR prostheses on postoperative analgesia following total knee arthroplasty (TKA) and provide a reliable clinical basis for optimizing postoperative analgesic effects. METHODS: Overall, 120 consecutive patients were enrolled in this study and classified into PS (n = 60) and CR (n = 60) groups according to the type of prosthesis used. All patients underwent TKA along with spinal anesthesia. All patients receiv

Discrete Tactile Feedback Based on Weber's Law Enhances Prosthetic Hand Approaching Performance Under Divided Visual Attention.

In multiple vision-demanding tasks, accurately controlling a prosthetic hand to approach a target object is particularly challenging for amputees, as visual attention diverted by other tasks forces them to rely heavily on peripheral vision. This study aims to initially validate that functionally effective sensory feedback can enhance the control of prosthetic hands during object approach under divided visual attention. To quantify prosthesis users' ability to approach and manipulate objects using central and peripheral vision in real-life scenarios, we conducted two experimental tasks-APPROACH

The role of nasendoscopy in the fabrication of a palatopharyngeal obturator - a case report.

Defects confined to the hard palate can be managed with relative ease because it is a static shelf creating oro- nasal separation. Since the soft palate is a dynamic separator between the oral and nasal cavities, defects involving the soft palate require careful consideration. Instrumental visualization methods can aid the fabrication of obturators in patients with such defects and prevent problems of under or over obturation. This case report presents the prosthodontic management of a patient with a Veau class II cleft palate using a palatopharyngeal obturator in order to re-establish normal

Comparison of short-term clinical efficacy between shoulder arthroplasty and plate fixation combined with bone cement augmentation for pathological fractures of the proximal humerus caused by metastatic tumors: a retrospective study of 40 cases.

BACKGROUND: The proximal humerus is an easily overlooked site for metastatic bone tumors, and pathological fractures here can cause severe pain and functional impairment. The selection of surgical regimens should take into account the location of bone metastases, the patient's physical status, and other relevant factors. Therefore, the optimal surgical strategy remains controversial. This study aimed to investigate and compare the clinical efficacy of two surgical approaches-tumor resection combined with prosthesis implantation versus plate fixation combined with bone cement augmentation-in pa

Amorphous silicon resistors enable smaller pixels in photovoltaic retinal prosthesis.

Objective.Clinical trials of the photovoltaic subretinal prosthesis PRIMA demonstrated feasibility of prosthetic central vision with resolution matching its 100µm pixel width. To improve prosthetic acuity further, pixel size should be decreased. However, there are multiple challenges, one of which is related to accommodating a compact shunt resistor within each pixel that discharges the electrodes between stimulation pulses and helps increase the contrast of the electric field pattern. Unfortunately, standard materials used in integrated circuit resistors do not match the resistivity required

Cellular migration into a subretinal honeycomb-shaped prosthesis for high-resolution prosthetic vision.

In patients blinded by geographic atrophy, a subretinal photovoltaic implant with 100 µm pixels provided visual acuity closely matching the pixel pitch. However, such flat bipolar pixels cannot be scaled below 75 µm, limiting the attainable visual acuity. This limitation can be overcome by shaping the electric field with 3-dimensional (3-D) electrodes. In particular, elevating the return electrode on top of the honeycomb-shaped vertical walls surrounding each pixel extends the electric field vertically and decouples its penetration into tissue from the pixel width. This approach relies on migr

Leaflet escape in an Edwards TEKNA mitral prosthesis.

In almost every type of artificial valve, structural failure has been described. We are reporting on a case of a sudden leaflet escape of an Edwards TEKNA mitral valve prosthesis 12 years after implantation. The patient had a sudden onset of dyspnea and severe pulmonary edema with subsequent cardiogenic shock. An emergency mitral replacement was successfully performed. A multi-detector computed tomography scanning and three-dimensional imaging showed two fragments that had embolized in the terminal aorta and the left common iliac artery. The patient presented visual field abnormality, and post

Evaluation of the Long-Term Clinical Results of 3 Patients Implanted with the Argus II Retinal Prosthesis.

This study presents the long-term clinical results of Argus II retinal prosthesis implantation in eyes with light perception and projection in 3 patients with end-stage retinitis pigmentosa. No conjunctival erosion, hypotony, or implant displacement was observed during postoperative follow-up. The electrical threshold values were lower in the macular region and higher close to the tack fixation region and peripherally. Optical coherence tomography scans showed fibrosis and retinoschisis formation at the retina-implant interface in two cases. This was attributed to mechanical and electrical eff

Mechanical modifications of soft actuators for the use as a dynamic iris implant.

Aniridia is a condition characterized by defects or absence of the iris. Since the eyes are a central point of attention in the human face, these deformities are often covered with cosmetic implants. However, patients suffer from the static pupil diameter of these implants, resulting in high light sensitivity or inadequate night vision. Therefore, we present a functional iris implant based on dielectric elastomer actuators. These electric drives are characterized by a silent and continuous adaptation as well as a small construction volume and a low heat emission. Since they normally exhibit in

Harmonization of Outcomes and Vision Endpoints in Vision Restoration Trials: Recommendations from the International HOVER Taskforce.

Translational research in vision prosthetics, gene therapy, optogenetics, stem cell and other forms of transplantation, and sensory substitution is creating new therapeutic options for patients with neural forms of blindness. The technical challenges faced by each of these disciplines differ considerably, but they all face the same challenge of how to assess vision in patients with ultra-low vision (ULV), who will be the earliest subjects to receive new therapies. Historically, there were few tests to assess vision in ULV patients. In the 1990s, the field of visual prosthetics expanded rapidly

An update on retinal prostheses.

Retinal prostheses are designed to restore a basic sense of sight to people with profound vision loss. They require a relatively intact posterior visual pathway (optic nerve, lateral geniculate nucleus and visual cortex). Retinal implants are options for people with severe stages of retinal degenerative disease such as retinitis pigmentosa and age-related macular degeneration. There have now been three regulatory-approved retinal prostheses. Over five hundred patients have been implanted globally over the past 15 years. Devices generally provide an improved ability to localize high-contrast ob
