Oil and petrol drum explosions. Injuries and casualties by exploding oil and petrol drums containing various inflammable liquids.

If ignited, an evaporated inflammable liquid remaining mixed with air in an oil or petrol drum may cause an explosion in which the top and bottom of the drum are blown off by the blast and act as projectiles causing extensive injuries to persons nearby. To analyse the occurrence of this type of accident and to study the injuries involved information was sought from all police districts in Denmark and all the departments of plastic surgery with a burns unit. The investigation revealed a total of 21 accidents caused by oil drum explosions over a period of 36 years with 16 injured within the last

Avascular necrosis of the femoral head following trochanteric fractures in adults: a systematic review.

INTRODUCTION: Avascular necrosis of the femoral head (ANFH) following trochanteric fractures (TFx) is infrequent. The causal relationship between ANFH and TFx remains controversial. Although several major risk factors for ANFH have been proposed, most of them remain under discussion. In this study we undertook a systematic review of the literature to investigate the incidence of AVN, risk factors and outcomes following Tfx fixation. MATERIALS AND METHODS: A comprehensive review of the literature was undertaken using the PRISMA guidelines with no language restriction. Case reports of ANFH and

Articular congruity is associated with short-term clinical outcomes of operatively treated SER IV ankle fractures.

BACKGROUND: With regard to supination-external rotation type-IV (SER IV) ankle fractures, there is no consensus regarding which patient, injury, and treatment variables most strongly influence clinical outcome. The purpose of this investigation was to examine the impact of articular surface congruity on the functional outcomes of operatively treatment of SER IV ankle fractures. METHODS: A prospectively generated database consisting of operatively treated SER IV ankle fractures was reviewed. Postoperative computed tomography (CT) scans were used to assess ankle joint congruity. Ankles were con

Intramedullary Nail Failure in a Subtrochanteric Fracture in a 62-Year-Old Woman.

Subtrochanteric fractures in older patients are typically due to low-energy falls. The standard of care is intramedullary nailing. The Smith & Nephew Trigen Intertan (Memphis, TN, US) is an intramedullary nail with a novel design that incorporates two integrated compression screws. We present a case of intramedullary nail failure in a patient with a subtrochanteric fracture. A 62-year-old woman with osteoporosis, a 40-year history of smoking, and consumption of two to three alcoholic beverages daily, weighing 177 pounds and five feet two inches tall (BMI 32.4), experienced a low-energy fall in

Peripartum Hysterectomy Outcomes: Our Eleven-Year Experience at a Tertiary Care Hospital.

INTRODUCTION: There has always been a debate regarding the fixation strategy of transverse acetabular fractures. We hypothesized that fixation strength of both column screws would be sufficient to withstand the physiological load across hip joint, resulting in similar functional and radiological outcomes with lower morbidity than plate and column screw combinations in transverse acetabular fractures. MATERIAL AND METHODS: A total of 44 transverse acetabular fracture patients without significant comminution were divided them into three groups based on the type of treatment received, viz., grou

Distal Femoral Replacement as a Primary Treatment Method for Distal Femoral Fractures in the Elderly.

Distal femoral fractures account for 3-6% of all femoral fractures with a similar demographic as patients suffering from proximal femoral fractures. The mortality risk can be high in such injuries, which has prompted NHS England to extend the scope of the Best Practice Tariff to include all fragility fractures of the femur. Poor bone quality, intra-articular extension, and significant comminution can make these fractures difficult to manage with fixation techniques, while early mobilisation is a key outcome in the treatment of this injury. In this study, a comprehensive literature search was p

Interobserver reliability of coronoid fracture classification: two-dimensional versus three-dimensional computed tomography.

PURPOSE: This study tests the hypothesis that 3-dimensional computed tomography (CT) reconstructions improve interobserver agreement on classification and treatment of coronoid fractures compared with 2-dimensional CT. METHODS: A total of 29 orthopedic surgeons evaluated 10 coronoid fractures on 2 occasions (first with radiographs and 2-dimensional CT and then with radiographs and 3-dimensional CT), separated by a minimum of 2 weeks. Surgeons classified fractures according to the classifications of Regan and Morrey and of O'Driscoll et al., identified specific characteristics, recommended the

Outcomes of AO/OTA 43C Pilon Fractures Treated With Open Reduction Internal Fixation or Tibiotalocalcaneal Nailing.

BACKGROUND: AO/OTA 43C pilon fractures are severe intra-articular distal tibia injuries associated with high complication rates. Although open reduction internal fixation (ORIF) remains the standard treatment, tibiotalocalcaneal (TTC) nailing has been used in select patients; this study compared characteristics and outcomes of patients treated with ORIF or TTC nailing. METHODS: A retrospective cohort study was performed of adult patients (≥18 years) with AO/OTA 43C pilon fractures and ≥90 days of follow-up treated operatively at a single level 1 trauma center between 2020 and 2025. Patients t

High rate of avascular necrosis but excellent patient-reported outcomes after open reduction and internal fixation (ORIF) of proximal humerus fracture dislocations: should ORIF be considered as primary treatment?

BACKGROUND: Proximal humerus fracture dislocations, excluding 2-part greater tuberosity fracture dislocations, are rare injuries. Outcomes after open reduction and internal fixation (ORIF) of these injuries have not been well described in the literature. The purpose of this study was to report the radiographic and functional outcomes of patients who underwent ORIF of a proximal humerus fracture dislocation. METHODS: All skeletally mature patients who underwent ORIF of a proximal humerus fracture dislocation between 2011 and 2020 were identified. Patients with isolated greater tuberosity fract

Femoral neck fractures: a cohort comparison of nonunion and complication rates after ballistic versus blunt mechanism fractures.

PURPOSE: This study aims to review complications and union rates of ballistic femoral neck fractures when compared to their blunt counterparts. We hypothesize that gunshot injuries to the femoral neck may have higher rates of complications (nonunion, avascular necrosis [AVN], and implant failure) as a result of increasing comminution and disruption to the already tenuous blood supply through the femoral neck. METHODS: Following the Institutional Review Board approval, a retrospective chart review was performed identifying femoral neck fractures at a single level 1 trauma center from 2018 to 2

Evaluation of methods and timing in nail dynamisation for treating delayed healing femoral shaft fractures.

INTRODUCTION: Dynamisation is a simple method for treating femoral shaft fractures (FSFs) in patients with delayed healing after intramedullary nailing. However, only around half of patients with a slow-healing FSF have been successfully treated with this strategy. Although it is thought that dynamisation with a screw preserved in the dynamic locking hole improves treatment outcome in these patients, to date, no studies have demonstrated empirical evidence supporting this technique. In this study, we investigated the effects of dynamisation with and without rotation and length control as well

320 mm InterTan nail optimizes biomechanics in AO/OTA 31A2.3 fractures: superior stress distribution, micromotion, and strain for enhanced healing.

PURPOSE: This study compares the biomechanical performance of InterTan nails of three lengths (180 mm, 240 mm, and 320 mm) in treating AO/OTA 31A2.3 comminuted intertrochanteric fractures, which are highly unstable and prone to fixation failure. The research question focuses on identifying the nail length that optimizes stress distribution, displacement, and strain to enhance fracture healing and reduce failure risk, thereby providing a theoretical foundation for clinical decision-making. METHODS: Femoral CT images from a healthy 24-year-old male were used to reconstruct cortical and cancello
