General anesthesia with local infiltration reduces urine retention rate and prolongs analgesic effect than spinal anesthesia for hemorrhoidectomy.

INTRODUCTION: Postoperative pain and complications pose significant challenges following a hemorrhoidectomy. Attaining effective anesthesia with minimal complications is crucial. The ideal anesthesia method for ambulatory hemorrhoidectomy remains uncertain. This study aimed to investigate whether the combination of general anesthesia plus local infiltration (GAL) is associated with lower complications and reduced pain compared to spinal anesthesia (SA) in the context of hemorrhoidectomy. METHODS: This retrospective single-center cohort study, conducted in a tertiary medical center in East Asi

A Case of Bilateral Anesthesia Mumps after Cesarean Section under Spinal Anesthesia: A Rare Case and Literature Review.

A 30-year-old pregnant female presented to the emergency department with labor pain in her 39th week of pregnancy. Cesarean section under spinal anesthesia was the chosen route of delivery for this patient due to previous cesarean section in her first pregnancy. The delivery itself was uneventful but seven hours after the surgery, bilateral swelling of parotid glands were noted. Rehydration therapy and a single dose of hydrocortisone (100 mg IV route) were administered, and swellings were completely resolved on postoperative day 2. This is one of the rare cases of anesthesia mumps after spinal

Hemodynamic effects of high spinal anesthesia under general anesthesia in infants undergoing cardiac surgery: a retrospective cohort study.

BACKGROUND: High spinal anesthesia (HSA), in combination with general anesthesia (GA), has been proposed as a technique to reduce the surgical stress response while minimizing opioid use in cardiac surgery. However, concerns remain regarding the potential for HSA-induced hemodynamic instability, particularly in infants with congenital heart disease. Therefore, this study aimed to evaluate the impact of HSA on hemodynamic parameters in infants undergoing cardiac surgery. METHODS: This single-center retrospective cohort study was conducted at a tertiary medical center and included pediatric pat

Segmental thoracic spinal anesthesia versus general anesthesia for breast cancer surgery: A prospective randomized-controlled open-label trial.

BACKGROUND AND AIMS: Breast surgery is associated with moderate-to-severe postoperative pain, nausea, and vomiting. For this, neuraxial anesthesia might be a better alternative to general anesthesia (GA), providing superior analgesia, with higher patient satisfaction and lesser incidence of nausea vomiting. This randomized-controlled open-label trial was done to compare segmental spinal and GA for breast cancer surgery. MATERIAL AND METHODS: The present study enrolled 56 female patients scheduled to undergo breast cancer surgery. They were randomly divided into two groups, group G (received s

Comparing local tumescent anesthesia and spinal anesthesia methods during and after endovenous radiofrequency ablation of great saphenous vein.

BACKGROUND: Local or spinal anesthesia methods can be used during radiofrequency ablation (RFA) of the great saphenous vein. There is a gap in the literature regarding comparing and contrasting the side effects of the mentioned two methods. This study aims to retrospectively compare the spinal anesthesia method with the local tumescent anesthesia method during RFA of the great saphenous vein that also includes mini-phlebectomy. METHODS: We retrospectively analyzed patients who underwent RFA and mini-phlebectomy divided in two groups according to the anesthesia applied method, i.e. spinal anes

[Continuous spinal anesthesia in very elderly patients with high anesthesia risk in traumatologic-orthopedic and general surgery interventions].

Continuous spinal anaesthesia (CSA) was carried out via a 28-gauge spinal catheter in 154 surgical patients at the Department of Anaesthesiology and Critical Care at Radeberg Asklepios-ASB Hospital between May 1992 and March 1999. The method was used preferably in patients aged over 70 (mean age 82.3 years) with high general risk during anaesthesia (ASA III-IV) who underwent orthopaedic or general surgery of the lower limb and hypogastrium. Remarkably, an anaesthetic level of between Th 8 and Th 10 was achieved with the low initial dose of 7.5 mg of 0.5% hyperbaric bupivacaine. Only minimal ca

Impact of two sedation protocols on cardiac electrophysiology during spinal anesthesia.

BACKGROUND/AIM: Perioperative factors such as anesthetic drugs, central blocks, surgical stress, pain, and anxiety can affect cardiac electrophysiology and increase the risk of arrhythmias. Arrhythmia susceptibility can be assessed using ventricular markers such as the QT interval (QT), heart rate-corrected QT interval (QTc), QT dispersion (QTd), time between peak and end of T wave (Tp-e), cardiac electrophysiologic balance index (iCEB), and heart rate-corrected index of cardiac electrophysiologic balance (iCEBc), obtained from noninvasive electrocardiography (ECG) data. This study compared th

Postpartum Cerebral Venous Thrombosis and Spinal CSF Leak after Epidural Anesthesia.

Postpartum headache following obstetric epidural anesthesia may be secondary to post-dural puncture headache (PDPH) from spinal CSF leakage or cerebral venous thrombosis (CVT). Although these entities are generally considered distinct, intracranial hypotension from persistent spinal CSF leakage may contribute to the development of CVT. We present four postpartum patients with imaging-confirmed CVT and coexisting spinal CSF leaks after lumbar epidural anesthesia. All patients initially presented with headache and were diagnosed with CVT, prompting anticoagulation therapy. However, imaging findi

Pediatric spinal anesthesia at a tertiary care hospital: Eleven years after.

BACKGROUND: Pediatric spinal anesthesia is an old technique whose use is not widespread, in spite of reducing the risk of cardiorespiratory events (hypoxemia, bradycardia, and hypotension) associated with general anesthesia, especially in neonates and infants. This retrospective cohort study aimed to assess the safety and effectiveness of the pediatric spinal anesthesia program at our tertiary care hospital over 11 years. METHODS: Two hundred children, between 8 days and 13 years of age, who underwent lower body surgery under spinal anesthesia from May 2010 to July 2021 were included. Demogra

Endoscopic posterior decompression under local anesthesia for degenerative lumbar spinal stenosis.

OBJECTIVEVarious minimally invasive techniques have been described for the decompression of lumbar spinal stenosis (LSS). However, few reports have described the results of endoscopic posterior decompression (EPD) with laminectomy performed under local anesthesia. This study aimed to evaluate the clinical and radiological outcomes of EPD performed under local anesthesia in patients with LSS and to compare the procedural outcomes in patients with and without preoperative spondylolisthesis.METHODSFifty patients (28 female and 22 male) who underwent EPD under local anesthesia were included in thi

Takotsubo Cardiomyopathy after Spinal Anesthesia for a Minimally Invasive Urologic Procedure.

We present the case of a patient who suffered from Takotsubo cardiomyopathy (TCM) immediately after the initiation of subarachnoid anesthesia for a minimally invasive urologic procedure (tension-free vaginal tape (TVT) surgery for stress urine incontinence). TCM mimics acute coronary syndrome and is caused by an exaggerated sympathetic reaction to significant emotional or physical stress. Our patient suffered from chest pain, palpitations, dyspnea, and hemodynamic instability immediately following subarachnoid anesthesia and later in the postanesthesia care unit. Blood troponin was elevated an

Low-dose spinal anesthesia for urgent laparotomy in severe myasthenia gravis.

Myasthenia gravis (MG) is an autoimmune disease with an incidence of 2-10/100,000 cases per year, characterized by muscle weakness secondary to destruction of postsynaptic acetylcholine receptors. In these patients, important perioperative issues remain unresolved, namely, optimal administration of cholinesterase inhibitors, risks of regional anesthesia, and prediction of need of postoperative mechanical ventilation. We describe the use of a low-dose spinal anesthesia in a patient with MG who was submitted for emergence exploratory laparotomy. The utilization of low-dose spinal anesthesia allo
