A case of nasopharyngeal angiofibroma removed using a minimally invasive endoscopic endonasal technique.

Nasopharyngeal angiofibroma is a rare head and neck tumor characterized by hypervascularity. With the development of various surgical devices and hemostatic agents, endoscopic endonasal surgery for nasopharyngeal angiofibroma, which is characterized by significant intraoperative bleeding, has become possible. We describe our experiences performing an effective endoscopic endonasal removal of a nasopharyngeal angiofibroma without embolization, using advanced devices and hemostatic agents. A 21-year-old male patient presented with massive epistaxis. The characteristic findings of angiofibroma we

Para-testicular cellular angiofibroma: a rare tumour in a male renal transplant patient.

Cellular angiofibroma is a rare, benign, mesenchymal neoplasm that should be distinguished from other more aggressive mesenchymal lesions. A 32-year-old male presented with a painless para-testicular mass. Pathological examination showed features characteristic of cellular angiofibroma. He had a history of renal transplantation and was on immunosuppressive drugs. The pathogenesis of cellular angiofibroma is largely undetermined. The role of immunosuppressive therapy in the genesis of this tumour awaits further analysis.

Results of surgical management of nasopharyngeal angiofibroma. The Cleveland Clinic experience, 1977-1986.

During 1977 through 1986, 15 patients were evaluated and underwent surgery for nasopharyngeal angiofibroma at the Cleveland Clinic Foundation. All were males aged five to 20 years. Diagnoses were established clinically and radiographically. The most common presentations were nasal obstruction and epistaxis. Computed tomographic scans were obtained in almost all cases. In 14 patients, preoperative embolization of the tumor was performed at the time of diagnostic angiography; embolization of tumors made surgical excision easier and reduced blood loss. The most common surgical exposure used was a

Association of Preoperative Embolization and Vascularization Patterns With Outcomes in Juvenile Nasopharyngeal Angiofibroma Resection.

BACKGROUND: Preoperative vascular embolization (PVE) is commonly used in managing juvenile nasopharyngeal angiofibroma (JNA) to improve outcomes. However, few studies have evaluated how embolization patterns and vascular features affect prognosis. METHODS: A 15-year retrospective review was performed on 44 patients with JNA undergoing surgery at one institution. Data included demographics, vascular supply, embolization techniques, surgical outcomes, and recurrence. RESULTS: Forty-three patients underwent PVE. Most tumors were embolized transarterially (93.0%) using polyvinyl alcohol-based ag

TSC angiofibroma and ungual fibroma have different mutation signatures, with recurrent mutations in KMT2C.

PURPOSE: Tuberous sclerosis complex (TSC) is an autosomal dominant tumor suppressor syndrome characterized by tumors affecting multiple tissues, including skin, due to inactivating TSC1/TSC2 variants. Genome-wide profiling of somatic mutations in a unique collection of angiofibroma (FAF) and ungual fibroma (UF) TSC skin tumors was performed. METHODS: Genome sequencing was performed on 9 samples, comprising 4 FAF and 5 UF, along with 6 matched normal samples from 6 individuals with TSC. RESULTS: TSC-FAF and TSC-UF skin tumors have different mutation signatures, with a predominance of UV-relat

Novel Surgical Strategy System for Juvenile Nasopharyngeal Angiofibroma: A Focus on Feeding Artery Management.

OBJECTIVES: For intraoperative bleeding control in patients with juvenile nasopharyngeal angiofibroma (JNA), we developed a novel surgical strategy system. This system is focused on feeding artery management, based on the spatial relationship between the tumor entity and feeding artery as determined by three-dimensional reconstruction. METHODS: This historical controlled study was conducted at a tertiary medical institution from October 2013 to September 2022. The retrospective group comprised of previous JNA patients received traditional nasoendoscopic sursery. Three-dimensional CT reconstru

Juvenile Nasopharyngeal Angiofibroma: A Case Report of a Rare Adolescent Head and Neck Tumor.

Juvenile nasopharyngeal angiofibroma (JNA) is a rare benign head and neck tumor. We report a rare case of JNA, provide a brief literature review, and treatment options, and emphasized the role of flutamide as pre-surgical medication for tumor regression. JNA primarily affects adolescent males aged 14 to 25 years. There are various theories explaining the formation of the tumor. However, sex hormones are found to play a crucial role in the etiology of the tumor. In recent years testosterone and dihydrotestosterone receptors have been identified on the tumor thus suggesting the strong influence

A case report of an extremely rare type of cardiac tumor: Primary cardiac angiofibroma.

Cardiac angiofibroma is a very rare diagnosis when a patient develops an intracardiac mass. It is a primary benign cardiac tumor with a scarcity of information in the literature. This case report illustrates a 26-year-old man with a complaint of chronic chest tightness who was first diagnosed with right ventricle tumor by echocardiography then underwent cardiac magnetic resonance imaging (CMR) which confirmed the presence of a highly-vascular tumor with radiologically benign behavior. Then his tumor was excised, his postoperative course was uncomplicated and he was well within almost 2 months

68 Ga DOTANOC PET/CT Scan in Primary Juvenile Nasopharyngeal Angiofibroma - A Pilot Study.

OBJECTIVES/HYPOTHESIS: Somatostatin receptors (SSTRs) are highly expressed in neuroendocrine tumors and is exploited for its imaging and treatment. SSTRs expression is also demonstrated in diverse benign and malignant tumor cell types and proliferating peri-tumoral vessels. Similarly, Juvenile Nasopharyngeal Angiofibroma (JNA) expresses different SSTRs and may be utilized for its imaging and treatment using DOTA, 1-Nal3-octreotide (DOTANOC)-PET/CT scan. STUDY DESIGN: Prospective cohort. METHODS: Nineteen clinico-radiologically diagnosed primary JNA patients underwent a 68 Ga-DOTANOC PET-CT s

Assisted resection of lateral extension of juvenile nasopharyngeal angiofibroma using a sphenopalatine fossa dissector.

BACKGROUND: Juvenile nasopharyngeal angiofibroma often attaches firmly to the adjoining bony region around the sphenopalatine foramina-sphenopalatine fossa-pterygomaxillary fissure. This can result in hourglass-shaped constriction and predispose to incomplete resection (residual disease) with a transpalatal approach. This paper describes attempts to address this 'inaccessible' area with a novel instrument, used since 2012. METHODS: Measurements of the sphenopalatine foramen, nasal septum, posterior nasopharyngeal wall and hard palate were undertaken in 20 skulls and 10 computed tomography sca

Imaging of Unilateral Meningo-ophthalmic Artery Anomaly in a Patient with Bilateral Nasopharyngeal Angiofibroma.

A 12-year-old boy with epistaxis presented with a rare midline nasopharyngeal angiofibroma that extended lateral into the pterygoid and infratemporal fossae. Pre-operative angiography revealed bilateral prominent feeder arteries and two major anastomotic connections, and a rare left meningo-ophthalmic artery (M-OA) anomaly that was the sole path of supply to the eye. A literature search using Pubmed and Medline was conducted. For imaging, a six-vessel study (i.e. external and internal carotid and vertebral arteries on both sides) was selected. Embolization of prominent tumor feeder arteries wa

A systematic review of surgical outcomes for advanced juvenile nasopharyngeal angiofibroma with intracranial involvement.

OBJECTIVES/HYPOTHESIS: Although transnasal endoscopic resection has become an established treatment for most juvenile nasopharyngeal angiofibroma (JNA), surgical management of JNA with intracranial extension remains challenging. This study systematically reviews the JNA literature to determine surgical outcomes. STUDY DESIGN: Systematic review. METHODS: A systematic search of the PubMed was undertaken using a combination of MeSH terms: angiofibroma, nasopharynx. The search was limited to articles published in the English language between January 1990 and April 2012. Each article was reviewed

[Nasal angiofibroma in a 60 year-old female. A clinical case and literature review].

We are reporting an atypical and very rare clinical case of nasal angiofibroma in a 60 year-old female who presented with recurrent epistaxis and nasal blockage. The tumour was located on the right inferior turbinate C.T scans., M.R.I. and Angiography were required as complementary studies. After trying without success a previous arterial embolization, the patient underwent surgery by Functional Endoscopic Nasal Surgery (F.E.N.S.) with removal of the mass in the right nasal fossa. The anatomopathologic result confirmed the histology of angiofibroma and the clinical evolution was satisfactory.

Growth factors and receptors in juvenile nasopharyngeal angiofibroma and nasal polyps: an immunohistochemical study.

BACKGROUND: Juvenile nasopharyngeal angiofibroma is a rare nasopharyngeal tumor that occurs exclusively in adolescent boys. It is a histologically benign but locally persistent growth of stromal and vascular tissue. Although male hormones and some growth factors, such as transforming growth factor beta1 (TGF-beta1), insulin-like growth factor II (IGF-II), and, lately, the proto-oncogene beta-catenin, have been implicated in the histogenesis of the tumor, the biologic signaling pathways that drive this peculiar fibrovascular proliferation are still nuclear. OBJECTIVE: To evaluate immunoexpress

Nasopharyngeal angiofibroma: a reappraisal of clinical features and treatment at National Taiwan University Hospital.

Nasopharyngeal angiofibroma (NAF), occurring mostly in young men, is a histologically benign tumor with aggressive clinical behavior that includes repeated epistaxis and intractable nasal obstruction. This paper reviews our recent experience at National Taiwan University Hospital (NTUH), and compares the results with those of a previous study (1955-1980) at NTUH to highlight the developments in the treatment of NAF. Fifteen patients with a diagnosis of NAF from 1984 to 1997 were included, and their clinical presentations, radiographic studies, treatments, and outcomes were retrospectively anal

Timely resection of juvenile nasopharyngeal angiofibroma following embolization for uncontrolled acute epistaxis case report.

Uncontrolled epistaxis in patients with juvenile nasopharyngeal angiofibroma requires prompt intervention. When embolization is required, neovascularization can occur, making expedited surgical resection essential. We conducted a single-institution, retrospective review of one patient. This case study details the patient's clinical presentation and his preoperative, intraoperative, and postoperative course. We present the management of a 14-year-old male with a juvenile nasopharyngeal angiofibroma, who 3 days after initial evaluation at our institution, developed uncontrolled epistaxis requiri
