Circumcision for HIV Prevention in Men Who Have Sex With Men: An Updated Meta-Analysis.

BACKGROUND: Circumcision reduces HIV risk for men who have sex with women, but for men who have sex with men (MSM), prior meta-analyses were ambiguous because of inconsistent, highly confounded data suggesting regional differences. This has left the weight of evidence unclear even after a small 2024 trial in primarily insertive MSM (PI-MSM) in China showed protection. Any protection would be for PI-MSM and could have substantial impact given high incidence. SETTING: No restrictions. METHODS: We performed an updated literature search on the association of male circumcision with HIV among MSM.

A 'snip' in time: what is the best age to circumcise?

BACKGROUND: Circumcision is a common procedure, but regional and societal attitudes differ on whether there is a need for a male to be circumcised and, if so, at what age. This is an important issue for many parents, but also pediatricians, other doctors, policy makers, public health authorities, medical bodies, and males themselves. DISCUSSION: We show here that infancy is an optimal time for clinical circumcision because an infant's low mobility facilitates the use of local anesthesia, sutures are not required, healing is quick, cosmetic outcome is usually excellent, costs are minimal, and

Circumcised hypospadias.

PURPOSE: Patients with circumcised hypospadias have been of significant concern over many decades due to the belief that prior circumcision might negatively affect the results of hypospadias' repair. We evaluated outcomes in consecutive males with anterior, distal penile, and the megameatus with intact prepuce variant of hypospadias (MIP). METHODS: After IRB approval a retrospective of 48 consecutive males with circumcised hypospadias was reviewed. In all cases the urethroplasty was accomplished with either urethral plate tubularization or a MAGPI procedure. No skin flaps were used. RESULTS:

Why circumcision is a biomedical imperative for the 21(st) century.

Circumcision of males represents a surgical "vaccine" against a wide variety of infections, adverse medical conditions and potentially fatal diseases over their lifetime, and also protects their sexual partners. In experienced hands, this common, inexpensive procedure is very safe, can be pain-free and can be performed at any age. The benefits vastly outweigh risks. The enormous public health benefits include protection from urinary tract infections, sexually transmitted HIV, HPV, syphilis and chancroid, penile and prostate cancer, phimosis, thrush, and inflammatory dermatoses. In women circum

The circumcision controversy: an update.

One constructive feature has come out of all of the circumcision controversy. It has made us aware that neonatal circumcision is not a simple procedure and that it should not be regarded as "routine." It is a surgical procedure and it should be taught as one. The authors of this article wrote a review article on circumcision in 1981 and Dr. Grossman has recently published a book designed to teach proper circumcision technique, and to familiarize the operator with the common congenital anomalies as well as the prevention and treatment of the complications of circumcision. The advocacy of neonat

Female circumcision.

The letter on unsafe male circumcision by Dr. H. Gretahun (Africa Health, March 1997) brings into very clear focus the whole issue of genital mutilation. Circumcision, whether male or female, does not per se amount to genital mutilation unless performed unsafely. Female circumcision involves only reducing the rudimentary penis, the clitoris, in order to tuck it neatly and beautifully under the labia majora. When the operation involves slicing off the labia, it ceases to be circumcision. Any scar tissue formed, both in size and location, is never enough to interfere with childbirth, otherwise t

Urosepsis and postrenal acute renal failure in a neonate following circumcision with Plastibell device.

Plastibell is one of the three most common devices used for neonatal circumcision in the United States, with a complication rate as low as 1.8%. The Plastibell circumcision device is commonly used under local anesthesia for religious circumcision in male neonates, because of cosmetic reasons and ease of use. Occasionally, instead of falling off, the device may get buried under the skin along the shaft of the penis, thereby obstructing the normal flow of urine. Furthermore, the foreskin of neonates is highly vascularized, and hence, hemorrhage and infection are possible when the skin is cut. Ne

[Modified circumcision with a disposable suture device].

OBJECTIVE: To improve the methods and reduce the complications of circumcision with a disposable suture device. METHODS: This study included 325 male patients aged 14-65 (mean 28.9) years, treated for redundant prepuce or phimosis by modified (n = 201) or conventional circumcision with a disposable suture device (n = 124). We compared the incidence of complications and the patients' satisfaction between the two surgical methods. RESULTS: Compared with conventional circumcision, the modified method showed a significantly lower incidence of postoperative bleeding (14.52% vs 2.49% , P < 0.05) a

Examination of short and long term complications of thermocautery, plastic clamping, and surgical circumcision techniques.

OBJECTIVE: In this study, thermocautery, plastic clamping, and conventional (open surgical) circumcision techniques were compared in terms of their complications. METHODS: Male patients who underwent circumcisions between May 2014 and May 2015 in two separate pediatric surgery clinics were retrospectively analyzed using the hospital registry system. These patients were evaluated in terms of age, accompanying pathologies, anesthesia techniques, complication rates, duration of surgery, and circumcision techniques. A statistical analysis of the data was performed, with a P<0.05 considered to be

Foreskin and the molecular politics of risk.

In this paper, I examine disputes over recent claims that male circumcision reduces HIV risk to suggest a complicated relationship between risk individualization and categorization. Whereas randomized controlled trials (RCTs) conducted in sub-Saharan Africa appear to have provided key evidence for the World Health Organization's endorsement of male circumcision as an HIV prevention strategy, RCTs alone did not provide evidence for the underlying causal mechanism. For that, medical authorities have turned to histo-immunological studies of the foreskin's biomolecular vulnerability to HIV, thus m

Penile transplantation as an appropriate response to botched traditional circumcisions in South Africa: an argument against.

Traditional male circumcision is a deeply entrenched cultural practice in South Africa. In recent times, there have been increasing numbers of botched circumcisions by untrained and unscrupulous practitioners, leading to genital mutilation and often, the need for penile amputation. Hailed as a world's first, a team of surgeons conducted the first successful penile transplant in Cape Town, South Africa in 2015. Despite the euphoria of this surgical victory, concerns about the use of this costly intervention in a context of severe resource constraints have been raised. In this paper, we explore

Sexual Risk Behaviours and Willingness to Be Circumcised among Uncircumcised Adult Men in Uganda.

BACKGROUND: There has been substantial demand for safe male circumcision (SMC) in Uganda in the early programme scale-up phase. Research indicates that early adopters of new interventions often differ from later adopters in relation to a range of behaviours. However, there is limited knowledge about the risk profile of men who were willing to be circumcised at the time of launching the SMC programme, i.e., potential early adopters, compared to those who were reluctant. The aim of this study was to address this gap to provide indications on whether it is likely that potential early adopters of

Inflammatory changes in the prepuce and clinical findings according to the stages of phimosis.

BACKGROUND/AIM: Circumcision refers to the surgical removal of all or part of the prepuce. Circumcision is commonly performed on males in childhood to prevent urinary tract infections and for religious reasons. This study was designed to investigate the inflammatory processes in the prepuce according to the degree of phimosis in patients who underwent circumcision. MATERIALS AND METHODS: We conducted a prospective study on 173 male children under the age of 18 who underwent circumcision between June and September 2024. A comprehensive physical examination of the genitourinary system was perfo

Use of topical versus injectable anaesthesia for ShangRing circumcisions in men and boys in Kenya: Results from a randomized controlled trial.

BACKGROUND: The ShangRing is a disposable, collar clamp circumcision device pre-qualified for use in men and boys 13 years and above. It has been shown to be faster than conventional circumcision with comparable adverse event (AE) rates and high client satisfaction. Voluntary medical male circumcision (VMMC) has been shown to dramatically reduce the risk of HIV acquisition in males. However, the fear of pain during circumcision is an important barrier to uptake. Use of topical anesthesia thus presents an opportunity to address this. OBJECTIVES: We sought to evaluate the safety, effectiveness

Avoidance of Circumcision during the Phallic Stage: Myth or Reality?

OBJECTIVE: Circumcision is a practice that is commonly implemented across the globe for reasons pertaining to medical necessity, social attitudes, and religious beliefs. This procedure, when performed on young boys, results in the infliction of harm upon the penis, thereby giving rise to the development of castration anxiety. Castration anxiety can be defined as an overwhelming fear of damaging or losing the penis during the phallic period, which, according to Sigmund Freud's theory of psychosexual development, occurs between the ages of 3 and 6 years. The present study aims to compare social

Foreskin Morbidity in Uncircumcised Males.

OBJECTIVE: As a consequence of the discussion on whether the health benefits of newborn male circumcision outweigh the risks and the discrepancies in reported figures of complications, we evaluated the incidence and morbidity of foreskin surgery due to medical indications in boys from the Capital Region of Denmark in 2014. METHODS: Medical records from all boys operated on the foreskin due to medical reasons in the Capital Region in 2014 were reviewed. Patients with hypospadias, ritual circumcision, and redo-surgery because of complications to nontherapeutic circumcision were excluded. RESUL
