Practical Recommendations for the Diagnosis and Management of Lysosomal Acid Lipase Deficiency with a Focus on Wolman Disease.

Lysosomal acid lipase deficiency (LAL-D) is an ultra-rare lysosomal storage disease with two distinct phenotypes, an infantile-onset form (formerly Wolman disease) and a later-onset form (formerly cholesteryl ester storage disease). The objective of this narrative review is to examine the most important aspects of the diagnosis and treatment of LAL-D and to provide practical expert recommendations. The infantile-onset form occurs in the first weeks of life and is characterized by malnourishment and failure to thrive due to gastrointestinal impairment (vomiting, diarrhea, malabsorption), as wel

Infantile Systemic Hyalinosis: A Case Report and Literature Review.

Infantile systemic hyalinosis (ISH) is a very rare disorder belonging to the heterozygous group of genetic fibromatosis. There is a diffuse deposition of hyaline material in the skin, gastrointestinal tract, muscle, lymph node, spleen, thyroid, and adrenal gland due to which it presents clinically with multiple subcutaneous skin nodules, gingival hypertrophy, osteopenia, joint contractures, failure to thrive, and diarrhea with protein-losing enteropathy, and is associated with recurrent infections. The disease is caused by mutations in ANTXR2 also known as the CMG2 gene, which encodes the tran

Study on the mechanism of Yihuang powder ethanol extract on spleen deficiency diarrhea in young rats based on intestinal flora and metabonomics.

Infantile spleen deficiency diarrhea (SDD) impacts children's health. Yihuang Powder (YHP) is a traditional prescription for treating it, and the ethanol extract of YHP (EYHP) has been optimized by the research group, yet its differences from the traditional water extract of YHP (WYHP) remain unclear. This study aimed to clarify the variances in chemical composition and pharmacological effects between EYHP and WYHP, and reveal EYHP's mechanism in treating SDD via intestinal flora and fecal metabolomics. Ultra performance liquid chromatography coupled with quadrupole-time of flight mass spectro

Does access to improved sanitation reduce childhood diarrhea in rural India?

Almost nine million children under 5 years of age die every year. Diarrhea is considered to be the second leading cause of under-five mortality in developing countries. About one out of five deaths is caused by diarrhea. In this paper, we use the newly available data set District Level Household Survey 3 to quantify the impact of access to improved sanitation on diarrheal morbidity for children less than 5 years of age in India. Using propensity score matching, we find that access to improved sanitation reduces the risk of contracting diarrhea by 2.2 percentage points. There is considerable he

[Clinical study of acute diarrhea caused by rotavirus].

To establish the implication of Rotavirus in acute diarrhea of babies and children. Prospective study on 164 patients aged between 1 month and 3 years with acute diarrhea, admitted in the Clinical Hospital of Infectious Diseases Iaşi between 01.01.2005 - 31.03.2005. Rotavirus cause the illness in 30 patients (18.29%). The disease was more prevalent among children with dystrophy or with other comorbidities like: iron-deficiency anemia, intestinal parasitosis, rhinopharyngitis, pneumococcal meningitidis. In seven cases the infectious was considered to be nosocomial. During the illness patients a

Coccidian parasites as a cause of watery diarrhoea among protein energy malnourished and other immunocompromised Egyptian children.

The role of coccidian parasites in the pathogenesis of watery diarrhea was studied among children with protein energy malnutrition (PEM), immunocompromised due to causes other than PEM and immunocompetent diarrheic of matched age and sex, as controls. The results showed that the prevalence of infection was 15.48%, Cryptosporidium was the most prevalent and showed 14.19% (18.3%, 17.5% and 7.3% in PEM, immunocompromised and immunocompetent cases respectively). Cyclospora oocysts were detected only among 2 cases (1.29%) of PEM group. Isospora oocysts were not detected in any of the studied groups

Knowledge, attitude, practice, and prescribing pattern of oral rehydration therapy among private practitioners in Nigeria.

To determine the knowledge, attitude, and practice of oral rehydration therapy (ORT) among private medical practitioners in Enugu, Nigeria, 91 doctors were interviewed using a structured questionnaire. All the doctors had heard of ORT and believed in its efficacy. The commonest source of information on ORT was the medical school (44%). Fifty percent would recommend salt-sugar solution (SSS) rather than oral rehydration solution (ORS). The main reason is its cost-effectiveness and easy availability. Only 55% of the respondents knew how to prepare SSS correctly. The percentage of doctors who pre

The impact of systematic use of oral rehydration therapy on outcome in acute diarrheal disease in children.

A review of our experience with diarrheal disease (DD) at the University of Istanbul Children's Hospital in Capa, covering the years 1987 to 1989, is presented in this paper. DD is one of the most common conditions encountered among patients presenting to the outpatient clinic (5.9% of all cases). The majority of DD cases were in the four- to 12-month age group. During the surveillance period, summertime peaks in DD were observed, suggesting an increase in bacterial infection. Out of 8749 cases of DD, 5.2% showed severe dehydration. Rates of hospitalization for DD did not show any differences

[Oral rehydration therapy: an analysis of its results and impact on the hospitalization and mortality of children with diarrhea].

We present results of four years in oral rehydration therapy (ORT) in the Hospital Infantil del Estado de Sonora. There was 10.2 consults by diarrhoea for day. Children lower of one year old received oral rehydration therapy in 86.8%, were included 11% of prolonged diarrhoea and 32.3% of children with malnutrition. During the procedure diarrhoea there was complicated in 3% with paralytic ileus sepsis and pneumonia. Effectivity of ORT was in 90.9%; 92.8% in light dehydration and 78.7% moderate. Failure in 8.6% was due to vomitus, no acceptation of the oral solution, abundant evacuations and oth

[Rapid rehydration by intravenous route in dehydrated children by diarrhea].

Sixty-two children (13 days to 11 years old) with 1-12% dehydration due to acute watery diarrhoea were rehydrated in an Emergency Room Setting by means of intravenous fluid therapy in 6.34 +/- 0.57 hr. Mean duration of hospital stay was reduced and 87% of patients stayed 24 hrs. or less as compared to 36% in previous studies at this hospital. The correction of dehydration and its electrolyte imbalance was done in a shorter time; recovery of the patients was better and the complications were less; the method allowed a more rational use of beds in the Emergency Room Setting.

A Particularly Severe Diarrhea in Young Infants With Klebsiella spp. Cultured From the Stool: A Case Series.

BACKGROUND: Klebsiella spp. are frequently found as commensals of the gastrointestinal tract in infants, and their presence in stool cultures is often dismissed as colonization. However, their role in causing severe diarrhea in previously healthy infants remains unclear. METHODS: We retrospectively analyzed 4 cases of previously healthy term infants under 3 months of age hospitalized between 2016 and 2024 with unusually severe diarrhea. All had Klebsiella spp. isolated in stool cultures as the sole pathogen. Clinical and laboratory features were evaluated. Additional hospital records from 202
