[Dissecting aneurysms at the bases of the brachiocephalic artery and the left common carotid artery due to localized dissection of the aortic arch; report of a case].

A 43-year-old male lost consciousness immediately after archery practice, and was brought to our hospital by ambulance. Angiography showed dissecting aneurysms at the bases of the brachiocephalic artery and the left common carotid artery, causing compression of these arteries. Under cardiopulmonary bypass with selective cerebral perfusion, the blood supply to these arteries was restored with a bifurcated graft. Surgical specimen showed localized dissection of the aortic arch at the bifurcation to the brachiocephalic artery and the left common carotid artery, with the formation of dissecting an

Mapping of the neonatal Fc receptor in the rodent eye.

PURPOSE: The neonatal Fc receptor (FcRn) has been known to modulate IgG transport and protect against IgG catabolism, resulting in extension of the serum half-life of IgG. The goal of this study was to localize FcRn receptor expression in the rat's eye. METHODS: The cornea, retina, conjunctiva, ciliary body and iris, retinal pigment epithelium and choroid, and lens were dissected from each rat's eye, and total RNA was purified. The first-strand cDNAs were synthesized and subjected to PCR reaction. For control samples, reverse transcriptase was omitted. A monoclonal antibody against the FcRn h

Siltation and tidal effects: a comprehensive, clinical, and didactic approach to reasoning about vertebrobasilar hypoflow conditions.

OBJECTIVES: To propose a clinically applicable model for understanding vertebrobasilar hypoperfusion by categorizing its causes into two mechanisms: the siltation effect, related to local vascular obstruction, and the tidal effect, related to systemic reduction in posterior circulation blood flow. METHODS: A non-systematic narrative review was conducted using selected literature in the fields of neurotology, vascular neurology, and cardiovascular medicine. The analogy of coastal rivers affected by sedimentation and tides was used to explain the proposed classification. Clinical examples were

Complications of preoperative balloon test occlusion of the internal carotid arteries: experience in 300 cases.

Treatment of some tumors and aneurysms of the skull base may require internal carotid artery (ICA) sacrifice. Preoperatively to determine the dependence of the cerebral blood flow on a particular vessel, we perform a balloon test occlusion (BTO) by temporarily occluding the vessel in an awake patient. During occlusion, clinical evaluations and cerebral blood flow measurements are assessed. We have performed 300 BTOs. Eleven patients (3.7%) have had complications. Six (2%) were asymptomatic dissections. Five (1.7%) had neurologic deficits that persisted beyond the test period. Of these five, on

Characteristics and Management of Dissection in Early Cannulation Arteriovenous Grafts.

INTRODUCTION: Vascular access is critical for hemodialysis in patients with end-stage renal disease. Autogenous arteriovenous fistula is the first-line option, while arteriovenous grafts are an alternative for patients with poor native vascular conditions. Over the past decade, early cannulation arteriovenous grafts with a multi-layered structure have emerged, allowing cannulation within 24-72 h after implantation to meet urgent dialysis needs. However, this specific structure renders them susceptible to dissection, a unique complication. To date, no systematic review has summarized the charac

Transcatheter endothermal septotomy in complex endovascular repair of dissected aortic aneurysms.

Extensive aortic aneurysms with chronic dissection present a challenge in complex endovascular aortic repair due to limited luminal working room, suboptimal landing zones, and target vessels which may originate from the false lumen. Adjunctive endothermal septotomy is a technique to modify the aortic anatomy to a state more amenable to minimally invasive repair. This technique uses radiofrequency energy to transect the aortic septum, creating a common lumen within which to conduct the aortic repair. In this narrative review, the technique for endothermal septotomy is described and the supporti

Novel and Innovative Hybrid Technique for Type A Aortic Dissection.

Acute Stanford type A aortic dissection (TAAD) is a surgical emergency characterized by a high mortality rate and numerous complications. In the treatment of TAAD, the timing of surgery and the choice of surgical procedure are of paramount importance. Open total aortic arch reconstruction remains the gold standard for aortic arch surgery and is one of the most challenging procedures. However, this approach is invasive, relatively lengthy, and associated with substantial bleeding, which necessitates high levels of operator skill and carries the risk of multiple complications, particularly neuro

It's in the game: A review of neurological lesions associated with sports.

INTRODUCTION: The practice of sports may lead to neurological injuries. While relatively uncommon (overall incidence of approximately 2.5%), and mostly benign and transient, some conditions may be life-threatening and permanent. Thus, both clinical neurologists and sports physicians should be aware of their existence and relevance. We aimed to review all sports-related neurological injuries and illnesses reported in the literature. METHODS: Following SANRA guidelines, we performed a narrative review and searched PubMed and Scopus databases. Relevant sports were selected based on their recogni

Fingertip Replantation: Surgical Technique, Tips, and Tricks.

Replantation of fingertip amputations restores the original tissue and is the ideal treatment to provide the best aesthetic and functional outcome. However, successful fingertip replantation is considered challenging because it requires supermicrosurgery techniques. This article provides a detailed surgical technique for fingertip replantation and the authors' preferences and recommendations. In the authors' experience, the most important factors for successful fingertip replantation are meticulous vascular dissection, reliable arterial repair, and venous anastomosis to avoid postoperative ven

Hemolytic anemia caused by kinked graft 6 months after aortic dissection repair.

BACKGROUND: Clinically insignificant hemolytic anemia is occasionally a complication of prosthetic valve replacement. However, hemolysis related to kinked grafts is a very rare complication after central repair for acute aortic dissection. CASE PRESENTATION: A 42-year-old man had undergone replacement of the ascending aorta and a root repair for type A aortic dissection 6 months previously. Laboratory data showed mild hemolysis 5 months later, and he began to complain of fatigue on exertion. The serum hemoglobin level reduced to 8.6 g/dL, and lactate dehydrogenase levels increased to 3071 IU/

A Case of Stanford Type A Acute Aortic Dissection With Preoperative Extensive Cerebral Infarction Successfully Treated Surgically.

Patients with organ malperfusion from acute aortic dissection (AAD) have poor outcomes, and the surgical indications for patients with AAD complicated by extensive cerebral infarction have not been established. Here, we report a successfully treated surgical case of a patient with cerebral infarction and Stanford type A, AAD. A 77-year-old man was admitted to the hospital with a chief complaint of left paresis. After confirming that there was no cerebral hemorrhage with a head computed tomography and an incision in the right neck, and the right internal carotid artery was ligated and closed, e

An Integrated Approach for Treatment of Acute Type A Aortic Dissection.

Background and objective: We reviewed a single-institution experience to verify the impact of surgery during different time intervals on early and late results in the treatment of patients with type A acute aortic dissection (A-AAD). Materials and Methods: From 2004 to 2021, a total of 258 patients underwent repair of A-AAD; patients were equally distributed among three periods: 2004-2010 (Era 1, n = 90), 2011-2016 (Era 2, n = 87), and 2017-2021 (Era 3, n = 81). The primary end-point was to assess whether through the years changes in indications, surgical strategies and techniques and increasi

One-stage surgery for type A aortic dissection and iliac artery aneurysm rupture.

We present a patient with Stanford type A aortic dissection with malperfusion of the left kidney and lower extremity, and rupture of a common iliac artery aneurysm. After establishing lower extremity reperfusion using a percutaneous femorofemoral shunt in the emergency room, simultaneous open aortic arch repair and open abdominal aorta repair were performed. Our approach allowed concomitant repair of proximal dissection, limb ischemia, and iliac artery aneurysm rupture, all of which are critical predictors of mortality.

[Endovascular treatment of descending aortic dissection (type B)].

Aortic dissection is one of the most serious aortic diseases by its potential for rupture, but also for other complications, such as cerebral or splanchnic ischemia, which may be fatal. If open surgery is the rule for lesions of the ascending aorta (type A), type B (not concerning the ascending aorta) is first a matter of medical treatment except when complications are present. In this case the placement of a stentgraft is a valuable alternative to open surgery. We report a patient presenting with a type B aortic dissection, characterized by rapid expansion and complicated by peri-aortic leaka

[Total arch replacement using a long elephant trunk for chronic dissecting aneurysm of a distal aortic arch].

A 74-year-old man was diagnosed with Stanford type B early thrombosed aortic dissection. Computed tomography (CT) 2 years later revealed that an aneurysm of the distal arch had enlarged from a maximal diameter of 47 mm to 62 mm and involved the descending aorta. Total arch replacement using a quadrifurcated arch graft proceeded through a median sternotomy under deep hypothermic cardiopulmonary bypass (20 degrees C) and selective brain perfusion. The long elephant trunk of another prosthesis was inserted into the descending aorta and a distal anastomosis was constructed immediately after the le

Expert consensus document on the treatment of descending thoracic aortic disease using endovascular stent-grafts.

Between 43,000 and 47,000 people die annually in the United States from diseases of the aorta and its branches and continues to increase. For the thoracic aorta, these diseases are increasingly treated by stent-grafting. No prospective randomized study exists comparing stent-grafting and open surgical treatment, including for disease subgroups. Currently, one stent-graft device is approved by the Food and Drug Administration for descending thoracic aortic aneurysms although two new devices are expected to obtain FDA approval in 2008. Stent-graft devices are used "off label" or under physician

Experimental comparison of mesenteric vessel sealing and thermal damage between one bipolar and two ultrasonic shears devices.

BACKGROUND: Several instruments are available for open and laparoscopic dissection, including electrothermal bipolar vessel sealers and ultrasonically coagulating shears. The vessel sealing ability of three devices in colorectal specimens was compared in an experimental study. METHODS: Surgical specimens from patients scheduled for elective open or laparoscopic colorectal resection were allocated to one of the three devices. After removal of the surgical specimen, up to eight mesenteric vessels were dissected ex vivo and sealed using the allocated instrument. The vessel seal was tested for th
