Bilateral distal fibular growth abnormalities in a dog.

A 6-month-old Shetland Sheepdog was examined because of a valgus deformity of the left hind limb. Six weeks earlier, the dog had developed a fracture of the distal portion of the right fibula, which was successfully treated by use of external coaptation. Radiography revealed valgus deformity centered at the left distal tibial and fibular physes. Radiographically, the left fibula was 1.5 cm shorter than the left tibia, and the right fibula was 1.3 cm shorter than the right tibia. A distal tibial closing wedge ostectomy was performed to correct the left hind limb angular deformity. The angular d

Maisonneuve fracture: case report of a missed diagnosis.

We report the case of a Maisonneuve fracture of the fibula in which the diagnosis was initially missed. The patient presented with a chief complaint of medial ankle pain. Rentograms of the ankle were unremarkable, and the proximal fibula was not examined. He returned four days later with increased pain, at which time examination revealed tenderness along the proximal fibula and medial ankle. Roentgenograms showed widening of the medial joint space of the ankle and an oblique crush of the proximal fibula. The patient underwent open reduction and internal fixation of the syndesmosis and repair o

Role of Lateral Ankle Ligaments in Vertical Stability of the Fibula: A Cadaveric Model.

BACKGROUND: In unstable ankle fractures, the role of the deltoid and syndesmosis ligaments has been widely studied. However, it is uncertain what the importance of the lateral ankle ligament complex (LALC) is in the vertical stability of the fibula. Given its anatomical position, it should prevent the proximal translation of the fibula. This study aims to evaluate the role of the LALC in stabilizing the fibula in the vertical plane. METHODS: Eleven below-knee cadaveric specimens were used in this study. Proximal traction of the fibula was performed by applying 50 N in the intact state and aft

Effects of isolated Weber B fibular fractures on the tibiotalar contact area.

Fractures of the lateral malleolus can occur without rupture of the deltoid ligament or fracture of the medial malleolus. Controversy exists regarding the necessity of surgery on supination-external rotation stage II ankle fractures. Theoretically, as long as the medial structures are intact, the talus cannot displace enough to cause degenerative arthritis of the ankle joint. The purpose of this study was to measure changes in contact area between the tibial plafond and the talar dome with serial displacement of the distal fibula in both a lateral and a superolateral direction. Twelve cadaver

[Minimally invasive fixation of distal fibular fractures with intramedullary nailing].

OBJECTIVE: Minimally invasive osteosynthesis of distal fibula fractures serves as a biomechanically stable and soft-tissue-friendly fixation method in the case of an unstable fracture, poor bone quality, and/or critical soft tissue conditions with restoration of the length, axis and rotation of the distal fibula as well as stabilization of the ankle mortise. The goal is to reduce and stabilize the distal fibular fracture in a quick and stable manner that protects the soft tissues in ankle fractures. INDICATIONS: Unstable malleolar fractures and fracture dislocations; fibular fractures in comb…

Bilateral fibular fractures in a pre-ambulant infant.

Multiple long-bone fractures, particularly bilateral fractures, are of moderate specificity for inflicted injury (physical abuse) in infants and young children. Bilateral healing fractures of the fibulae are rare and, depending on age, raise the suspicion of inflicted injury. We report healing undisplaced fractures of both fibulae, in almost identical positions, in a pre-ambulant infant. The caregivers reported that the infant repeatedly banged his legs against the metal frame of his playpen. A video of this mechanism was provided to the instructed radiology expert and showed that the point of

The reconstruction of tibial metaphyseal comminution using hybrid frames in severe tibial plafond fractures.

OBJECTIVES: To evaluate the treatment alternatives for the management of the metaphyseal tibial comminution in severe plafond fractures, and to investigate the role of the fibula fracture fixation. DESIGN: Retrospective clinical study. SETTING: Level-2 trauma hospital. PARTICIPANTS: Patients with highly comminuted tibial plafond fractures. INTERVENTION: All patients were treated with open reduction and internal fixation of the articular surface and external fixation of the metaphyseal fracture. If metaphyseal comminution was minimal, bone graft was applied and the fibular was plated (group

Proximal fibula resection in the treatment of bone tumours.

BACKGROUND AND OBJECTIVES: We present a large study of patients with proximal fibula resection. Moreover we describe a new classification system for tumour resection of the proximal fibula independent of the tumour differentiation. METHODS: In 57 patients the functional and clinical outcomes were evaluated. The follow-up ranged between six months and 22.2 years (median 7.2 years). The indication for surgery was benign tumours in ten cases and malignant tumours in 47 cases. In 13 of 45 patients, where a resection of the lateral ligament complex was done, knee instability occurred. In 32 patien

Transfibular excision of distal tibial interosseous osteochondroma with reconstruction of fibula using Sofield's technique - A case report.

Osteochondromas arising from the interosseous border of the distal tibia and involving distal fibula are uncommon. Considering its proximity to the ankle joint, early excision of this deforming distal tibial osteochondroma is done to avoid the future risk of pathological fracture of the distal fibula, ankle deformities and syndesmotic complications. We present a 16-year-old young girl with thinning and deformed distal fibula, secondary to an osteochondroma arising from the distal tibia which was managed with transfibular excision of mass and reconstruction of distal fibula using square nail by

Reconstruction of the Femoral Diaphysis Using the Vascularized Fibula Flap: A Case Report.

The vascularized fibula flap is often utilized in various reconstructive surgeries to cover a bone or soft tissue defect due to its generous length, adequate vascularization, and the possibility of removal with minimal loss of limb function. We present a 32-year-old male patient with a 27-cm femoral diaphysis defect caused by a high-impact car accident, which led to multiple femoral fractures and was further complicated by osteomyelitis. Initial consultations with other physicians advised limb amputation as the sole approach due to the severity of the case and potential complications. Upon pre

Dislocation of the distal tibiofibular joint.

A case report of a patient with posterior dislocation of an intact fibula at the distal tibiofibular joint is presented. This rare injury is a variant of a Bosworth fracture, which is a posterior dislocation of the fibula usually accompanied by an oblique fibular fracture. The injury was initially treated by open reduction of the fibular dislocation, repair of avulsed lateral and deltoid ligaments, and placement of a syndesmotic screw. At one year postoperative, the patient has a poor result secondary to talar avascular necrosis and secondary degenerative ankle arthritis.

Deltoid ligament repair versus trans-syndesmotic fixation for bimalleolar equivalent ankle fractures.

INTRODUCTION: The role of deltoid ligament repair is controversial in the treatment of bimalleolar equivalent ankle injuries. Our purpose was to compare midterm functional outcomes and reoperation rates of unstable distal fibula fractures treated with open reduction internal fixation (ORIF) of the fibula and either deltoid ligament repair, trans-syndesmotic fixation, or combined fixation. METHODS: Skeletally mature subjects were retrospectively identified after fixation of isolated unstable distal fibula fractures treated at a single academic level 1 hospital from January 2005 to May 2019. Maisonneuve fracture.

INTRODUCTION: Maisonneuve fracture (MF) is a generally known entity in ankle trauma. However, details about this type of injury can be found only rarely in the literature. For these reasons we have decided to perform a study on MF epidemiology and pathoanatomy. METHODS: The group comprised 70 patients (47 men, 23 women), with the mean age of 48 years, who sustained an ankle fracture-dislocation involving the proximal quarter of the fibula. Ankle radiographs in three views and lower leg radiographs in two views were performed in all patients. A total of 59 patients underwent CT examination in

Tibiofibula Transposition in High-Energy Fractures.

We report two cases of failed attempts at closed reduction of high-energy tibial fractures with an associated fibula fracture. The first case was a 39-year-old male involved in high-speed motorbike collision, while the second was a 14-year-old male who injured his leg following a fall of three metres. Emergency medical services at the scenes of the accidents reported a 90-degree valgus deformity of the injured limb and both limbs were realigned on scene and stabilized. Adequate alignment of the tibia could not be achieved by manipulation under sedation or anaesthesia. Open reduction and exposu

The impact of plate length, fibula integrity and plate placement on tibial shaft fixation stability: a finite element study.

BACKGROUND: Tibial shaft fractures account for approximately 15% of long bone fractures. Locked plates with minimally invasive plate osteosynthesis techniques are used widely by surgeons. The purpose of this study is to investigate the impact of factors meaning the plate length, fibula integrity, and placement of the plate on the stability of tibial shaft fracture fixation. METHODS: A finite element model of the tibial shaft fracture was built. An axial force of 2500 N was applied to simulate the axial compressive load on an adult knee during single-limb stance.
