Pustulosis palmaris et plantaris chronica et recidivans (pustular bacterid of Andrews) is not an exceptional disease. Its characteristic histological structure is to be considered as a major element of the diagnosis. The fully developed pustule is an oval cavity with transverse long axis, entirely situated within a loca-ly hyperplastic epidermis. Its formation goes through several stages. A 58-year-old woman presented painful chronic inflammation of the outside skin of the bilateral soles. A skin biopsy revealed chronic inflammation of the dermis and subcutaneous tissue, as well as the marked dilatation and proliferation of capillaries and veins, and the thick deposition of elastic fibers around the thick-walled veins. 3-D contrasted computed tomography of the lower extremities demonstrated early bilateral imaging of the veins, and fistulas of the peroneal as well as posterior tibial artery and vein of the bilateral soles. On the basis of a diagnosis of bilateral acroangioderma

The diagnosis of pain and other foot and ankle disorders can be facilitated by using a three-step approach during patient assessment. With perseverance and patience, the physician can diagnose all but the most uncommon foot problems. Most foot disorders can be managed successfully, either conservatively or surgically. Referral for surgery should not be considered a last resort; surgery can often alleviate problems easily, quickly, and permanently.

INTRODUCTION: Onychomycosis is a frequently occurring fungal infection that affects the nails of the fingers and toes. Infection is caused by yeasts, dermatophytes, and nondermatophyte molds. Onychomycosis should be confirmed by mycology. Diagnosis can be made by various techniques, such as microscopy, fungal culture, potassium hydroxide, polymerase chain reaction, and histopathology. This review was undertaken to study the challenges associated with topical therapy of onychomycosis and techniques to overcome these challenges.
