Predicting 90-day falls and unplanned medical visits in nursing home residents using rapid nursing scores including calf circumference, grip strength, Timed Up and go (TUG) test, weight change, and appetite: a prospective observational cohort study protocol.

BACKGROUND: Falls are the main health and safety threat faced by older adults in nursing homes, which can cause serious injuries and increase the risk of death. At present, there is a lack of simple fall risk screening tools that are applicable to the care practices of nursing homes and take into account both muscle function and nutritional status. Develop and validate a rapid scoring tool based on care accessibility indicators [calf circumference, grip strength, up/Walk timing Test (TUG), weight change and appetite] to predict the composite risk of falls or unplanned medical visits (emergency

[Functional and clinical outcomes in patients admitted to nursing homes after hip fractures. Implementation of a multi-level intervention program].

BACKGROUND AND OBJECTIVE: The aim of this study was to determine the clinical and functional outcomes of patients discharged to nursing homes after a hip fracture. METHODS: The study included all patients admitted to a group of nursing homes after a hip fracture in 2016. A geriatric assessment protocol was applied, and patients were treated with a specific protocol for 90 days. They were assessed for nutritional status (Mini-Nutritional Assessment and Body Mass Index), pain (Visual Analogue Scale, and the PAINAD Scale), the presence of pressure ulcers, blood test (D vitamin, haemoglobin, prot

Betwixt and between: workplace perspectives on work reintegration in the eldercare sector in Denmark.

PURPOSE: Contemporary return-to-work (RTW) policies in Denmark and other welfare nations recommend employees on long-term sick leave, due to physical or mental health problems, to RTW gradually. Factors that influence the process of work reintegration (WR) is well documented, however, co-workers experiences of this process are a rather new research topic. Moreover, in the context of the present research, no studies have so far explored the workplace as an arena for social interaction. The aim of this study was to explore co-workers' experiences of the reintegration process and how these experi

'People ring because they're frightened': findings from a realist evaluation on the impact of timely responsive care at home at the end of life.

BACKGROUND: Rapid response services (RRS) support patients who wish to die at home, providing flexible, timely, and specialist care. These services are regionally variable yet are valued by patients and caregivers in often uncertain end-of-life situations. Research on their effectiveness and implementation to date is limited. This study explores how RRS are experienced in practice and identifies key contexts and mechanisms underpinning their impact. OBJECTIVES: This study aimed to understand how different service models of RRS function, who they work for, how and why. By exploring patient, ca

Community Preferences for Allied Health Services in Residential Aged Care.

INTRODUCTION: Exploring health consumer preferences in care is an essential foundational, and ongoing activity, when designing and delivering models of care. We undertook a study to explore: (i) what allied health (AH) services are most important to health consumers and (ii) how health consumers expect to access these services in residential aged care (RAC) to determine consumer priorities in future AH models of care in RAC. METHODS: A mixed method study was conducted with aged care residents and community members (friends or family of residents/people who believe they may use RAC services).

"A normality for us without confinement for them": notes on the associations of families of the mentally ill.

Associations of families of the mentally ill arose in Italy in the early 1980s mainly in the country's northern and central regions. They denounced the inadequacies of an extramural service conceived of as an out-patient clinic filling the void left by the shutting down of asylums. The positions elaborated by these associations focus on four main themes: the distribution of the "burden of care" for the ill between services and families; the ill-conceived interpretation of the families' needs owing to the professional's alleged tendency to "point the finger of guilt" at the families themselves;

Nursing support of family caregivers.

The relationship between caregiver and elder is a unique one. It cannot be compared to a mother-infant dyad; there is no role reversal. Family caregiving is undertaken out of a sense of filial obligation on the family's part coupled with the needs of the elderly person. The relationship that emerges is based on the dependency of one adult on another and is functional for the family and for society at large. We need families to continue to care for their elderly members at home and families need us to insure their survival. We must constantly be aware that care needs increase over the years, wh

[Did entry into nursing home during the health crisis favor the emergence of traumatic symptoms in the elderly?: Entry into nursing home and Covid].

Our aim is to explore the possible emergence of traumatic symptoms and the identity-related repercussions of the restrictions on elderly, who entered into nursing homes during the Covid-19 health crisis in France. Twenty-five subjects institutionalised before the health crisis and twenty-six subjects institutionalised during the periods of lockdown into nursing homes completed scales assessing anxiety-depressive symptomatology, traumatic symptoms and identity. Anxiety and depression symptoms were similar between the groups. The institutionalised group showed a significantly higher prevalence o

Transport of fecal bacteria by boots and vehicle tires in a rural Alaskan community.

People living without piped water and sewer can be at increased risk for diseases transmitted via the fecal-oral route. One rural Alaskan community that relies on hauling water into homes and sewage from homes was studied to determine the pathways of fecal contamination of drinking water and the human environment so that barriers can be established to protect health. Samples were tested for the fecal indicator, Escherichia coli, and the less specific indicator group, total coliforms. Shoes transported fecal contamination from outside to floor material inside buildings. Contamination in puddles

Manual ability as a marker of dependency in geriatric women.

Little is known about the factors responsible for the institutionalization of the elderly. This study's objective was to discover markers associated with nursing home placement. Fifty-six white ambulatory women over 63 yrs of age were selected from 3 settings encompassing a range of dependency: 20 women from nursing homes; 16 women who lived in their own homes but requiring social assistance and 20 totally independent women. Each woman was examined for factors that might be associated with her state of dependency: age, socioeconomic status, morale, education, strength of social network, number

Exploring workplace violence among home care workers in a consumer-driven home health care program.

Nominal research has examined sexual harassment and workplace violence against home care workers within consumer-driven home care models such as those offered in Oregon. This study examined home care workers' experiences of violence while providing care to consumer employers, the patients who hire and manage home care workers. Focus groups and interviews were conducted in Oregon with 83 home care workers, 99 Oregon Department of Human Services (DHS) employees, and 11 consumer employers. Home care workers reported incidents of workplace physical violence (44%), psychological abuse (65%), sexual

Attachment representation in institutionalized children: a preliminary study using the child attachment interview.

UNLABELLED: The experience of being removed from one's home and the transition to a residential care system pose enormous challenges for a child. Substantial evidence has been found regarding severe developmental effects due to early exposition to extreme psychosocial and affective deprivation. The research on Bowlby's theoretical proposals has highlighted the link between insecure, disorganized and atypical attachment patterns and children both living in foster care facilities and adopted out of those institutions. The goal of this pilot study is to investigate the attachment representation i

Urine culture doubtful in determining etiology of diffuse symptoms among elderly individuals: a cross-sectional study of 32 nursing homes.

BACKGROUND: The high prevalence of bacteriuria in elderly individuals makes it difficult to know if a new symptom is related to bacteria in the urine. There are different views concerning this relationship and bacteriuria often leads to antibiotic treatments. The aim of this study was to investigate the relationship between bacteria in the urine and new or increased restlessness, fatigue, confusion, aggressiveness, not being herself/himself, dysuria, urgency and fever in individuals at nursing homes for elderly when statistically considering the high prevalence of asymptomatic bacteriuria in t

Effectiveness of Mindfulness-based Approaches on Stress, Depression, and Serum Cortisol among Elderly in Old Age Homes.

BACKGROUND: Mental health issues, particularly stress and depression, are highly prevalent among the elderly, especially those residing in old age homes. Elevated serum cortisol levels are associated with prolonged stress and depression, leading to further health complications. This study evaluates the effectiveness of a mindfulness-based approach in reducing stress, depression, and serum cortisol levels among elderly individuals in institutional care. MATERIALS AND METHODOLOGY: This quasi-experimental study was conducted in two old age homes in Chennai. A total of 48 elderly participants (ag

Adherence and barriers to the vitamin D and calcium supplement recommendation at Danish nursing homes: a cross-sectional study.

BACKGROUND: Nursing home residents are in high risk of vitamin D deficiency, which negatively affects bone health. Vitamin D and calcium supplements haves shown to increase bone density and reduce fracture risk. Therefore, The Danish Health Authority recommends all nursing home residents a daily supplement of 20 μg vitamin D and 800-1000 mg calcium. However, adherence to the recommendation and knowledge of it is unknown. The aims of this study were to investigate adherence, knowledge, and potential barriers to this recommendation in Denmark. METHODS: A cross-sectional electronic survey was co

PATCH: posture and mobility training for care staff versus usual care in care homes: study protocol for a randomised controlled trial.

BACKGROUND: Residents of care homes have high levels of disability and poor mobility, but the promotion of health and wellbeing within care homes is poorly realised. Residents spend the majority of their time sedentary which leads to increased dependency and, coupled with poor postural management, can have many adverse outcomes including pressure sores, pain and reduced social interaction. The intervention being tested in this project (the Skilful Care Training Package) aims to increase the awareness and skills of care staff in relation to poor posture in the older, less mobile adult and highl

Indoor mite allergens in patients with respiratory allergy living in Porto, Portugal.

We investigated the levels of mite allergens (Der p 1, Der f 1, Der 2, and Lep d 1) in dust samples from the homes of 59 patients with asthma, 36 sensitized to house-dust mites (HDM) and 23 to grass pollen (controls), living in Porto, northern Portugal. The relationship between exposure and sensitization to HDM and the influence of housing conditions on mite-allergen levels were also evaluated. Der p 1 (median 9.2 micrograms/g) and Der 2 (4.6 micrograms/g) were the main allergens, while Der f 1 and Lep d 1 levels were always < 1 microgram/g dust and undetectable in 11% and 47% of samples, resp

The effects of GAMotion (a giant exercising board game) on physical capacity, motivation and quality of life among nursing home residents: A pilot interventional study.

BACKGROUND: In 2017, our team highlighted promising results of a giant exercising board game on physical activity level and a broader array of physical and psychological outcomes among nursing home residents. However, some improvements of this game were needed to make it more suitable for nursing homes and more challenging in terms of exercises. Therefore, we decided to develop a new version of a giant exercising board game: the GAMotion. OBJECTIVES: The primary objective of this pilot study was to assess the impact of the GAMotion on physical capacity among nursing home residents. The second
