Revisiting the development of the atrioventricular conduction axis.

BACKGROUND: The precise steps involved in development of the atrioventricular conduction axis remain incompletely understood, despite over a century of anatomical and histological studies. Recent advances in imaging, and access to human embryonic and fetal heart specimens, offer new insights into this process. OBJECTIVE: This study aimed to clarify the morphological remodeling and insulation of the atrioventricular conduction axis during late embryonic and early fetal human heart development, with emphasis on the fate of nodoventricular and fasciculo-ventricular pathways. METHODS: Serial his

Congenital disorders of cardiac rhythm and conduction.

Electrical instability of the heart has numerous congenital origins. This review approaches the problems from a morphological standpoint, utilizing selected examples from my own studies of the human cardiac conduction system performed during the last three decades. Subjects discussed include multifocal Purkinje cell tumors, benign congenital polycystic tumors of the AV node, several types of congenital heart block, postnatal morphogenesis of the AV node and His bundle (including considerations of persistent fetal dispersion and crib death), the Wolff-Parkinson-White syndrome, left superior ven

Interventricular septal thickness on cardiac computed tomography as a novel risk factor for conduction disturbances in patients undergoing transcatheter aortic valve replacement.

AIMS: We examined whether thickness of the basal muscular interventricular septum (IVS), as measured by pre-procedural computed tomography (CT), could be used to identify the risk of conduction disturbances following transcatheter aortic valve replacement (TAVR). The IVS is a pivotal region of the electrical conduction system of the heart where the atrioventricular conduction axis is located. METHODS AND RESULTS: Included were 78 patients with severe aortic stenosis who underwent CT imaging prior to TAVR. The thickness of muscular IVS was measured in the coronal view, in systolic phases, at 1

The fibrous skeleton in the human heart: embryological and pathogenetic considerations.

The fibrous skeleton of the human heart is composed of several parts which are formed from different, mainly extracardiac sources. The atrioventricular valve rings are formed by invagination of sulcus tissue at the atrioventricular and bulboventricular transitions. The atrial part of the central fibrous body is formed from an ingrowth of tissue from the dorsal mesocardium, the ventricular part from ingrowth of sulcus tissue towards an endocardial structure, the left bulbar ridge. During valve development the atrioventricular endocardial cushions, initially situated between the developing atria

Left bundle branch pacing in alternating bundle branch block.

We present a case of symptomatic intermittent AV block showing during monitorization alternating bundle branch block. Presuming a high need of pacing, conduction system pacing was considered a more physiological alternative in this patient. Left bundle branch pacing restored a stable atrioventricular synchrony with a paced QRS complex similar to the native one and might be a good alternative in these cases.

Sudden death in a patient with lamin A/C gene mutation and near normal left ventricular systolic function.

Mutations in the lamin A/C gene seem to be important aetiological factors in familial DCM. Heart disease caused by lamin A/C gene mutations is characterised by conduction system disorders with the need for permanent pacemaker implantations, atrial fibrillation, severe heart failure, and increased risk for sudden cardiac death. We described an asymptomatic 28-year-old man with a R190W lamin A/C gene mutation and mild left ventricular enlargement and near normal left ventricular ejection fraction who suffered from sudden cardiac death during sleeping. His electrocardiogram did not show conductio

Feasibility of Left Bundle Branch Area Pacing via a Persistent Left Superior Vena Cava in a Patient with Repaired Ventricular Septal Defect.

Persistent left superior vena cava (PLSVC) is the most common systemic venous anomaly and may complicate cardiac device implantation, particularly in patients with a history of congenital heart surgery. We report the case of a 16-year-old girl with a surgically repaired ventricular septal defect who developed complete heart block, necessitating conduction system pacing (CSP). This case highlights the feasibility of CSP in the presence of PLSVC and prior congenital heart surgery.

More than meets the eye: Using AI to identify reduced heart function by electrocardiograms.

Electrocardiographic (ECG) assessment of patients with suspected heart disease is a bedrock of cardiology for diagnosing conduction system disease, arrhythmias, and heart attack. Now, using AI-assisted interpretation of ECGs, the signals within these studies are able to tell us so much more. In their recent randomized trial published in Nature Medicine, Yao and colleagues illustrate the power of utilizing AI-enabled ECGs to identify individuals with reduced heart function using a scalable, pragmatic approach.

Catheter-induced right bundle branch block: Practical implications for the cardiac electrophysiologist.

The right bundle branch (RBB), due to its endocardial course, is susceptible to traumatic block caused by "bumping" during right-heart catheterization. In the era of cardiac electrophysiology, catheter-induced RBB block (CI-RBBB) has become a common phenomenon observed during electrophysiological studies and catheter ablation procedures. While typically transient, it may persist for the entire procedure time. Compared to pre-existing RBBB, the transient nature of CI-RBBB allows for comparative analysis relative to the baseline rhythm. Furthermore, unlike functional RBBB, it occurs at similar h

Clinical outcomes of left bundle branch area pacing: Prognosis and specific applications.

Cardiac pacing has become a widely accepted treatment strategy for bradyarrhythmia and heart failure. However, conventional right ventricular pacing (RVP) has been associated with electrical dyssynchrony, which may result in atrial fibrillation and heart failure. To achieve physiological pacing, Deshmukh et al. reported the first case of His bundle pacing (HBP) in 2000. This strategy was reported to have preserved ventricular synchronization by activating the conventional conduction system. Nonetheless, due to the anatomical location of the His bundle (HB), several issues such as high pacing t

The spectrum of inter- and intraventricular conduction abnormalities in patients eligible for cardiac resynchronization therapy.

Although cardiac resynchronization therapy (CRT) has clearly demonstrated its clinical benefit in patients with congestive heart failure (CHF) and intraventricular conduction abnormalities, selection of eligible patients and/or optimal pacing site are still a matter of debate. The aim of the study was to analyze the spectrum of conduction abnormalities in CRT candidates. A total of 26 patients (mean age 62 +/- 9 years) with CHF and conduction disturbances (QRS > or = 130 ms) were studied. The underlying heart disease was dilated cardiomyopathy (DCM) (n = 12) or coronary artery disease (CAD) (n

Nonorgan specific autoantibodies and heart damage.

Heart damage, mediated by different autoantibodies can involve several anatomical heart structures: valves, arteries, conduction tissue. Verrucous endocarditis is frequently reported in patients with antiphospholipid syndrome (APS) with or without systemic lupus erythematosus (SLE), particularly if they suffer from central nervous system involvement. Antiphospholipid antibodies (aPL) were shown deposited at subendothelial level of the affected valves. According to several in vitro and in vivo experimental models, aPL, anti-oxidized LDL (oxLDL), anti-heat shock protein 65 (HSP65) and anti-endot

Comparison of acute effects of anthracyclines on cardiac electrophysiological parameters of isolated guinea-pig hearts.

This study was performed to evaluate the acute effects of two anthracycline derivatives, doxorubicin and 4'O-tetrahydropyranyl-doxorubicin [(THP)-doxorubicin], on the conduction intervals, heart rate and refractoriness of isolated spontaneously beating guinea-pig hearts using a high-resolution ECG recording technique (SST-ECG). Doxorubicin as well as (THP)-doxorubicin were added to the perfusate in increasing concentrations of 0.1, 1 and 10 microM. Doxorubicin did not significantly alter the heart rate or conduction intervals. Only the rate-dependent QT interval was significantly shortened und

A review of cardiac autonomics: from pathophysiology to therapy.

The effective management of cardiovascular diseases requires knowledge of intrinsic and extrinsic innervation of the heart and an understanding of how perturbations of said components affect cardiac function. The innate cardiac conduction system, which begins with cardiac pacemaker cells and terminates with subendocardial Purkinje fibers, is modulated by said systems. The intrinsic component of the cardiac autonomic nervous system, which remains incompletely elucidated, consists of intracardiac ganglia and interconnecting neurons that tightly regulate cardiac electrical activity. Extrinsic com

Peptidic regulation of heart rate and interactions with the autonomic nervous system.

Autonomic influences on the heart rate have been the subject of intense research for many decades and are classically devoted to the sympathetic and parasympathetic systems. However, developments over the past few years in our knowledge of the organization of the autonomic nervous system have led to the conclusion that in addition to the classical transmitters, peptidic transmitters are clearly present and have direct or indirect actions on cardiac conduction. Neuropeptides have been found to collocate with each other or with classical transmitters, thereby increasing the variety of chemical s

Nuclear envelope lamin-related dilated cardiomyopathy: a case series including histopathology.

BACKGROUND: Lamin A/C gene (LMNA) mutations cause myocardial fibrosis manifesting as arrhythmogenic, non-compaction, or dilated cardiomyopathies. Fibro-fatty replacement largely involves the conduction system and conduction disease commonly occurs prior to contractile dysfunction. CASE SUMMARY: Two young, unrelated Caucasian males, aged 34 and 25, were referred to our centre for treatment of advanced heart failure. Both patients had a family history of heart failure and sudden cardiac death among their first-degree relatives and were diagnosed with Lamin A/C mutations, but they had not been s
