Myopathy With Exercise-Induced Intolerance due to Novel Biallelic Variants in OBSCN-A Clinical, Morphological and Molecular Analysis.

The phenotype of OBSCN variants consists of exercise intolerance ranging from myalgia and cramps to rhabdomyolysis. Symptoms are mainly induced by high-intensity sports. Molecular analysis showing a deregulation of muscle processes associated with Ca2+ regulation, extrasarcolemmal integrity and autophagy emphasised the critical role of obscurin in skeletal muscle function.

Tarui disease and distal glycogenoses: clinical and genetic update.

Phosphofructokinase deficiency (Tarui disease) was the first disorder recognized to directly affect glycolysis. Since the discovery of the d … ore than one hundred patients have been described with prominent clinical symptoms characterized by muscle cramps, exercise intolerance, rhabdomyolysis and myoglobinuria, often associated with haemolytic anaemia and hyperuricaemia. The muscle phosphofructokinase gene is located on chromosome 12 and about 20 mutations have been described. Other glycogenoses hav …

Detecting muscle strain in professional football players through plasma myoglobin levels.

BACKGROUND: Muscle issues such as soreness, cramps, and strains are common consequences of sports exercise, making it critical to accurately identify the nature of muscle discomfort. It is important for not only medical professionals but also athletes and sports enthusiasts to independently identify and manage the risk of muscle strain. METHODS: This prospective observational study explores the effectiveness of measuring blood myoglobin (MB) levels as a method for detecting muscle strains among athletes. The study involved 30 male professional footballers monitored over one season. MB levels

Current concepts in dystrophinopathies.

Dystrophinopathies comprise a group of hereditary muscle disorders characterized by progressive wasting and weakness of skeletal muscle, as a result of degeneration of muscle fibers, and can be distinguished by the mode of transmission, age at onset and pattern of muscle weakness. The range of phenotypes associated with the region Xp21 has been expanding since identification of the gene in 1987. The mild end of the spectrum includes the phenotype of the muscle cramps with myoglobinuria and isolated quadriceps myopathy, while at the severe end, there are progressive muscle diseases that are cla…

Peripheral Nerve Hyperexcitability Syndromes.

PURPOSE OF REVIEW: This article provides a review of the clinical phenotypes and evaluation of peripheral nerve hyperexcitability syndromes. These rare diagnoses include cramp-fasciculation syndrome, Isaacs syndrome, and Morvan syndrome. Recent investigations have led to an understanding of the autoimmune underpinnings of these conditions and their specific associated antibodies. As the presentation of peripheral nerve hyperexcitability syndromes includes muscle stiffness, twitches, and spasms, which are also shared with certain central nervous system and myopathic conditions, the differential

Task specific grip force control in writer's cramp.

OBJECTIVE: Writer's cramp is defined as a task specific focal dystonia generating hypertonic muscle co-contractions during handwriting resulting in impaired writing performance and exaggerated finger force. However, little is known about the generalisation of grip force across tasks others than writing. The aim of the study was to directly compare regulation of grip forces during handwriting with force regulation in other fine-motor tasks in patients and control subjects. METHODS: Handwriting, lifting and cyclic movements of a grasped object were investigated in 21 patients and 14 controls. T

[Fasciculations and cramps: physiological bases and clinical approach of a complex phenomenon].

Fasciculations and cramps originate in the motor unit, a functional unit that includes the lower motor neuron and their innervated muscle fibres. Both are common complaints in outpatient practice. These symptoms can be secondary to neurological or medical pathology, presenting a broad differential diagnosis and a complex approach. Recent neurophysiological studies have increased the knowledge of their origin mainly in amyotrophic lateral sclerosis. The symptomatic management of fasciculations and cramps depends on their etiology and includes pharmacological and non-pharmacological treatments.

Each patient was observed for intradialytic complications, including muscle cramps and hypotension, during 12 sessions of hemodialysis. RESULTS: We included 26 participants with a mean age of 14.23 years undergoing hemodialysis. Carnitine deficiency was revealed in 54.8% of our participants. Also, there was no significant correl …

[Cutaneous and muscular unmyelinated afferent fibres. Clinical, histological and experimental study. Possible explanation of muscular cramps (author's transl)].

Unmyelinated afferent fibres are the most numerous of the whole afferent component. Their function in nociception is now well established. However some recent data allow to think that their function is much more complex. Cutaneous unmyelinated afferences are evaluated in controls and in 3 groups of patients (painful neuropathies, painless neuropathies with hypesthesia, diabetic neuropathies). Amyelinic myelinic ratio is not in accordance with the gate control theory. Muscular afferent fibres are studied, especially the lateral gastrocnemius soleus nerve of cat. Their predominance is obvious. O

[A case of Isaacs' syndrome causing various central nervous symptoms successfully treated with high-dose intravenous methylprednisolone therapy].

A 44-year-old man with a bilateral hand tremor suffered from a decline in concentration and abnormal vision for several months. He also complained of easily falling down because of muscle stiffness and cramps in his lower limbs. On admission, he demonstrated lower limb stiffness, muscle cramps, diplopia, hyperhidrosis, left upper limb ataxia and dysesthesia in all limbs. Laboratory examination showed a marked elevation in his serum creatine kinase level (26,890 U/l), and needle electromyography demonstrated myokymic discharges in the muscles of his lower extremities. Isaacs' syndrome was diagn…

Tc-99m pyrophosphate muscle labeling in McArdle syndrome.

This paper reports the findings on two patients with McArdle syndrome (myophosphorylase deficiency) in whom conventional bone scans with Tc-99m pyrophosphate revealed intense muscle labeling following exercise tests. The temporal pattern observed was similar to that seen with other types of muscle damage. The prolonged cramps often occurring with this entity appears to produce muscle damage that is readily demonstrable using conventional bone-scanning techniques.

Cramps following exercise.

A 13-year old boy presented with a 10-year history of severe muscle cramps experienced an hour after prolonged exercise. There was no history of exercise intolerance or myoglobinuria. A muscle biopsy showed a lipid myopathy and a deficiency of muscle carnitine palmityl transferase. He has responded to a high carbohydrate, low fat diet with added carbohydrate intake preceding extensive exercise. Diagnosis of this entity before an episode of rhabdomyolysis is unusual.

Calf enlargement associated with neurologic disease: two uncommon cases.

Muscle enlargement and hypertrophy are rare findings in neurogenic lesions. The two in combination have been reported in cases of peripheral nerve lesions, polyneuropathy, and poliomyelitis. True and pseudo muscle hypertrophy are the two possible etiologies, whereas infiltration, stretch, or exercise of the muscle are the causative factors. We report two cases of unilateral calf enlargement, one occurring after surgery for S1 radiculopathy with associated cramping, and the other after poliomyelitis.

MLIP-Related Myopathy: Two Unreported Pathogenic Variants - A Case Report.

INTRODUCTION: The muscle disorder known as myopathy with myalgia, increased serum levels of creatine kinase, and with or without episodic rhabdomyolysis-1 (MMCKR1) (MIM: 620138) is a MLIP-related myopathy characterized by muscle cramps and stiffness on exertion with a variable age at onset, cardiac involvement, episodic spikes of high levels of CK, and rhabdomyolysis. Electromyography shows myopathic changes, and muscle biopsy shows nonspecific myopathic and degenerative features. To date, around 15 cases have been reported worldwide. CASE PRESENTATION: Here, we describe 2 cases with MLIP-rel…
