Combined cataract extraction and trabeculectomy: phacoemulsification compared with extracapsular technique.

Thirty-four eyes undergoing phacoemulsification and trabeculectomy (PHACO + TRAB) were followed prospectively for 1 year postoperatively. The results of surgery were compared with those, obtained retrospectively, of the previous 34 extracapsular cataract extractions with trabeculectomy (ECCE + TRAB) performed by the same surgical team. All eyes received posterior chamber intraocular lens implants. Preoperatively, the two groups did not differ significantly in terms of age, gender distribution, or degree and duration of glaucoma and cataract. Mean postoperative follow up for the ECCE+TRAB group…

Endothelial cell loss following "modern" phacoemulsification by a senior resident.

Historically, extracapsular cataract extraction (ECCE) has been shown to cause less endothelial cell loss than phacoemulsification. To further evaluate endothelial cell loss following phacoemulsification, we prospectively studied 60 patients who underwent cataract extraction with "in situ" phacoemulsification within the capsular bag following a continuous-tear circular capsulotomy, with placement of a posterior chamber intraocular lens (PC-IOL) in the capsular bag. All procedures were performed by a single senior ophthalmology resident. The average preoperative endothelial count was 2318 +/- 3…

Phacoemulsification And Zonular Weakness: Contribution Of The Capsular Tension Ring With A Thread.

PURPOSE: Cataract surgery in patients suffering from zonular weakness can be difficult and lead to severe complications. The insertion of capsular tension rings to stabilize the capsular bag along all steps of the phacoemulsification is an option, although can be difficult to remove intra- or post-operatively. A capsular tension ring with a thread is a safe alternative, which allows its easy removal at the end of the surgery. PATIENTS AND METHODS: Retrospective study of regular practice on 14 patients with zonular weakness who underwent phacoemulsification. Among them, 12 suffered from pseudo…

[Influence of cataract phacoemulsification on eye hydrodynamics in patients with prior trabeculectomy].

At present, phacoemulsification and trabeculectomy are the most widespread surgery methods in cataract and glaucoma treatment, generally recognized as the gold standard. Among elderly patients, comorbid glaucoma and cataract are extremely prevalent, so the order of the two surgeries and the time interval between them are pivotal choices when planning treatment strategy. The reason for it is that almost any eye surgery is considered a risk factor for long-term trabeculectomy effectiveness. In attempts to solve this problem, numerous studies have been conducted on the impact of cataract surgery …

[Individualised correction of presbyopia by the use of phacoemulsification: monovision and multifocal lenses].

The cataract operation has transformed from a procedure for correcting spherical and astigmatic errors to one for correcting even presbyopia. Higher demands by the patients and more and more complex and individual lifestyle options need customised concepts of presbyopic correction, taking also into account comorbidities and neuroadaption. One concept for achieving this goal is multifocal lenses, undergoing a renaissance these days. Monovision using monofocal lenses is a cost neutral alternative with very few side effects, if well performed. It is defined by the far focus of one eye and the nea
