Enzyme replacement therapy reduces the risk for wheelchair dependency in adult Pompe patients.

BACKGROUND: Pompe disease is a rare metabolic myopathy. In adult patients, progressive weakness of limb-girdle and respiratory muscles often leads to wheelchair and respirator dependency. Clinical studies have shown enzyme replacement therapy (ERT) to positively affect motor and respiratory outcomes. Here we investigate whether ERT reduces patients' risk of needing a wheelchair or respirator. METHODS: Data were collected as part of a prospective international survey, the IPA/Erasmus MC Pompe survey, which was conducted annually between 2002 and 2016. We excluded patients who were already usin

Helmet noninvasive mechanical ventilation in patients with acute postoperative respiratory failure.

BACKGROUND: The physiological and clinical effects of noninvasive ventilation (NIV) on acute postoperative respiratory failure are relatively unknown. The aim of this study was to determine the prediction factors for failure in the use of NIV with a helmet in this context. METHODS: This was a prospective observational study. The use of NIV was assessed for a period of 2 years in a postoperative ICU. Demographic data were collected, as well as acute respiratory failure (ARF) and arterial gas readings. Hemodynamic changes were assessed using pulse contour cardiac output technology, and the clin

Successful application of helmet non-invasive ventilation in a parturient with acute respiratory distress syndrome.

Acute respiratory distress syndrome (ARDS) is an important cause of morbidity and mortality during pregnancy. The case of a twin pregnant woman in her 28th week who developed infection-related ARDS, undergoing a cesarean section for premature membrane rupture is described. It was performed epidural anaesthesia and helmet non-invasive ventilation (NIV) during the postoperative period. The combination of epidural anesthesia with NIV helped to restore physiological gas-exchange and to prevent common complications associated with a more invasive approach.

Noninvasive continuous positive airway pressure in acute respiratory failure: helmet versus facial mask.

OBJECTIVE: Noninvasive continuous positive airway pressure (nCPAP) is applied through different interfaces to treat mild acute respiratory failure (ARF) in infants. Recently a new pediatric helmet was introduced in clinical practice to deliver nCPAP. The objective of this study was to compare the feasibility of the delivery of nCPAP by the pediatric helmet with delivery by a conventional facial mask in infants with ARF. PATIENTS AND METHODS: We conducted a single-center physiologic, randomized, controlled study with a crossover design on 20 consecutive infants with ARF. All patients received

Modified N95 mask delivers high inspired oxygen concentrations while effectively filtering aerosolized microparticles.

STUDY OBJECTIVE: In a pandemic, hypoxic patients will require an effective oxygen (O2) delivery mask that protects them from inhaling aerosolized particles produced by others, as well as protecting the health care provider from exposure from the patient. We modified an existing N95 mask to optimize O2 supplementation while maintaining respiratory isolation. METHODS: An N95 mask was modified to deliver O2 by inserting a plastic manifold consisting of a 1-way inspiratory valve, an O2 inlet and a gas reservoir. In a prospective repeated-measures study, we studied 10 healthy volunteers in each of

Long-term pulmonary complications in combatants exposed to mustard gas: a historical cohort study.

BACKGROUND: Sulphur mustard (mustard gas), the most widely used chemical agent in the Iran-Iraq war, affects many organs including the skin, the gastrointestinal and respiratory tracts, and the central nervous system. The aim of this study was to assess the cumulative incidence rate and annual incidence rate of pulmonary complications, and the rate ratio of related factors. METHODS: In a retrospective cohort study of 1337 soldiers with a history of mustard gas exposure, factors such as age, smoking habit, number of exposure episodes, and the use of gas masks were determined, together with an

Effect of protective filters on fire fighter respiratory health during simulated bushfire smoke exposure.

BACKGROUND: Bushfire fighters are potentially subject to risks from bushfire smoke. Although many different protective masks and filters are available, it is not clear which is the most effective from a health and safety perspective. The effect of protective filters on the respiratory health of Western Australian urban career fire fighters under controlled simulated conditions is investigated. METHODS: Sixty-four healthy Fire and Emergency Services Authority of Western Australia (FESA) urban career fire fighters were subjected to controlled simulated bushfire smoke in an open smoke chamber fo

Air contaminants encountered by firefighters.

The concentrations of eight air contaminants suspected of causing acute and chronic health problems for firefighters were measured in over 200 fires in the City of Boston using a personal air sampler. Threatening concentrations of both carbon monoxide and acrolein were found in a small proportion of the fires. Less hazardous levels of hydrogen chloride, hydrogen cyanide, nitrogen dioxide and carbon dioxide were also noted. Benzene was found in most fires, but at concentrations well below those expected to cause acute injury. The air sampling data have application in treatment of smoke inhalati

The development and implementation of a low-cost mechanical ventilator in a low-middle-income country during the COVID-19 pandemic: The Unisabana-HERONS.

BACKGROUND: The COVID-19 pandemic in Latin America generated the need to develop low-cost, fast-manufacturing mechanical ventilators. The Universidad de La Sabana and the Fundacion Neumologica Colombiana designed and manufactured the Unisabana-HERONS (USH) ventilator. Here, we present the preclinical and clinical study results to evaluate its effectiveness and safety characteristics in an animal model (Yorkshire Sow) and five patients with acute respiratory failure receiving mechanical ventilatory support for 24 h. METHODS: The effectiveness and safety outcomes included maintaining arterial b

The addition of a membrane oxygenator to a ventricular assist device in a patient with acute respiratory distress syndrome.

A 12-year-old boy with Marfan's syndrome required a biventricular assist device (VAD) after an aortic root replacement. The patient developed acute respiratory distress syndrome and required escalating ventilator support. We hypothesized that the addition of a membrane oxygenator in series with the assist device would improve gas exchange and allow for a more lung-protective ventilator approach. A membrane oxygenator was placed in series with the right VAD resulting in a blood path of right atrium to VAD to oxygenator to pulmonary artery. Circuit function was gauged by monitoring flow and oxyg

Continuous positive airway pressure ventilation with helmet in infants under 1 year.

OBJECTIVE: To report the feasibility of helmet use in infants between 1 and 12 months old with acute respiratory failure. DESIGN AND SETTING: Observations were made before and 2 h after helmet CPAP of 6 cm H(2)O. Failure was defined as recourse to intratracheal ventilation. Patient stabilization or improvement was defined as a variation <10% or a decrease >10% in one of the following: respiratory rate, inspired oxygen fraction, or capillary partial pressure of CO(2). Tolerance was assessed by the pain and discomfort score, the systematic search for pressure sores, and the measurement of helme

Mechanical ventilation strategies in the management of congenital diaphragmatic hernia.

Most infants with congenital diaphragmatic hernia (CDH) require respiratory support. The goal of this report is to present an overview of mechanical ventilation strategies in the management of infants with CDH. The anatomic and physiologic limitations in the lungs of infants with diaphragmatic hernia make decisions on the best strategy and use of mechanical ventilation challenging. We will briefly review lung development in infants with CDH, identifying factors that provide a basis for lung protection strategies. Background on the use of specific mechanical ventilation modes and the rationale

Chronic respiratory failure in patients with neuromuscular diseases: diagnosis and treatment.

Neuromuscular diseases affect alveolar air exchange and therefore cause chronic respiratory failure. The onset of respiratory failure can be acute, as in traumas, or progressive (slow or rapid), as in amyotrophic lateral sclerosis, muscular dystrophies, diseases of the myoneural junction, etc. Respiratory muscle impairment also affects cough efficiency and, according to the current knowledge regarding the type of treatment available in Brazil to these patients, it can be said that the high rates of morbidity and mortality in these individuals are more often related to the fact that they cough

Solvent removal of beryllium from surfaces of equipment made of beryllium copper.

Exposure to beryllium compounds, both by inhalation and skin contact, may result in immune sensitization and chronic beryllium disease. The objective of the present research work was to study the feasibility of removing beryllium compounds from the surfaces of devices made of Be-Cu alloy and to estimate the frequency at which the surfaces had to be rubbed in order to evaluate the likelihood that beryllium can be removed from the surfaces by serial wipe sampling at concentrations exceeding the US Department of Energy (DOE) standard limit of 0.2 microg per 100 cm2. The standard limit was exceede

Design and development of a novel media-separated valve for flow-controlled expiration during mechanical ventilation.

Mechanical ventilation has become a central therapeutic means in modern medicine. However, mechanical ventilation carries the risk of ventilator-induced lung injury. Our group introduced flow-controlled expiration (FLEX) as an additional approach to effective, lung-protective mechanical ventilation. By gradually reducing the pneumatic resistance, FLEX controls expiratory flow toward a user-set constant target, replacing the exponential flow-time profile of a conventional expiration. Primarily developed to demonstrate the physiological concept, the initial FLEX-system prototype did not provide

Exposure assessment of airborne acrylamide for occupationally exposed workers by using an isotope-dilution gas chromatography coupled with mass spectrometry.

OBJECTIVES: To develop a highly sensitive analytical method for very low acrylamide (AA) exposure and to conduct an occupational exposure assessment by the developed method. METHODS: Seventy-five air samples from four plants were collected and analyzed using an isotope-dilution gas chromatography coupled with mass spectrometry (GC/MS). RESULTS: This isotope-dilution GC-MS method is sufficiently sensitive for assessing very low AA level as 4.37 ng m(-3), which is 10- to 7500-fold lower than the current analytical method. Field study showed that most airborne AA was gaseous rather than particu

Airborne contact dermatitis and asthma in a nail art operator.

Nail art (creation and decoration of artificial nails) is a growing fashion trend. Proportionally to its popularity, the number of allergic reactions to the materials used has recently increased. We report the case of a nail art operator who developed occupational allergy to acrylates, manifested with the unusual simultaneous presence of asthma and dermatitis. Cutaneous lesions only affected areas not covered by individual protection devices or clothes, even if such areas were not in direct contact with acrylates, suggesting airborne allergic contact dermatitis. While respiratory problems were

Technological advances in avalanche survival.

Over the last decade, a proliferation of interest has emerged in the area of avalanche survival, yielding both an improved understanding of the pathophysiology of death after avalanche burial and technological advances in the development of survival equipment. The dismal survival statistics born out of the modern era of winter recreation unmistakably reveal that elapsed time and depth of burial are the most critical variables of survival and the focus of newer survival devices on the market. Although blunt trauma may kill up to one third of avalanche victims, early asphyxiation is the predomin
