Surgery for thoracic myelopathy caused by ossification of the ligamentum flavum.

BACKGROUND: Ossification of the ligamentum flavum overlying the lower thoracic spine frequently produces myelopathy. This study analyzed the postoperative outcomes after decompressive laminectomy for thoracic OLF. METHODS: We retrospectively studied 13 patients (10 male, 3 female; mean age, 58 years; range, 39-69). The mean follow-up duration was 66 months (range, 21-107). All patients had undergone decompressive laminectomy and excision of the OLF. The clinical course was evaluated according to the Frankel grading system and JOA scores. The number of vertebral segments demonstrating OLF, the

Surgery for synchronous double cancer in the hypopharynx and thoracic esophagus.

Problems involved in the surgical treatment of synchronous double cancer comprising hypopharyngeal and thoracic esophageal cancers were considered from the aspects of the radicality of treatment and the quality of life following surgery. Advanced esophageal cancers were treated by resection with lymph node dissection via right thoracotomy, and early cancers by transhiatal esophagectomy or endoscopic mucosal resection. When the entire thoracic esophagus was extirpated, the gastric tube was usually used for reconstruction. When the hypopharyngeal cancer extended to the higher oropharynx or when

Society for Vascular Surgery clinical practice guidelines of thoracic endovascular aortic repair for descending thoracic aortic aneurysms.

Thoracic aortic diseases, including disease of the descending thoracic aorta (DTA), are significant causes of death in the United States. Open repair of the DTA is a physiologically impactful operation with relatively high rates of mortality, paraplegia, and renal failure. Thoracic endovascular aortic repair (TEVAR) has revolutionized treatment of the DTA and has largely supplanted open repair because of lower morbidity and mortality. These Society for Vascular Surgery Practice Guidelines are applicable to the use of TEVAR for descending thoracic aortic aneurysm (TAA) as well as for other rare

Nerve injury: injury to the recurrent laryngeal, phrenic, vagus, long thoracic, and sympathetic nerves during thoracic surgery.

Nerve injury occurs in about 1% of patients undergoing thoracic surgery. Most commonly, it is the result of surgical trauma specific to the area of operation. Alternatively, it may be due to retraction on adjacent structures during the exposure of complex surgical procedures. This article reviews the most common types of nerve injury and discusses the pathophysiology, etiology, diagnosis, and treatment of each.

Thoracic paraplegia due to missed thoracic compressive lesions after lumbar spinal decompression surgery. Report of three cases.

The authors discuss the cases of three patients in whom thoracic paraplegia developed after lumbar spinal decompressive surgery for slight lumbar spinal canal stenosis. Careful computerized tomography myelography and magnetic resonance imaging examination of the thoracic spine revealed another compressive lesion (spinal cord tumor, disc herniation, osteophyte of vertebral body, and ossification of the ligamentum flavum). Additional thoracic decompressive surgery provided partial amelioration of each patient's neurological condition. The authors suggest that to avoid such a complication physica

The diagnostic value of thoracic ultrasound in thoracic surgery consultations: a new tool in pleural effusion decision-making.

INTRODUCTION: Thoracic ultrasound (US) is a practical tool in radiology that has been commonly used for decades. Its daily clinic use in thoracic surgery is rising, and scientific evidence is needed to support its use. AIM: We aimed to determine the diagnostic value of handheld, portable thoracic ultrasound scanning (HH-US) in patients referred to the thoracic surgery outpatient clinic with suspected pleural effusion and compare the diagnostic role with chest X-rays (CXR) or computed tomography (CT). MATERIAL AND METHODS: We analyzed the patients between 2020 and 2023 who were referred to ou…

Surgeries for Patients with Tandem Spinal Stenosis in Cervical and Thoracic Spine: Combined or Staged Surgeries?

BACKGROUND: Cervical and thoracic tandem spinal stenosis (ct-TSS) is a rare yet challenging degenerative disease. When the diagnosis is made, surgical decompression is indicated for both lesions. However, literature about the surgical approaches and prognosis of this disease is lacking. METHODS: From March 2005 to April 2013, 30 patients with ct-TSS and a mean age of 49.8 years were recruited. We outlined 2 types of ct-TSS lesions-adjacent and skip lesions. The surgical approach for ct-TSS with adjacent lesions was combined cervical and thoracic decompression via a single posterior incision;
