Persistent air leak and failure of the lung to expand completely after closed thoracostomy complicate the course of spontaneous pneumothorax in patients with AIDS. In such cases, attempts to discontinue chest tube drainage may fail. The following is a case of a patient with AIDS and spontaneous pneumothorax who responded to chemical pleurodesis with tetracycline.

An unusual patient with chylothorax following blunt chest trauma is presented. Diagnosis was, in this acute case, clear after thoracostomy. In our patient conservative therapy with total parenteral nutrition failed and he was cured with a surgical closure of the duct leak. Chylothorax can be the cause of important morbidity and mortality, and straightforward diagnosis and therapy are mandatory.

Pigtail pleural catheters are becoming more popular because they are a less painful and equally efficacious alternative to the traditional large-bore thoracostomy tube. This report describes the case of a 48-year-old male whose deteriorating condition required the placement of a pigtail pleural catheter for treatment of parapneumonic pleural effusion. Spontaneous and traumatic pneumothorax are most often treated with chest tube (CT) thoracostomy. However, it appears that small-bore drainage systems have similar success rates with lower complications, pain, and discomfort for the patient. We present the description of the ultrasound-guided technique for pneumothorax drainage with an 8.3-French pigtail catheter (PC) in a case series of 10 patients.

Spontaneous hemothorax is rare, with limited data available on its etiology and treatment. We report a case of massive spontaneous hemothorax with a ruptured variceal phrenic vein during pregnancy, likely a complication of the Kasai procedure. Despite closed thoracostomy, the patient's symptoms and imaging findings did not improve. Emergent open thoracotomy and bleeding control were performed.
