"Think nerve" in upper extremity reconstruction.

Upper extremity reconstruction may be greatly facilitated by "thinking nerve" in the initial evaluation. Frequently, the obvious soft tissue deformity or bone or joint pathology distracts the reconstructive surgeon from an underlying nerve problem. When pain is a portion of the presenting complaints, thinking nerve becomes more obvious. This article attempts to direct the reconstructive surgeon approaching the upper extremity to look for underlying nerve injury in the region of a damaged wrist capsule, neuromas of the cutaneous nerve, and sites of nerve entrapment.

[Ulnar nerve lesions at the elbow--clinical aspects and surgical treatment using medial subperiosteal epicondylectomy].

Ulnar neuropathy is a well recognized clinical entity caused by a variety of pathological lesions around the elbow and the sulcus olecrani. Numbness and paraesthesia in the little finger and medial half of the fourth digit are the most common complaints. Weakness of the small muscles of the hand innervated by the ulnar nerve, and a positive Tinel's sign are other features. Generally the diagnosis is confirmed by an electro-physiological study. We have described the technique of epicondylectomy and discussed the reasons why we believe it to be preferable to anterior transposition.

Tuberculoma of the ulnar nerve: case report.

A peripheral nerve tuberculoma, the third on record, involved the ulnar nerve, as did the other two. This case report is followed by a discussion of the pathogenesis and diagnosis of this lesion. Its pathogenesis is unclear. Direct infection from a tuberculous sister was disproved, and infection via a local wound is highly unlikely. A previous, silent form of tuberculosis seems to have been the cause.

Ulnar nerve entrapment at the elbow. A surgical series and a systematic review of the literature.

Chronic compression of the ulnar nerve at the elbow is the second most common entrapment neuropathy. Various surgical options have been described. Timing of surgery is also debated. In this study we report the long-term results of a consecutive surgical series of anterior subcutaneous transpositions and review the pertinent literature. Sixty consecutive patients underwent anterior subcutaneous transposition at our Institution to treat ulnar nerve compression at the elbow. McGowan scale was used in the neurological exam before surgery. Bishop rating system was used to assess outcome. Seventy-ei

Supercharged end-to-side anterior interosseous nerve transfer to restore the intrinsic motor function in osteoarthritis-induced cubital tunnel syndrome.

BACKGROUND: There have been no reports on treating motor deficits caused by ulnar nerve palsy (cubital tunnel syndrome) in patients with elbow osteoarthritis, except for traditional ulnar nerve management around the elbow joint. Herein, we report favorable outcomes of supercharged end-to-side anterior interosseous nerve (AIN) transfer to the motor branch of the ulnar nerve in patients with concurrent elbow arthritis based on a retrospective case series. METHODS: From 2019 to 2022, 27 patients with high ulnar nerve lesions showing intrinsic muscle wasting, paralysis, and weakness in the hand w

Compressive neuropathy in the upper limb.

Entrampment neuropathy or compression neuropathy is a fairly common problem in the upper limb. Carpal tunnel syndrome is the commonest, followed by Cubital tunnel compression or Ulnar Neuropathy at Elbow. There are rarer entities like supinator syndrome and pronator syndrome affecting the Radial and Median nerves respectively. This article seeks to review comprehensively the pathophysiology, Anatomy and treatment of these conditions in a way that is intended for the practicing Hand Surgeon as well as postgraduates in training. It is generally a rewarding exercise to treat these conditions beca

Anatomy of the ulnar tunnel and the influence of wrist motion on its morphology.

PURPOSE: To examine the anatomy of the ulnar tunnel, with emphasis on the pisohamate arcade and pisohamate hiatus, and study the influence of wrist kinematics on the morphology of these structures. METHODS: Ten fresh-frozen cadaveric hands were dissected. The dimensions and relationships of the ulnar tunnel, pisohamate arcade, and pisohamate hiatus were recorded. The effect of 4 wrist positions on these dimensions and relationships was investigated. RESULTS: The ulnar tunnel has 3 compartments--proximal, middle, and distal relative to the pisiform-with variable morphologies, dimensions, and

The effect of long-distance bicycling on ulnar and median nerves: an electrophysiologic evaluation of cyclist palsy.

BACKGROUND: Distal ulnar neuropathies have been identified in cyclists because of prolonged grip pressures on handlebars. The so-called cyclist palsy has been postulated to be an entrapment neuropathy of the ulnar nerve in the Guyon canal of the wrist. Previous studies utilizing nerve conduction studies have typically been either case reports or small case series. HYPOTHESIS: Electrophysiologic changes will be present in the ulnar and median nerves after a long-distance multiday cycling event. STUDY DESIGN: Cohort study; Level of evidence, 2. METHODS: A total of 28 adult hands from 14 subje

[Total ulnar nerve paralysis due to acute traumatic aneurysm at the forearm].

We are reporting a case of complete ulnar nerve paralysis due to compression from a false aneurysm located in the distal forearm. The aneurysm developed after the patient stabbed himself with a meat hook. The ulnar paralysis developed after a 14-day period of latency. The diagnosis was made using detailed patient history, clinical examination, and a sonogram. Early surgical decompression of the nerve led to complete restitution of all functions.

Delayed ulnar neuropathy at the wrist following open carpal tunnel release.

Open carpal tunnel release is a common and successful treatment of median neuropathy at the wrist (carpal tunnel syndrome). We report a case of delayed ulnar neuropathy at the wrist with onset 2 months after open carpal tunnel release. Clinical findings, electrophysiological studies, magnetic resonance imaging, and surgical exploration demonstrated ulnar nerve compression at Guyon's canal resulting from translocation of the carpal tunnel contents. To our knowledge, this is an unreported complication of open carpal tunnel release that merits wide appreciation.

Compression and entrapment neuropathies.

Peripheral nerve entrapments are frequent. They usually appear in anatomical tunnels such as the carpal tunnel. Nerve compressions may be due to external pressure such as the fibular nerve at the fibular head. Malignant or benign tumors may also damage the nerve. For each nerve from the upper and lower limbs, detailed clinical, electrophysiological, imaging, and therapeutic aspects are described. In the upper limbs, carpal tunnel syndrome and ulnar neuropathy at the elbow are the most frequent manifestations; the radial nerve is less frequently involved. Other nerves may occasionally be damage

Presurgical management of ulnar nerve entrapment in patients with and without diabetes mellitus.

Diabetes mellitus (DM) is a risk factor for the development of ulnar nerve entrapment (UNE). Differences in surgical outcomes for UNE between patients with and without DM have been reported, but studies on presurgical management are scarce. This study aimed to evaluate the presurgical management of UNE in patients with (DM diagnosis < 2 years of UNE diagnosis) and without DM regarding the level of care for diagnosis and the proportion that proceeds to surgery. Data from 6600 patients with UNE between 2004 and 2019 were included from the Skåne Health Care Register (SHR) and cross-linked with da

T2-signal of ulnar nerve branches at the wrist in guyon's canal syndrome.

OBJECTIVE: To evaluate T2-signal of high-resolution MRI in distal ulnar nerve branches at the wrist as diagnostic sign of guyon's-canal-syndrome (GCS). MATERIALS AND METHODS: 11 GCS patients confirmed by clinical/electrophysiological findings, and 20 wrists from 11 asymptomatic volunteers were prospectively included to undergo the following protocol: axial T2-weighted-fat-suppressed and T1-weighted-turbo-spin-echo-sequences (3T-MR-scanner, Magnetom/Verio/Siemens). Patients were examined in prone position with the arm extended and wrist placed in an 8-channel surface-array-coil. Nerve T2-signa

A suspected case of ulnar tunnel syndrome relieved by chiropractic extremity adjustment methods.

BACKGROUND: There has been little published about ulnar tunnel syndrome (UTS) as it relates to the practice of chiropractic, despite chiropractors' apparent interest in nerve compression syndromes and a growing trend toward providing chiropractic extremity care. This syndrome is not very common and could be mistaken for carpal tunnel syndrome by practitioners who are not aware of the differences. OBJECTIVE: To discuss the case of a patient with ulnar tunnel syndrome whose symptoms were resolved by chiropractic extremity adjustment. Clinical features A 45-year-old female patient complained of

The relation between cubital tunnel syndrome and the elbow alignment.

The authors reviewed forty-three surgically managed cases of cubital tunnel syndrome treated during the past twelve years. Surgery had been performed on thirty-one men and twelve women. Their average age was 46.8 years old. The etiology of cubital tunnel syndrome had been determined as follows: There were twenty-three cases of osteoarthritis of the elbow joint. There were four cases of isolated cubitus valgus deformity. There were three cases of isolated cubitus varus deformity. There were three cases of rheumatoid arthritis. One patient had sustained trauma to the cubital tunnel. In nine othe

Cubital tunnel syndrome caused by amyloid elbow arthropathy in long-term hemodialysis patients: report of 4 cases.

Carpal tunnel syndrome occurs frequently in long-term hemodialysis patients. However, the literature contains few detailed reports of other nerve entrapment syndromes of the upper extremity in these patients. We encountered 4 cases in which cubital tunnel syndrome occurred in long-term hemodialysis patients. In all cases, a hypertrophic synovial mass projecting from the humeroulnar joint compressed the ulnar nerve, and Congo red staining revealed that the mass contained amyloid deposition. Synovial proliferation resulting from amyloid arthropathy of the elbow joint appears to be the primary ca

Compression of the ulnar nerve and spasm of the ulnar artery in Guyon's canal caused by a hypermobile pisiform bone.

We describe compression of the ulnar nerve at Guyon's canal caused by a hypermobile pisiform bone and associated with spasm of the ulnar artery. Treatment included excision of the pisiform bone, and repair of the flexor carpi ulnaris, hypothenar musculature, and periosteum. Postoperatively, the patient reported complete relief of symptoms, which had still been maintained at final follow-up one year later.
