Elbow hemiarthroplasty for acute distal humeral fractures and their sequelae: medium- and long-term follow-up of 41 cases.

BACKGROUND: The earliest distal humeral hemiarthroplasty (or elbow hemiarthroplasty [EHA]) implants, which date back to the late 1940s, were performed in patients with severe elbow joint injuries as an alternative to arthrodesis. After some clinical reports and case studies with a short follow-up, published in the 1990s, a new "anatomically convertible" EHA model was introduced in 2005 and became a common surgical option to treat complex elbow fractures and their sequelae. We describe the mid- and long-term outcomes of EHA performed to treat acute intra-articular fractures or their sequelae.

Bedside ultrasound detection of long bone fractures.

INTRODUCTION: Rapid identification of fractures of long bones is prudent, since associated bleeding and neurovascular compromise can cause significant morbidity and mortality. We describe two patients who presented to the Emergency Department with orthopedic trauma and underwent bedside ultrasound of the affected extremity. Ultrasound rapidly revealed fractures in both patients, and led to early treatment and disposition. MATERIALS AND METHODS: Rapid bedside ultrasound of long bones was performed on affected extremities and compared to X-rays. CONCLUSION: Bedside ultrasonography of long bone

A case in which a bone fragment caused by a bullet made a second channel in addition to the bullet channel.

A 64-year-old male was found dead in his house with his face covered with blood and a 38-caliber revolver between his legs. He had been suffering from type 2 diabetes mellitus and aftereffects of cerebral infarction. Autopsy revealed a normal round contact wound in the left lateral cervical region. A bullet from the firearm had entered through the left lateral cervical region and traveled to the outer right sternocleidomastoid muscle. This also triggered another wound from the fifth cervical vertebra to the muscle tissue near the right cartilage thyroid. At the end of this channel, there were

Excision of a bony spike without fixation of the fractured clavicle in a jockey.

Various modalities are available for treatment of a clavicle fracture. Conservative treatment is successful in most cases. Open reduction, when indicated, usually is accompanied by fixation of the fracture. In the case of a jockey with a fractured clavicle, the fracture was complicated by tenting of the skin. The fracture was opened and a bony spike was excised extraperiosteally without doing internal fixation of the fracture to avoid skin necrosis and allow early return to work. The patient returned to his profession 2 weeks after surgery. Radiologic and clinical examinations revealed a satis

Non-free osteoplasty of the mandible in maxillofacial gunshot wounds: mandibular reconstruction by compression-osteodistraction.

We have treated 33 young men with medium to large (3-8 cm) bony and soft tissue defects of the lower third of the face caused by gunshot wounds. After debridement, collapsing the proximal segments for primary approximation of soft and hard tissues and a closed osteotomy of a small fragment of mandible, we used an original compression-distraction device, designed in 1982 and tested during 1983 (analogous devices were absent at that time) to reposition the mandible and cause callus to form (during distraction) between the fragment and to use the remaining stumps of bone to fill in the defect. [Minimally invasive fixation of the pelvic ring with a transiliacal internal fixator].

PURPOSE OF THE STUDY: To evaluate the results of stabilisation of the posterior pelvic segment with a transiliacal internal fixator (TIFI) in type C fractures of the pelvis. MATERIAL AND METHODS: Between 2008 and 2011, the TIFI method was used in 27 patients with unstable type C pelvic ring fractures (AO classification) at our department. The patient group included 15 men and 12 women with an average age of 35 years (range, 15 to 65 years). All patients suffered high-energy trauma in traffic accidents (59.3%), by falls from a height (33.3%) or due to other causes (7.4%). In 40.7% of the patie

Complications of articular distal humeral fracture fixation: a systematic review and meta-analysis.

BACKGROUND: Distal humeral fractures are relatively rare injuries in adults. Fractures that involve the articular surface can be particularly challenging to treat. Open reduction-internal fixation (ORIF) remains the preferred treatment for most intra-articular distal humeral fractures, depending on the degree of comminution and functional demands of the patient. Many surgical approaches, fixation techniques, and potential complications have been described in the literature; however, the relative incidence and associated characteristics of these complications have not been studied. The purpose

Pediatric T-Condylar Humerus Fractures: A Systematic Review.

BACKGROUND: The pediatric T-condylar humerus fracture is different from its adult counterpart, and its rarity makes general consensus for treatment algorithms difficult to define. Pediatric orthopaedic surgeons tend to think of this fracture as a supracondylar humerus fracture with intra-articular extension. The transition age at which this injury resembles the adult distal humerus fracture and less so the pediatric supracondylar humerus fracture with intra-articular extension is unclear. The goal of this study is to synthesize the literature and identify factors associated with good and poor

Mono segmental fixation of selected types of thoracic and lumbar fractures; a prospective study.

AIM OF STUDY: A prospective study to evaluate the results of monosegmental fixation; fixation of the fractured level with the adjacent vertebra sharing the same disc, in selected types of lumbar and thoracic fractures. This technique aims at saving motion levels by fusion of the only affected motion segment without sacrificing other levels. METHODS: Forty patients enrolled in this study between August 2011 and October 2013. The inclusion criteria were recent thoracic or lumbar vertebral fractures (less than 2 weeks). The fracture involves only one of the end plates of the vertebrae (either th

Staged screw removal in external plate fixation for distal tibial fractures: a finite element study on adaptive biomechanics.

High-energy distal tibial fractures present treatment challenges due to poor soft tissue coverage and risk of delayed union. External locking plates offer stability while preserving biology, yet the optimal strategy to dynamically modulate fixation rigidity in accordance with healing progression remains unclear. We constructed finite element models of a comminuted distal tibial fracture incorporating callus at four healing stages (0, 1, 3, and 6 months). Seven screw configurations and the effect of an intact fibula were evaluated under 800 N axial load. Outcome measures included fracture site

Mini-Screws-Only Fixation Method for Small Fragments of Medial Malleolus Fractures.

BACKGROUD: Open reduction and internal fixation is the standard treatment for a displaced medial malleolus fracture (MMFx), achieving ankle stability and bony union to prevent post-traumatic arthritis. Previous fixation techniques including tension band wiring and unicortical screw fixation are not optimal for fixation of small fragments in MMFx due to their small size and poor manipulability. Here, we describe a novel surgical method using mini-screws only for fixation of small fragments in MMFx. METHODS: We conducted a retrospective consecutive study of patients who underwent surgery using

Combined Medial Column Arthrodesis with Open Reduction Internal Fixation of Central Column for Treatment of Lisfranc Fracture-Dislocation: A Review of Consecutive Cases.

Lisfranc fracture-dislocations can be devastating injuries with significant long-term sequelae with or without surgical intervention. The main goal of treatment is to minimize the common long-term complications including pain, progressive arch collapse, degenerative joint disease, hardware failure, and reoperation. Partial primary fusion involving the first, second, and third tarsometatarsal joints has become a common approach for primarily dislocation injuries, with open reduction and internal fixation (ORIF) favored for Lisfranc injuries involving fracture. ORIF commonly requires revision su
