A 59-year-old woman had persistent sepsis after abdominal operations because of a volvulus and subsequently a retroperitoneal abscess, in spite of antibiotic treatment against Klebsiella pneumoniae, which grew in blood cultures. In order to maintain antegrade ejaculation after retroperitoneal lymphadenectomy (RLND) for low stage non-seminomatous germ cell cancer, we implemented a nerve-sparing technique in which the sympathetic nerves were preserved. Thirty-eight patients were clinically staged: 27 were stage I and 11 were either IIa or IIb. Laparotomy revealed that in 2 patients the nerve-sparing technique was not feasible. Anastomotic false aneurysm (AFA) of the aorta or iliac artery is a potentially lethal complication of prosthetic grafts. To study this complication, the records of 18 patients with 22 noninfected AFAs (15 aortic and seven iliac) were reviewed. Patients with an intact AFA had a pulsatile abdominal mass, abdominal pain, an occluded graft, peripheral emboli, or a femoral anastomotic false aneurysm. Ultrasound is the method of choice in the detection and differentiation of solid or cystic masses in the abdomen. The differential diagnosis of anechoic and hypoechoic lesions comprises a large variety of benign and malignant diseases or abnormalities. This paper describes a number of such lesions and, in addition, discusses the possible differential diagnoses so far reported in the literature. An attempt is made to describe the different features of such lesions. Most of them are rare but have to be considered if a lesion of unknown origin or clinical relevance is detected by ultrasound.

Patients with durable left ventricular assist devices pose special problems for management in the setting of COVID-19 infection. We describe the successful management of a 44-year-old man with severe COVID-19 infection and HeartMate 3 left ventricular assist device. His course was complicated by cytokine storm and COVID-19-associated coagulopathy. We describe our institutional protocol for managing COVID-19 infection in patients on mechanical circulatory support, focusing on the need for a thoughtful, multidisciplinary approach.

PURPOSE: For the past few years, anterior exposure for surgery of the lumbar spine has gained popularity for the treatment of disk disease or spondylosis. Cancellous bone remains the gold standard for fusion. Iliac crest bone harvesting is safe but there are donor site complications. Bone substitutes exist, like recombinant human bone morphogenic protein-2 rhBMP-2. This alternative offers a high rate of fusion but with local and general complications. The aim of our study is to show the feasibility of an endopelvic approach for iliac bone crest harvesting to avoid donor site complication.

The indication for use of laparoscopy, in the pediatric population, was initially for diagnostic purposes. As confidence with the technology and utility grew, it began to be applied for therapeutic indications. With equivalent surgical outcomes and decreased morbidity, the usefulness of a laparoscopic approach became more apparent, and today minimally invasive surgery is an indispensable tool in the management of many pediatric urologic conditions. The management of renal pathologies using laparoscopy is now currently the approach of choice for most pediatric renal maladies.

BACKGROUND: Retroperitoneoscopic renal pedicle lymphatic disconnection has been described in the management of intractable chyluria.
METHODS: We retrospectively reviewed the clinical outcomes of 76 patients with intractable chyluria undergoing renal pedicle lymphatic disconnection via a retroperitoneoscopic (n = 59) or conventional open approach (n = 17). Operative time, intraoperative blood loss, volume of postoperative drainage, postoperative draining time, postoperative intestinal recovery, intraoperative and postoperative complications, and postoperative hospital stay were evaluated.

BACKGROUND: Traumatic spinal cord infarction was initially described in the era previous to the availability of MRI. This entity occurs in children and affects the thoracic spinal cord, usually presenting as a delayed cord injury. Patients have a high incidence of associated blunt thoracic or retroperitoneal trauma and arterial hypotension at admission. The described mechanism of injury is a traumatic occlusion of the aortic branches nourishing the spinal cord. It shares several characteristics with SCIWORA. In past years, the presence of this injury has received little importance.

In a prospective randomized trial, 225 patients with stage IIB nonseminomatous testis tumor after radical retroperitoneal lymph node dissection received 2 versus 4 courses (arms 1 and 2, respectively) of adjuvant chemotherapy with cis-platinum, vinblastine and bleomycin.
