Nonabsorbable Suture Knot on the Tendon Affects Rotator Cuff Healing: A Comparative Study of the Knots on Tendon and Bone in a Rat Model of Rotator Cuff Tear.

BACKGROUND: Nonabsorbable suture knots are usually used to link the tendon and bone during rotator cuff repair surgery. There are many variations in the arthroscopic knot-tying technique; however, the location of suture knot placement for rotator cuff healing has rarely been studied. HYPOTHESIS: The authors compared the rotator cuff healing between knots tied on tendon and bone in a rotator cuff tear rat model. It has been hypothesized that knots can cause chronic inflammation and create the weakest link between tendon and bone, thus affecting rotator cuff healing. STUDY DESIGN: Controlled l

Reconstruction of the Superior Glenoid Labrum With Biceps Tendon Autograft: A Cadaveric Biomechanical Study.

PURPOSE: To describe 2 superior labral reconstruction techniques using long head of the biceps (LHB) autograft and to investigate the ability of the 2 reconstruction techniques to restore superior restraint to the glenohumeral joint compared with superior labrum-deficient models. METHODS: In this biomechanical study, 10 cadaveric shoulders were cycled on a servohydraulic machine while the force required to cause superior subluxation was recorded. Each specimen was cycled under 4 conditions: intact labrum, SLAP tear, posterior (9- to 12-o'clock position) labral reconstruction using LHB autogra

Shoulder Strength Requirements for Upper Limb Functional Tasks: Do Age and Rotator Cuff Tear Status Matter?

Understanding upper limb strength requirements for daily tasks is imperative for early detection of strength loss that may progress to disability due to age or rotator cuff tear. We quantified shoulder strength requirements for 5 upper limb tasks performed by 3 groups: uninjured young adults and older adults, and older adults with a degenerative supraspinatus tear prior to repair. Musculoskeletal models were developed for each group representing age, sex, and tear-related strength losses. Percentage of available strength used was quantified for the subset of tasks requiring the largest amount

Axillary Intraneural Ganglion Cysts.

Aside from affecting the stability of the glenohumeral joint, tears in the joint capsule can give rise to extraneural (paralabral) and, very rarely, intraneural ganglion cysts. This report presents the first two cases of axillary intraneural ganglion cysts in the literature with magnetic resonance imaging. Both cases were incidentally noted to have coexisting lesions (lymphadenopathy from an undifferentiated malignancy and suprascapular nerve entrapment, respectively). This report reinforces the applicability of the articular theory for intraneural ganglion cysts at a novel site.

Partial-thickness tears involving the rotator cable lead to abnormal glenohumeral kinematics.

HYPOTHESIS: The objective of this study was to determine the biomechanical function of the rotator cable when a partial-thickness (>50%) tear is present. We compared intact specimens with partial tears of the anterior cable followed by partial anterior and posterior tears in regard to glenohumeral kinematics and translation. The hypothesis was that partial-thickness tears will lead to abnormal glenohumeral biomechanics, including glenohumeral translation and path of glenohumeral articulation. METHODS: Five fresh frozen cadaveric shoulders with intact labrum, rotator cuff, and humerus were tes

A 70° Arthroscope Significantly Improves Visualization of the Bicipital Groove in the Lateral Decubitus Position.

PURPOSE: The purpose of this study was to quantify the length of the bicipital groove viewable with a 70° arthroscope and to compare this distance visualized with use of a 30° arthroscope in both cadavers and living subjects. METHODS: Diagnostic glenohumeral arthroscopy in the lateral decubitus position was performed on 10 fresh-frozen cadaveric shoulders from a posterior portal. Using 70° and 30° arthroscopes, the distalmost viewable portion of the bicipital groove was percutaneously marked. Dissection of each specimen was then performed, and the distances between the articular margins of th

Scapulohumeral rhythm relative to active range of motion in patients with symptomatic rotator cuff tears.

BACKGROUND: Some patients with rotator cuff tears feel pain without functional limitation, whereas others show a decrease in range of motion. To investigate this distinction, the scapulohumeral rhythm was used to conduct a functional evaluation of shoulder joints' coordination. The objective was to characterize patients according to their active range of motion without pain and their scapulohumeral rhythm compared with healthy individuals. MATERIALS AND METHODS: Fourteen patients with rotator cuff tears and 14 healthy individuals were set up with 35 reflective markers on the trunk and upper l

Arthroscopic fixation of isolated type II SLAP lesions using a two-portal technique.

The purpose of this study was to assess the outcomes of twenty isolated type II SLAP lesions which were repaired arthroscopically using a two-portal technique. Shoulder function was evaluated at a mean follow-up of 2 years using the modified Constant-Murley score. The average preoperative score was 65.2%; it improved postoperatively to 86.4% (p < 0.0001). Pain improved from 6 points to 13 points (p <0.01) and strength increased from 10 kg to 17 kg (p < 0.01). Seventeen of the 20 patients participated in athletic activity before the arthroscopic SLAP repair; they were subdivided into two groups

Surgical exposures of the humerus.

The neurovascular and muscular anatomy about the humerus precludes the use of a truly "safe" fully extensile approach. Working around a spiraling radial nerve at the posterior midshaft requires either a transmuscular dissection or a triceps-avoiding paramuscular technique. To gain maximal exposure, the radial nerve must be mobilized at the spiral groove. For exposure of only the proximal humeral shaft, many surgeons prefer the anterolateral approach because it uses the internervous plane between the axillary and deltoid nerves proximally and the radial and musculocutaneous nerves distally. Pro

Reversed arthroscopic subacromial decompression for massive rotator cuff tears.

This prospective study evaluates the results of a procedure for massive rotator cuff tears that we term reversed arthroscopic subacromial decompression (ASD). The procedure includes an arthroscopic debridement of the subacromial space and glenohumeral joint, an arthroscopic tuberoplasty, and depending on the pathologic condition of the long head of the biceps, a biceps tendon tenotomy. Reversed ASD avoids a classic acromioplasty in order to preserve the integrity of the coracoacromial arch. Twenty-three patients with a mean age of 69 years underwent this procedure. After a mean follow-up of 40

[The phenomena of irradiating pain in rotator cuff tear: quantitative analysis].

The aim of this study was the quantitative analysis of the phenomena of pain radiation in the case of rotator cuff tear. The clinical material concerns 97 patients, age range from 45 to 86 years. Radiating pain was noted in 33 cases; radiation into distal part of arm--12.4%, elbow--9.3%, forearm--6.2%, hand--1%, hand and thumb 2.1%, 5th finger--1.0%, neck--1.0% and into axilla--1.0%. The radiation of pain was provoked by one finger pressure over tender point (84.8%) or by impingement sign (15.2%). Next the impingement test was performed and additionally supplemented by intraarticular lidocaine

Transplantation of teres major muscle for infraspinatus muscle in irreparable rotator cuff tears.

The authors suggest transfer of teres major muscle for a detached and atrophic infraspinatus muscle in irreparable rotator cuff tears. Original studies were done on cadavers. In the period between November 1993 and June 1994, we used this technique on 6 patients. Those patients were evaluated with the Constant-Murley test and improved from an average of 40 points (minimum 27 to maximum 54) before surgery to an average of 62 points (minimum 47 to maximum 78) after surgery. Active external rotation recovered 35 degrees in abduction and 24 degrees in adduction. The teres major transferred on grea

Severity of rotator cuff disorders and additional load affect fluoroscopy-based shoulder kinematics during arm abduction.

BACKGROUND: Rotator cuff disorders, whether symptomatic or asymptomatic, may result in abnormal shoulder kinematics (scapular rotation and glenohumeral translation). This study aimed to investigate the effect of rotator cuff tears on in vivo shoulder kinematics during a 30° loaded abduction test using single-plane fluoroscopy. MATERIALS AND METHODS: In total, 25 younger controls, 25 older controls and 25 patients with unilateral symptomatic rotator cuff tears participated in this study. Both shoulders of each participant were analysed and grouped on the basis of magnetic resonance imaging int

Mini-open rotator cuff repair does not result in intractable stiffness.

We conducted a study to determine whether intractable postoperative stiffness or deltoid damage results from primary arthroscopic anterior acromioplasty and mini-open repair of full-thickness tears of the superior rotator cuff. Eighty-three repairs (80 patients) were available for follow-up at a mean (range) of 57.2 (12-98) months. Range of motion at presentation and motion in contralateral shoulder at final follow-up were used for comparison. Significant improvements were seen in active forward elevation, passive forward elevation, and active external rotation at 90° abduction. Final motion i
