First study of topical selamectin efficacy for treating cats naturally infected with Brugia malayi and Brugia pahangi under field conditions.

Lymphatic filariae are important human and animal parasites. Infection by these parasites could lead to severe morbidity and has significant socioeconomic impacts. Topical selamectin is a semi-synthetic macrocyclic lactone that is widely used to prevent heartworm infection. Up until now, there were no studies that investigated the efficacy of selamectin in lymphatic filariae. Therefore, we aimed to study the chemotherapeutic and chemoprophylactic efficacies of selamectin use for cats in brugian filariasis-endemic areas in Southern Thailand. To assess chemotherapeutic efficacy of topical selame

Topical Use of Mammalian Target of Rapamycin (mTOR) Inhibitors in Tuberous Sclerosis Complex-A Comprehensive Review of the Literature.

BACKGROUND: Tuberous sclerosis complex is a genetically determined multisystem disorder that may affect almost any human organ. The discovery of the mammalian target of rapamycin (mTOR) pathway and its involvement in tuberous sclerosis complex-related pathology has led to the introduction of mTOR inhibitors into clinical practice. Topical administration of mTOR inhibitors for skin lesions related to tuberous sclerosis complex may represent a reasonable alternative for more invasive procedures. A growing number of patients have been described exhibiting positive therapeutic effects from the top

Utilization of Topical Ruxolitinib in Dermatology: A Review.

As systemic administration of Janus kinase-inhibitors is associated with safety concerns, local alternatives, such as topical ruxolitinib, have been developed. This review summarizes utilization of topical ruxolitinib in dermatology. A literature search was performed of studies reporting topical use of ruxolitinib in dermatologic conditions. Twenty-four articles were included, representing 2618 patients. Results show improvement with topical ruxolitinib formulations in atopic dermatitis, vitiligo, psoriasis, and lichen planus. Results are conflicting in alopecia areata. Minimal bioavailability

Successful treatment of Fusarium keratitis with cornea transplantation and topical and systemic voriconazole.

A case of invasive Fusarium keratitis in a previously healthy male patient was treated successfully with cornea transplantation and systemic and topical voriconazole after treatment failure with topical amphotericin B and systemic itraconazole. Topical voriconazole was well tolerated, and, in conjunction with the oral administration, it resulted in a high level of the drug in the anterior chamber of the eye (which was 160% of the plasma drug level).

Rationale for topical antibiotics.

Topical antibiotics are under attack by critics who claim that no proof of efficacy exists and that systemic administration is far more effective. In this paper, we review the existing evidence and conclude that topical antibiotics have a place in the prevention and treatment of superficial skin infections. Our own experimental studies showed that topical antibiotics promptly reduce the density of Staphylococcus aureus in secondarily infected chronic dermatoses, associated with clinical improvement. In addition, topical antibiotics were shown to be highly effective in preventing and suppressin

Topically administered clindamycin in the treatment of acne vulgaris and other dermatologic disorders.

Clindamycin is a macrolide antibiotic that has been used orally and topically in the treatment of acne vulgaris. Unfortunately, oral administration is associated with pseudomembranous colitis in up to 10% of patients; consequently, it is no longer a generally accepted form of acne therapy. Topical application is an effective, safer alternative. Topical formulations may be extemporaneously prepared or purchased as a pre-mixed hydroalcoholic solution. Adverse effects associated with topical preparations are few and mostly minor. Topical clindamycin is also used in the treatment of erythrasma, ro

Antigastrolesive, gastric antisecretory, diarrheagenic and mucus-stimulating effects in rats following topically applied rioprostil, a synthetic prostaglandin E1 analog.

Prostaglandins may have many biological actions including hypotensive and antipeptic ulcer activity. The purpose of this investigation was to determine if the primary alcohol prostaglandin E1 analog rioprostil1 prevents ethanol-induced gastric lesions (antigastrolesive activity), inhibits gastric acid secretion (antisecretory activity), or causes diarrhea in rats when administered topically, and to compare these responses to the effect of rioprostil following enteral (oral or intraduodenal) administration. Rioprostil exhibited antigastrolesive activity in rats when administered either orally o

Effect of Routes of Administration of Tranexamic Acid in Peri-Operative Blood Loss in Total Hip Arthroplasty: A Prospective Comparative Study.

INTRODUCTION: Perioperative blood loss is a significant challenge in total hip arthroplasty (THA), and it often necessitates blood transfusion. Tranexamic acid (TXA), an antifibrinolytic agent, is widely used to reduce surgical blood loss, but to date; there is no clear consensus on the optimal and most efficacious route of administration of TXA. Therefore, this study was planned to evaluate and compare the efficacy of different routes of administration of TXA in reducing perioperative blood loss through a prospective comparative study. MATERIALS AND METHODS: A total of 102 patients undergoin

Oral vs Intravenous vs Topical Tranexamic Acid in Primary Hip Arthroplasty: A Prospective, Randomized, Double-Blind, Controlled Study.

BACKGROUND: Tranexamic acid (TXA) has demonstrated efficacy in reducing blood loss, reduction in hemoglobin, and blood transfusion requirements in primary total hip arthroplasty (THA). The optimal mode of TXA administration for patients undergoing primary THA is unclear. The purpose of this randomized controlled trial is to determine whether oral administration of TXA was superior to intravenous or topical routes in these patients. METHODS: In this double-blinded, placebo-controlled trial, patients undergoing primary THA were randomized to oral (2 g TXA orally 2 hours preoperatively), intrave

Risk of intraocular pressure elevation after topical steroids in children and adults: A systematic review.

Topical steroids may induce a rise in intraocular pressure. The risk may increase with prolonged use, high frequency of administration, young age, higher ocular penetrance and higher anti-inflammatory potency. We aimed to study this relationship by comparing published rates of intraocular pressure elevation following administration of topical steroids and compared the risk of higher versus lower dosage regimes, high- versus low-potency/penetration steroids and adults versus children. Data sources used were Ovid Embase, Ovid Medline, the Cochrane Central Register of Controlled Trials, Web of Sc

Impact of repeated topical-loaded manganese-enhanced MRI on the mouse visual system.

PURPOSE: Optic nerve degeneration in diseases such as glaucoma and multiple sclerosis evolves in months to years. The use of Mn(2+)-Enhanced Magnetic Resonance Imaging (MEMRI) in a time-course study may provide new insights into the disease progression. Previously, we demonstrated the feasibility of using a topical administration for Mn(2+) delivery to the visual system. This study is to evaluate the impact of biweekly or monthly repeated Mn(2+) topical administration and the pH levels of the Mn(2+) solutions for MEMRI on the mouse visual pathway. METHODS: Using groups of mice, the MEMRI with

[Three cases of uveitis induced by mycobacteriosis therapy using rifabutin].

BACKGROUND: Rifabutin has been used clinically to combat tuberculosis as well as nontuberculosis Mycobacterium since 2008 in Japan. Uveitis associated with rifabutin is described in many reports, but none of these are of Japanese. We report three Japanese cases of nontuberculosis Mycobacterium with rifabutin-associated uveitis. CASE 1: A 91-year-old woman developed bilateral uveitis with hypopyon and vitreous opacity at 2 months after the initiation of rifabutin administration. The uveitis persisted regardless of topical administration of corticosteroids and the discontinuance of rifabutin. He

Therapeutic Effects of Intranasal Tofacitinib on Chronic Rhinosinusitis with Nasal Polyps in Mice.

OBJECTIVES: The Janus kinase/signal transducer and activator of transcription (JAK-STAT) pathway play a key role in immune modulation, especially in the polarization of T helper cells. JAK inhibitors reduce inflammation by inhibiting the phosphorylation of STAT. We investigated whether a JAK inhibitor, tofacitinib, can reduce inflammation in a mouse model of chronic rhinosinusitis with nasal polyps (CRSwNP). METHODS: An eosinophilic CRSwNP model was induced using 4-week-old BALB/c mice. The therapeutic effects of topical tofacitinib were compared with the effects of triamcinolone acetonide (T

Attenuation of basilar artery spasm after experimental subarachnoid hemorrhage in rabbit by potassium channel activator cromakalim--a preliminary result.

Cerebral vasospasm associated with aneurysmal subarachnoid hemorrhage (SAH) remains a major complication in patients suffering from SAH. Regulation of membrane potential of arterial smooth muscle through activation or inhibition of potassium (K+) channel activity provides an important mechanism to dilate or constrict arteries. The present study examined the effect of a K+ channel activator, cromakalim, on cerebral vasospasm following experimental SAH. By the route of topical application and intra-arterial injection, basilar arteries were exposed transclivally and measured on-line using videomi

Clinical evaluation of 1% cyclosporine for topical treatment of keratoconjunctivitis sicca in dogs.

Keratoconjunctivitis sicca (KCS) is a prevalent and often vision-threatening condition in dogs. In several reports, 2% cyclosporine (cyclosporin A, CsA) was described as effective in modulating the clinical signs of KCS. This study was designed to compare the efficacy of 1% CsA vs a placebo, using a randomized double-blind clinical trial. Topical administration of 1% CsA significantly improved Schirmer wetting values and subjective markers of corneal health as compared with the placebo. In the dogs treated with topical administration of 1% CsA, the clinical signs of KCS were improved in 81.8%
