Bone cement enhanced pedicle screw fixation combined with vertebroplasty for elderly patients with malignant spinal tumors.

BACKGROUND: Older patients with malignant spinal tumors are difficult to treat because they have many co-morbidities including osteoporosis. The purpose of this research is to discuss the technique and clinical outcome of bone cement enhanced pedicle screw fixation combined with vertebroplasty (the Sandwich Procedure) for elderly patients with severe osteoporosis and malignant spinal tumors. METHODS: This study includes 28 consecutive elderly patients with malignant thoracic or lumbar spinal tumors. There were nine patients with myelomas, and 19 patients with metastatic bone tumors. The Sandw

Dynamic hip screw versus DHS blade: a biomechanical comparison of the fixation achieved by each implant in bone.

We biomechanically investigated whether the standard dynamic hip screw (DHS) or the DHS blade achieves better fixation in bone with regard to resistance to pushout, pullout and torsional stability. The experiments were undertaken in an artificial bone substrate in the form of polyurethane foam blocks with predefined mechanical properties. Pushout tests were also repeated in cadaveric femoral heads. The results showed that the DHS blade outperformed the DHS with regard to the two most important characteristics of implant fixation, namely resistance to pushout and rotational stability. We conclu

Treatment of Jones fracture nonunions and refractures in the elite athlete: outcomes of intramedullary screw fixation with bone grafting.

BACKGROUND: Fractures of the proximal fifth metatarsal metaphysis (ie, the Jones fracture) can be problematic in the elite athlete because of a high incidence of nonunion and refracture with nonoperative treatment. Although these fractures are not common, athletes can suffer refracture or nonunion of a Jones fracture despite operative stabilization. This is often attributable to hardware of insufficient strength, aggressive postoperative rehabilitation, or biologic insufficiency at the fracture site. PURPOSE: The authors review the results of revision intramedullary screw fixation with cancel

The effect of screw position on the initial fixation of a reverse total shoulder prosthesis in a glenoid with a cavitary bone defect.

Patients with rotator cuff tear arthropathy can be treated successfully with a reverse total shoulder prosthesis. In patients with significant glenoid bone loss, achieving stable bone fixation can be challenging, as the surgeon must know when bone grafting is necessary and when the plan to implant the reverse total shoulder prosthesis should be abandoned because of the likelihood of early implant loosening. The purposes of this study were (1) to determine the initial stability of a metal glenoid implant fixed in a glenoid with a central cavitary defect and (2) to determine whether an altered s

Pelvic girdle reconstruction based on spinal fusion and ischial screw fixation in a case of aneurysmal bone cyst.

A case of lytic lesion of the pelvis in a 23-year-old woman is presented. A biopsy led to the diagnosis aneurysmal bone cyst (ABC). Due to the histologically very aggressive growth of the tumor, a low malignant osteosarcoma could not be excluded. In an initial operation the tumour, affecting the sacrum, the iliac crest and the lower lumbar spine was resected. Temporary restabilisation of the pelvic ring was achieved by a titanium plate. The histological examination of the entire tumour confirmed the diagnosis ABC. After 6 months, the MRI showed no recurrence. The observed tilt of the spine to

Assessment of unicortical and bicortical fixation in a quasistatic cadaveric model. Role of bone mineral density and screw torque.

STUDY DESIGN: The stabilizing potential of the Caspar Trapezial Osteosynthetic Plate was studied using unicortical and bicortical screw placement in cadaveric cervical spines. OBJECTIVE: To determine if Caspar plates secured with unicortical screws provide the same degree of stability as those anchored with unlocked bicortical screws. SUMMARY OF BACKGROUND DATA: Previous work has demonstrated that bicortical cervical vertebral screws are superior to unicortical screws in terms of pullout strength and decreased wobble. However, these two methods of plate fixation have not been directly compar

Optimal graft fixation--the effect of gap size and screw size on bone plug fixation in ACL reconstruction.

In reconstruction of the anterior cruciate ligament (ACL), using central one-third patellar tendon graft, an early weak link is the bone plug junction. To study this, we carried out experiments to determine the relationship between gap size, screw size, and pullout force of patellar bone plugs inserted in the femur of the porcine model. Forty-nine porcine knees were obtained. Holes 11 mm in diameter were drilled in the intercondylar notch of the femur. Bone plugs fashioned from the patella were made to fit the holes with gap sizes of 1, 2, 3, or 4 mm. The bone plugs were inserted into the hole

Modified repair of a defect in spondylolysis or minimal spondylolisthesis by pedicle screw, segmental wire fixation, and bone grafting.

A surgical technique for treatment of spondylolysis or minimal isthmic spondylolisthesis is described. The authors based their treatment on the Scott technique, which involves placing an 18-gauge stainless steel wire around the transverse process bilaterally and then tightening the wires to each other inferiorly to the posterior spinal process. The modified technique consists of a tension band wire around the posterior spinous process and a 4.5 mm AO cortical screw in the pedicle. A case study of a 20-year old female athletic student is presented to illustrate the effectiveness of the modified

[Severe atrophy of the alveolar ridge in edentulous patients. Combined treatment using bone grafts and screw implants].

In cases of advanced alveolar ridge atrophy, when the bone available for endosseous implants is not sufficient, endosseous implants are combined with augmentation of the alveolar ridge by means of autologous bone transplants. Depending on the type of atrophy found, the authors use three different surgical procedures: onlay technique, sandwich technique and alveolar extension technique. In all cases functional loading of the implants was performed by way of an entirely implant-supported bridgework. Results to date indicate that initial postoperative resorption is followed by a virtual standstil

Fractures and non unions of the proximal pole of the carpal scaphoid bone. Internal fixation by a proximal to distal screw.

We report a new proximal to distal screw fixation technique for proximal pole fractures and non unions of the carpal scaphoid (type I of the Schernberg, Elzein and Gerard classification). Seven cases of non union with a live proximal polar fragment were operated from 1984 to 1986 with 4 excellent and 2 fair results and one failure. Three recent displaced fractures were also treated this way, with 3 excellent results. This useful technique is worth knowing and deserves a place in the treatment of scaphoid fractures, especially type I proximal pole fractures which represent a challenging problem

Pilon Fractures of the Base of the Proximal Phalanx: Direct Intraosseous Joint Reduction with Bone-Grafting and Mini-Screws: A Case Report.

CASE: A 31-year-old woman sustained a closed pilon fracture of the base of the second proximal phalanx. Computed tomography showed comminution, central depression, and splaying of the articular surface. We used a method of direct intraosseous joint reduction in combination with bone-grafting and mini-screws to treat the fracture. For reduction, a golf-club-shaped instrument was inserted into the medullary cavity through a drill-hole in the phalanx. CONCLUSION: The advantage of this technique is that it leaves the joint capsular complex and the extensor hood intact. A stable joint was achieved

Biomechanical evaluation of lumbar pedicle screws in spondylolytic vertebrae: comparison of fixation strength between the traditional trajectory and a cortical bone trajectory.

OBJECTIVE In the management of isthmic spondylolisthesis, the pedicle screw system is widely accepted surgical strategy; however, there are few reports on the biomechanical behavior of pedicle screws in spondylolytic vertebrae. The purpose of the present study was to compare fixation strength between pedicle screws inserted through the traditional trajectory (TT) and those inserted through a cortical bone trajectory (CBT) in spondylolytic vertebrae by computational simulation. METHODS Finite element models of spondylolytic and normal vertebrae were created from CT scans of 17 patients with adu

Early cephalad adjacent segment degeneration after posterior lumbar interbody fusion: a comparative study between cortical bone trajectory screw fixation and traditional trajectory screw fixation.

OBJECTIVE: Cortical bone trajectory (CBT) screw insertion through a caudomedial starting point provides advantages in limiting dissection of the superior facet joints and reducing muscle dissection and the risk of superior-segment facet violation by the screw. These advantages of the cephalad CBT screw can result in lower rates of early cephalad adjacent segment degeneration (ASD) after posterior lumbar interbody fusion (PLIF) with CBT screw fixation (CBT-PLIF) than those after PLIF using traditional trajectory screw fixation (TT-PLIF). Here, the authors investigated early cephalad ASD after C

Treatment of neglected femoral neck fractures using the modified dynamic hip screw with autogenous bone and bone morphogenetic protein-2 composite materials grafting.

BACKGROUND: The neglected femoral neck fracture is one where there has been a delay of more than 30 days to seek medical help from the time of the original injury. Salvage procedures, such as osteotomy and other treatment options such as vascularized and nonvascularized bone grafts have high failure rates and arthroplasty procedures are not ideal, given the patient's young age and higher levels of activity. We designed a hollow bone graft dynamic hip screw (Hb-DHS) (modified DHS, Hb-DHS) for use in neglected femoral neck fractures. This study evaluates the efficacy and safety of the modified d
