[Clinical reasoning and decision making in practice. A 9-year-old boy with isolated splenomegaly].

Splenomegaly was discovered by chance in a 9-year-old boy who had no further complaints. Apart from splenomegaly and mild thrombocytopenia, no other pathological condition was found in the first instance. Ultrasound revealed a spleen with a median length of 16.7 cm. Blood tests remained stable and a bone marrow biopsy showed no pathology either. Doppler ultrasound of the splenic vessels was normal; screening for a coagulation disorder was not performed. A wait-and-see policy was instituted. Later, the patient developed haematemesis because ofoesophageal varices. It turned out that he had porta

[Clinical reasoning and decision making in practice. A depressive foreign woman with symptoms of malaise].

A 27-year-old woman was admitted to the hospital with a depression, anaemia and fatigue. She had come from Angola to the Netherlands as a refugee 2 years before this evaluation. As an explanation for her symptoms tropical infectious diseases of parasitic origin were considered, but no clues were found in this direction. The test for trypanosomiasis was considered to be suggestive for an infection in the past (persistent titre 1:200). She was discharged but readmitted 6 months later because of a deterioration of her clinical condition. Magnetic resonance imaging showed bilateral signal abnormal

Semantic distance metrics in anatomy education: Toward AI-assisted mapping of anatomy-to-clinical reasoning.

Anatomy education traditionally emphasizes structural knowledge acquisition, with the implicit assumption that clinical reasoning capabilities will naturally follow. However, the cognitive pathway from anatomical understanding to clinical application is rarely linear or uniform. This article proposes a conceptual framework in which semantic distance-the conceptual separation between anatomical knowledge and clinical endpoints-operates at three analytically distinct levels: (1) the learner's internal cognitive network, shaped by prior experience and schema formation; (2) the structural distance…

Bridging Data, Semantics, and Clinical Reasoning: A Knowledge Graph Framework for Pediatric Obstructive Sleep Apnea.

BACKGROUND/OBJECTIVES: Pediatric obstructive sleep apnea (OSA) is a complex disorder with a variable presentation and often challenging diagnostic testing. The history and physical examination in pediatric OSA frequently differ from those in adults. The treatment options are multifaceted and must be tailored to the individual patient. Artificial intelligence (AI) modalities currently employed in pediatric sleep medicine face several important limitations: modality fragmentation, lack of explainability, and limited semantic integration. METHOD: Our team proposes a new vision for AI and pediatr

Development and initial validation of a practical instrument supporting clinical reasoning and self-directed learning.

INTRODUCTION: Clinical reasoning is a core competency in health professions education, which is supported by self-regulated learning (SRL) and adaptive expertise. The Master Adaptive Learner (MAL) framework describes four internal characteristics (curiosity, motivation, growth mindset, and resilience) that support clinical reasoning and SRL development. However, existing instruments typically measure these characteristics in isolation and lack validation within clinical reasoning contexts, limiting their utility for educators. The aims of this study were to generate a list of items that would …

Clinical Reasoning: A 73-Year-Old Man With Progressive Proximal Muscle Weakness and Binocular Diplopia.

Progressive weakness and diplopia have a wide differential diagnosis, which can be narrowed by obtaining a detailed history and performing a careful physical examination. It is essential to determine whether these 2 symptoms are due to a single, unifying process or separate processes. We review the case of a 73-year-old man with progressive proximal lower than upper extremity weakness, followed by binocular horizontal diplopia. Clinical history, with attention to rate of progression and associated symptomatology, guided our diagnostic approach and final diagnosis.

A 32-year-old gravida 1, para 0 woman at 36 weeks and 6 days of gestation presented to the hospital with a 2-day history of new acute-onset headache. She had no personal or family history of headache, and her medical history was remarkable for colitis treated with vedolizumab. Her pregnancy had been unremarkable, without hypertension or gestational diabetes. MRI with venography and time-of-flight angiography was unremarkable. CSF was negative for signs of inflammation and xanthochromia. She had minimal, transient improvement with nerve block. On the third day of admission, she experienced mild

Clinical Reasoning: A 56-Year-Old Woman With Right-Sided Weakness and Recurrent MRI Lesions.

Evaluating and managing patients with recurrent stroke presents a significant challenge, particularly in individuals with underlying autoimmune or rheumatologic conditions. We present a case of a 56-year-old woman with a long-standing history of medically controlled systemic lupus erythematosus (SLE) who has experienced multiple ischemic strokes and a subarachnoid hemorrhage. Despite an unremarkable standard stroke workup and adherence to secondary prevention medications, including antithrombotic and lipid-lowering therapies, the patient continued to experience recurrent strokes, warranting fu

Lessons in clinical reasoning - pitfalls, myths, and pearls: how search satisfying can keep eyes crossed.

OBJECTIVES: Missed opportunities for diagnostic excellence are common and can lead to delayed diagnoses and initiation of treatment. Medical teams incorporate many elements into the diagnostic process, including patient factors, medical knowledge, data gathering, care environment, subspecialized personnel, and hospital processes. A case-based discussion describes how search satisfying - the tendency to stop searching once something has been found - and authority gradients can lead to delays in diagnosis. CASE PRESENTATION: A 2-year-old girl repeatedly presents to the emergency department with

Clinical Reasoning: A 29-Year-Old Woman Presenting With Seizures and an Atypical Frontal Lesion.

Intracranial pseudotumoral lesions pose a considerable diagnostic challenge, arising from various etiologies, with multiple sclerosis being 1 such etiology. While characteristic radiologic features often enable differentiation from neoplastic processes, in certain instances, pseudotumoral lesions can present similarly to highly malignant tumors, complicating the diagnostic process.

Clinical Reasoning: An Unusual Cause of Diplopia and Ptosis in a 67-Year-Old Woman.

Diplopia is a relatively common chief complaint encountered in an outpatient neurology clinic and carries a broad differential diagnosis. In this case, a 67-year-old woman presented with new horizontal, binocular diplopia and ptosis of 8-month duration, which persisted without significant progression. This case highlights the need for a comprehensive list of differential diagnoses for patients with acquired ophthalmoplegia and ptosis. Key learning points include an illustration of the stepwise diagnostic approach to evaluate for common etiologies, the importance of interpreting test results in

Clinical Reasoning: A 39-Year-Old Man With Asymmetric Distal Weakness and Loss of Sensitivity.

A 39-year-old man presented with an asymmetric distal weakness and loss of sensitivity sequentially affecting both lower extremities and the left upper limb. Nerve conduction studies showed a multifocal sensory and motor axonal neuropathy, and a pseudo-conduction block of the right fibular nerve, the whole being consistent with a mononeuropathy multiplex. An uncommon etiology was found after an extensive workup. Axonal loss was severe, with only partial response to treatment with corticosteroids and IV immunoglobulins.

Clinical Reasoning: A 37-Year-Old Man Presenting With Intermittent Dizziness and Brief Choreiform Movements.

In this report, we describe the case of a 37-year-old man presenting with intermittent episodes of dizziness and choreiform movements. MRI of the head showed an acute infarction in the left cerebellar hemisphere and multiple chronic infarctions in the cerebellum and pons. Vessel imaging also showed dynamic stenoses in the posterior circulation. This case provides a framework for evaluating young patients with intermittent dizziness and recurrent strokes and highlights the need to combine anatomical angiographies or hemodynamic studies with head rotational behavior.

Clinical Reasoning: A 12-Year-Old Girl With Acute-Onset Diplopia, Dizziness, and Upbeat Nystagmus.

We report a case of a 12-year-old girl who presented with acute-onset diplopia, dizziness, and upbeat nystagmus. On examination, she had right internuclear ophthalmoplegia with right eye hypertropia and exotropia and impaired convergence. In addition, she also had spontaneous primary position symmetric upbeat nystagmus. In this report, we discuss the clinical approach with meticulous neuro-ophthalmologic examination and neuroanatomic localization in pediatric patients with acute-onset diplopia.
