OBJECTIVES: This article seeks to demonstrate the use of the extended middle cranial fossa approach in the treatment of tumors arising in the anterior cerebellopontine angle and petroclival region.
STUDY DESIGN: We conducted a retrospective chart review.
SETTING: Tertiary referral center.
PATIENTS: : Ten-year retrospective chart review of over 800 skull base surgical cases demonstrated 16 cases in which the senior author used the extended middle cranial fossa as the sole approach to access the posterior cranial fossa, petroclival junction, or the anterior cerebellopontine angle. There were

Arachnoid cysts of the posterior fossa are rather uncommon compared with their supratentorial counterparts. Spontaneous disappearance of middle cranial fossa arachnoid cysts has been reported but there are none in the English literature on the spontaneous fluctuation in size of a posterior fossa arachnoid cyst. We present a 41-year-old male, with a midline posterior fossa arachnoid cyst, which appeared to lessen in size spontaneously with complete disappearance of symptoms and then enlarged with a worsening in neurological status.

Petroclival meningiomas are rare skull base lesions, which originate at the upper two thirds of the clivus, medially to cranial nerves V-XI. Interposition of the cranial nerves between the tumor and surgeon and the proximity/involvement of the basilar artery and brainstem make surgical treatment challenging. Nevertheless, documented growth, brainstem compression, and neurologic symptoms argue in favor of resection. Depending on the size of the lesion, its medial origin along the clivus, extension into the middle fossa, and preoperative hearing, different approaches have been described. A 44-ye

BACKGROUND: In the last decade, endoscopic skull base surgery has significantly developed and generated a plethora of techniques and approaches for access to the cranial ventral floor. However, the exploration for the least-aggressive, maximally efficient approach continues.
OBJECTIVE: To describe in detail an anatomical study, along with the technical nuances of a novel endoscopic approach to Meckel's Cave (MC) using a lateral transorbital (LTO) route.
METHODS: Eighteen orbits of injected cadaveric specimens were operated on, using an endoscopic LTO approach to MC, middle cranial fossa, and

This article reviews the microsurgical anatomy important to preserving the involved cranial nerves and adjacent neural and vascular structures during acoustic neuroma removal. These anatomic considerations are divided into sections dealing with the relationships at the lateral end of the tumor in the meatus and those on the medial end of the tumor at the brain stem. The anatomy of the region offers the opportunity for three approaches to the tumor in the meatus and cerebellopontine angle. One is directed through the middle cranial fossa and the roof of the meatus. Another is directed throught

BACKGROUND: The soft tissue dissection for the middle fossa approach requires adequate management of the neuro, vascular, and muscular structures in order to maximize exposure and diminish morbidities.
METHODS: An incision anterior to the tragus is performed, extending from the zygomatic process to the superior temporal line. The superior temporal artery is exposed, followed by a subfascial dissection of the frontalis nerve. The temporal muscle is dissected and released from the zygoma. All cranial landmarks are exposed for the 5 × 5 cm temporal fossa craniotomy.
CONCLUSION: This novel appro

A total of 311 patients with a unilateral acoustic neurinoma were operated on via the enlarged middle cranial fossa approach. A total tumor removal was achieved in 98% of cases. The mortality was 0.6%. Overall in acoustic neurinoma surgery, the percentage rates of meningitis (1.6%), cerebrospinal fluid fistula (1.6% requiring surgery), and neurological deficits were fairly low. The facial nerve could be preserved anatomically in 99%. A House I or House II classification was demonstrated in 91% of all tumors and in 98% of small- and medium-sized tumors. Preservation of the hearing function was

A 66-year-old woman complained of right-sided headache and was found to have progressive dysfunction of cranial nerves V and VIII through XII on the right side. At autopsy, there was a granulomatous pachymengitis involving the floor of the right middle and posterior cranial fossae due to Candida tropicalis infection. Inflammatory tissue compressed the clinically affected cranial nerves.

BACKGROUND: True lambdoid synostosis (TLS) produces a consistent morphology that includes occipital flattening, an ipsilateral occipitomastoid prominence, and a mild contralateral hemifacial deficiency that minimally improves with surgery. Prior studies have demonstrated that dysmorphic middle and posterior cranial fossae contribute to the craniofacial scoliosis characteristic of TLS. We hypothesize that these endocranial features remain after surgery, causing the persistent hemifacial deficiency seen in these patients.
METHODS: Three-dimensional anthropometric measurements were made on preop

BACKGROUND: Arachnoid cysts are a relatively common incidental finding on CT scans of the brain. They most commonly occur in the middle cranial fossa, where familial occurrence has rarely been reported. Posterior fossa arachnoid cysts are more unusual.
CASE HISTORIES: We report the presence of quadrigeminal cistern arachnoid cysts in siblings.

Cognitive function has not been well studied after neurosurgery for posterior fossa lesions despite its potential importance in determining surgical indications and approaches. The present study evaluated changes in cognitive functions after posterior fossa surgery to detect any differences between the middle cranial fossa and lateral suboccipital approaches in 50 patients with posterior fossa lesions such as tumors and vascular diseases. Twenty-five patients underwent surgery via the middle fossa and 25 via the lateral suboccipital approaches. Computerized test battery (CogState) and conventi

The pituitary fossa, sphenoid rostrum, sphenoid sinus, nasopharynx, pterygopalatine fossa and clivus are the components of the central compartment of the middle cranial base. It is a surgical challenge to gain access to this region. This fact has led to the development of a number of surgical procedures reported in the literature. However, none of these techniques can provide a wide and direct exposure to the middle cranial fossa without morbidity on its own. In this report, nasomaxillary osteotomy is described as a satisfactory alternative to reach the middle cranial fossa. With the nasomaxil

OBJECTIVE: The surgical anatomy of the infratemporal fossa and parapharyngeal space is often not properly understood by neurosurgeons, because these areas are more related to other medical disciplines. This article provides a detailed description of the infratemporal fossa and parapharyngeal space anatomy in cadaveric specimens and offers a neurosurgical perspective on a surgical approach that allows wide exposure and complete resection of giant tumors in this location.
METHODS: Ten cadaveric specimens were prepared for anatomic study. Dissections were performed to emphasize the relationship

The purpose of this study was to relate the facial asymmetries with possible impairments of the skull base, particularly with regard to the three-dimensional position of the glenoid fossa. Fifty-eight subjects met the inclusion criteria (25 men and 33 women) and were included in the analysis. All patients underwent three-dimensional computed tomographies. Patients were classified into 3 asymmetry groups: vertical plane asymmetries, transversal plane asymmetries (TAs), and hybrid forms. A control group, composed of 29 subjects having obstructive sleep apnea syndrome with no asymmetries, was use

The aim of this study was to perform a morphometric analysis of the skull base to understand the mechanism of asymmetry between malformational and deformational plagiocephaly. Since 1988, we have carried out 102 CT scans with three-dimensional reconstruction of the endocranial side of the skull base. There were 20 cases of malformational plagiocephaly, consisting of 18 unicoronal synostoses (UCS) and two unilambdoid synostoses (ULS), and 82 cases of plagiocephaly without synostosis, made up of 60 cases of frontal plagiocephaly (FP) and 22 of occipital plagiocephaly (OP). The skull base asymmet

PURPOSE: To describe two cases of arachnoid cyst that altered the bony orbit.
METHODS: Case report.
RESULTS: Cases of unilateral arachnoid cyst are described in two men, both over the age of 60. The patients were evaluated with computed tomography (CT) scans and in each, an arachnoid cyst in the middle cranial fossa caused bone remodeling with exophthalmos in one. These patients were followed clinically and radiographically over a number of years with no change.
CONCLUSION: Arachnoid cysts in the middle cranial fossa have the potential to result in clinically significant proptosis due to bo

OBJECTIVE: There has been a considerable growth in the indications of endonasal surgery that now include malignant tumours of the nasal fossae and anterior and middle cranial fossa. However, new limitations have also been identified, such as bleeding and cerebrospinal fluid leak, as well as the need to use several instruments simultaneously. Can robotics provide solutions to these problems?
METHOD: Review of the literature based on the three main databases: Medline, Pubmed and Cochrane.
RESULTS: Ten publications were identified. Some authors have developed surgical approaches to the skull ba

INTRODUCTION: Volumetric studies in relation to CM/SM have not been reported in Pomeranians. In this study, we aim to (1) report the intermodality agreement between CT- and MRI-based volumetric measurements of the skull and cervical spinal canal, and (2) assess for differences and associations between the volumetric measurements and CM/SM status.
METHODS: Pomeranians were included that underwent CT and MRI studies during the period of February 2022-June 2024. Frontal sinus volume (FSV), caudal cranial fossa volume (CCFV), rostral and middle cranial fossa volume (RMCFV), caudal cranial fossa p
