BACKGROUND: Our purpose was to summarise the prognostic associations between various clinical risk factors and development of acute respiratory distress syndrome (ARDS) following traumatic injury.
METHODS: We conducted this review in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) and CHARMS (Critical Appraisal and Data Extraction for Systematic Reviews of Prediction Modeling Studies) guidelines. We searched six databases from inception through December 2020. We included English language studies describing the clinical risk factors associated wi

Sepsis and septic shock represent important infection-related medical emergencies that result in significant morbidity and mortality. The prevalence and microbiology of these processes are evolving. Nonetheless, timely and appropriate antibiotic therapy continues to represent the most important determinant of survival. Recent trials have clarified that crystalloids are preferred for initial resuscitation, and balanced crystalloids appear superior to 0.9% saline. Controversy remains regarding not only the rate and rapidity of fluid resuscitation but also about the timing and use of vasopressors

PURPOSE OF REVIEW: This review summarizes current literature pertaining to fluid management for critically ill children. It includes an overview on crystalloid fluid used throughout the critical illness course, management of fluid output and complications with fluid overload.
RECENT FINDINGS: Observational paediatric studies and adult randomized trials show mixed results regarding risk of mortality and kidney injury with 0.9% saline and crystalloid fluid. A recent adult randomized trial suggests that a fluid restrictive strategy may be well tolerated in critically ill adults with septic shock

BACKGROUND AND OBJECTIVES: The Beach Chair Position (BCP) has many advantages such as less neurovascular injury and better intra-articular visualization, but it has also negative consequences, including hemodynamic instability. Although maintaining normal Mean Arterial Pressure (MAP) is important, fluid management is also a crucial concept for hemodynamic stability. The main objective of this study is whether preloading before positioning would be effective for less hemodynamic instability.
METHODS: This randomized, controlled study was conducted in a single center in the Istanbul University,

BACKGROUND: The best fluid replacement strategy and the role of albumin in immunocompromised patients with sepsis is unclear.
METHODS: We performed a secondary analysis of immunocompromised patients enrolled in the ALBIOS trial which randomized patients with severe sepsis or septic shock to receive either 20% albumin (target 30 g per liter or more) and crystalloid or crystalloid alone during ICU stay.
RESULTS: Of 1818 patients originally enrolled, 304 (16.4%) were immunocompromised. One-hundred-thirty-nine (45.7%) patients were randomized in the albumin while 165 (54.2%) in the crystalloid g

PURPOSE OF REVIEW: The decision of the European Medicines Agency (EMA) against the use of hydroxyethyl starch (HES)-based volume replacement solutions in critically ill patients has led to a general uncertainty when dealing with HES-based solutions, even though HES-containing solutions can still be used for the treatment of hypovolaemia caused by acute (sudden) blood loss. This review discusses current evidence of the intraoperative use of HES-based solutions.
RECENT FINDINGS: HES solutions are often criticized for possible side-effects on the kidney, the coagulation system or tissue storage.

BACKGROUND: The Krebs-Henseleit buffer is the best perfusion solution for isolated mammalian hearts. We hypothesized that a Krebs-Henseleit buffer-based cardioplegic solution might provide better myocardial protection than well-known crystalloid cardioplegic solutions because of its optimal electrolyte and glucose levels, presence of buffer systems, and mild hyperosmolarity.
METHODS: Isolated Langendorff-perfused rat hearts were subjected to either global ischemia without cardioplegia (controls) or cardioplegic arrest for either 60 or 180 min, followed by 120 min of reperfusion. The modified

Shock (hypovolemic and infectious-toxic) is a severe complication of acute intestinal infections. Of 35096 patients with acute intestinal infections (90.6% had food poisoning, 9.4% had salmonellosis), 24 (0.07%) patients developed hypovolemic, 26 (0.08%) infectious-toxic shock. Hypovolemic shock is based on decompensated dehydration, infectious-toxic one arises from generalized circulation disorders with severe affection of hemostasis. Hypovolemic shock was effectively managed with polyionic crystalloid solutions. In infectious-toxic shock hemodynamic and homeostatic defects should be correcte…

Infusion of 1000 ml of 6% hydroxyethylstarch (HES) of the third generation 130/0.42 or crystalloid solutions was conducted to patients before performance of operation on abdominal cavity organs using epidural anesthesia. While HES application the average arterial pressure after induction into anesthesia was less pronounced and concentration of C-reactive protein on the third postoperative day was lesser. It is concluded, that HES 130/0.42 secures the hemodynamic indices stability and reduction of systemic inflammatory reaction severity in the patients and do not influence hemostasis and renal …

The optimal fluid for the resuscitation of critically ill and injured patients remains the subject of considerable controversy. Hypertonic crystalloid solutions such as hypertonic saline provide rapid volume expansion, have an acceptable safety profile, and are easy to store and transport. Recent meta-analyses suggest a trend toward increased survival in patients given hypertonic saline, and it has been suggested that they may have particular benefit in certain groups of patients such as hypovolaemic head injury patients. This short review examines the physiological and experimental evidence s…

Hydroxyethyl starch (HES 450.000/0.7; Hespan 6.0 g/100 ml) was compared with standard crystalloid solutions in postoperative volume replacement in 20 patients undergoing routine orthopaedic surgery. The HES group showed no clinical evidence of haemorrhage and no laboratory evidence of significant haemostatic defects as assessed by standard coagulation tests, platelet aggregation and fibrinogen concentrations. There was a slight shortening in the thrombin time and a smaller increase in post-operative FVIII RAg and FVIII RCof levels in the HES group. HES is a safe and effective volume expander f…

The present study was undertaken to determine the safety of type-specific uncrossmatched blood transfusions for severely hypovolemic trauma patients. During a 3-year period 875 units of type-specific uncrossmatched blood were given to 160 severely hypovolemic trauma patients who could not be adequately resuscitated with crystalloid solutions. No transfusion reactions resulted from the administration of type-specific uncrossmatched blood. Subsequent major crossmatch failed to identify either blood incompatibility or significant antibodies. Type-specific uncrossmatched blood is safe and is a rap…

BACKGROUND: DHF is characterized by plasma leakage and abnormal hemostasis. About 20% of DHF patients do require colloidal solution in addition to conventional crystalloid solution for the treatment. There is only one colloidal solution, 10% Dextran-40 in NSS that proved to be effective for this group of DHF patients.
OBJECTIVE: To compare 10% dextran-40 in NSS with 10% Haes-steril in NSS in the management of DHF cases with severe plasma leakage for their effectiveness and impact on renal function, hemostasis, disease severity, and complications.
MATERIAL AND METHOD: DHF patients admitted to

BACKGROUND: In the present randomized study, we evaluated the differential effects of a colloid and a crystalloid fluid on renal oxygen delivery (RD(O2)), glomerular filtration (GFR), renal oxygen consumption ((RV(O2))), and the renal oxygen supply-demand relationship (i.e., renal oxygenation) after cardiac surgery with cardiopulmonary bypass.
METHODS: Thirty patients with normal preoperative renal function, undergoing uncomplicated cardiac surgery, were studied in the intensive care unit in the early postoperative period. Patients were randomized to receive a bolus dose of either a crystallo

OBJECTIVES: Current cardiac surgery patients are older, sicker, with more diffuse disease and hence a reduced tolerance to ischaemia-reperfusion injury. We previously demonstrated that esmolol, an ultra-short-acting β-blocker, can be used as an arresting agent at high (millimolar) concentrations, and that a crystalloid-based esmolol cardioplegia afforded cardioprotection at least equivalent to hyperkalaemic (St Thomas' Hospital) cardioplegia. Esmolol is rapidly metabolized by blood esterases, so it was important to determine the feasibility of its use in blood-based solutions. This study compa

Intravenous fluid resuscitation is ubiquitous throughout medicine and is often considered a benign procedure. Yet, there is now clear recogn … rged to suggest that outcomes may be different between resuscitation with chloride-rich vs balanced crystalloid solutions. In this article, we review the current literature regarding choice of intravenous fluids for resuscitation in the intensive care setting and describe the dangers associated with fluid overload in critically ill patients.
