[Work-related stress urticaria].

A medical doctor occasionally experienced itching papular exanthema during work hours. Careful history taking and a diary of symptoms demonstrated an association between the urticaria and stressful work situations. Urticaria can have many triggers, and stress is among the well-known triggers. The usual treatment of urticaria consists of avoiding triggers and, if necessary, antihistamines. Whereas it might be difficult to follow these recommendations in the case of work-related urticaria, the disorder may be perceived as a reminder of work situations which are going awry.

Triage of a febrile patient with a rash: a comparison of chickenpox, monkeypox, and smallpox.

The immediate and correct recognition of an infectious exanthema can be aided with a focused history and minor assessment. False alarms can consume health care resources and create unnecessary anxiety. Clinicians can use specific references to not only help with educating staff, but to ensure a more accurate diagnosis and trigger notification of appropriate infectious disease protocols. The authors recommend that al emergency departments have a process in place to immediately isolate suspicious cases until a more thorough medial workup can be performed.

[Marseille fever imported from Spain].

The authors are presenting a case of young female with Marseille fever contracted in Spain. The clinical manifestation of the illness was characterized by fevers, exanthema, headache and a typical skin rash ("the black spot") and prompted the authors to strongly consider the diagnosis of Marseille fever and to initiate appropriate antibiotic therapy. The diagnosis was confirmed later by serology. The article introduces new taxonomy of Rickettsial species and presents an overview and epidemiological aspects of specific diseases caused by them. The clinical manifestation, diagnosis and treatment…

[Isolation and identification of the smallpox virus which caused iatrogenic vaccinia in children in the city of Vladivostok].

The comparative study of the specimens of the morphological elements of exanthema obtained from 8 children with the clinical diagnosis of secondary exogenic vaccinia, dried smallpox vaccine and the cultures of other orthopoxviruses (rabbit pox, monkey pox and buffalo pox viruses) was made. The isolation and identification of the causative agents from the specimens was carried out with the use of modern virological, electron microscopic and molecular methods. The study proved the fact that 8 children had orthopoxvirus infection with its causative agent identified as vaccinia virus.

[Measles. A viral illness with risk of permanent damage].

Measles is a highly contagious exanthematous disease. After an incubation period of almost two weeks, catarrhal prodromic, associated with initial attacks of fever appear. Typical manifestations are Koplik's spots. The exanthema appears together with the second rise in temperature. The illness confers lifelong immunity. In individual cases, an encephalitis resulting in permanent neurological deficits must be expected. Every effort should be made to prevent this condition from arising. To this end, immunization with a combination measles, mumps, rubella (MMR) vaccine is recommended.

[Acute encephalopathy following acquired rubella. Apropos of 6 cases].

The authors report 6 cases of acute encephalopathy during the course of primary rubella infection. The mean age of the patients was 7 years and 6 months. Clinical features included, 3 days after an inconstant exanthema, seizures, coma and hyperthermia. The diagnosis of rubella was confirmed by a high rate of antibodies in serum with presence of IgM fraction. Five children recovered fully, one patient remained epileptic. The authors discuss the problems of diagnosis, the nosological position and the treatment of non-congenital rubella encephalitis.

[Lichenoid drug reactions].

Lichenoid drug reactions are rare compared to typical morbilliform drug exanthema or urticaria. They are associated with specific drugs or drug families like gold, antimalarial drugs, β‑blockers and angiotensin-converting-enzyme inhibitors. Recent observations included associations with novel drugs such as biologics (e. g. tumour necrosis factor antagonists) and immune checkpoint inhibitors (anti-programme cell death protein 1 antibodies). Lichenoid drug reactions most often resemble lichen planus mainly in areas of ultraviolet-light exposed skin, but also mucosal lichen planus and even bullou…

[Drug-induced skin reactions].

Antimicrobials and anti-inflammatories are the most common drugs causing skin reactions, but reactions are also brought about by ACE inhibitors, antiepileptics, many anticancer and certain other drugs. Exanthema and urticaria are the most common types of drug reactions. Urticaria may or may not be accompanied by angioedema or anaphylaxia. Possible life-threatening drug reactions include also toxic epidermal necrolysis, Stevens-Johnson syndrome and eosinophilia with systemic symptoms. A drug reaction resembles a typical allergic reaction. Diagnosis is based on the clinical picture and anamnesis…

Acute obstructive respiratory diseases (ARD) and bacterial complications of ARD (pneumonia, sinusitis) in infants and children associated with human herpesvirus-6 infection.

The existence of catharreal respiratory symptoms (such as pharyngitis, tracheitis, bronchitis etc.) is established as the usual manifestation of "exanthema subitum" due to acute human herpesvirus-6 (HHV-6) infection. But so far pneumonia, purulent sinusitis, purulent otitis media and/or acute obstructive bronchitis (bronchiolitis) in infants and children have not been described. Here we report the results of observations of 2 children with bronchopneumonia/sinusitis maxillaris and severe bronchiolitis associated with an acute HHV-6 infection. Other respiratory viruses were excluded as agents c…

[Leukocytopenia as an incidental finding. Finding the etiology].

Any leukopenia of less than 1000/microliter poses an acute threat to life, and mandates an immediate search for the underlying cause. An extensive history-taking (use of drugs? visits abroad? previous illnesses?) and physical examination (splenomegaly? exanthema? signs of hemorrhage?) are mandatory. In addition to a manual differential blood count, bone marrow aspiration for cytological and histological evaluation must be requested. In this overview, the major differential diagnoses, such as allergic agranulocytosis, leukemia, pernicious illnesses (e.g. malaria), hypersplenic syndrome and a nu…

Toxic Epidermal Necrolysis During Dronedarone Treatment: First Report of a Severe Serious Adverse Event Of A New Antiarrhythmic Drug.

A 77-year-old female patient with symptomatic atrial fibrillation with fast ventricular rate despite conventional antiarrhythmic therapy was treated with dronedarone. Five days later, she developed a maculopapulous exanthema and small flaccid blisters, which spread over the common integument predominantly located on the dorsal trunk. Over few days, the patient showed a severe epidermal necrolysis of approximately 30 % of the body area and ultimately died in multiorgan failure. Here, we report a rare case of toxic epidermal necrolysis during treatment with dronedarone leading to patient death.

[Cercaria dermatitis (swimmer's itch). Case report of cercaria dermatitis caused by Trichobilharzia (Digena, Schistosomatidae)].

A 63-year-old woman developed a pruritic exanthema on the extremities after one hour of work in her garden pond in Planegg, Southern Bavaria. As the appropriate vectors (ducks and snails) were present, we made the tentative diagnosis of cercarial dermatitis (swimmer's itch). By different serological methods (cercarial fluorescent antibody test, cercarial Hüllen reaction, circumoval precipitin test) antibodies against cercariae could be demonstrated in the serum of the patient 14 days later. Cercarial dermatitis appears worldwide, but in Central Europe the disease is often not recognized.

[Gianotti-Crosti syndrome in Epstein-Barr virus infection].

Gianotti-Crosti syndrome, a relatively rare, distinctive eruption occurring after hepatitis B infection, is characterized by a lichenoid papular exanthema, usually localized on the face, limbs, and buttocks. Hepatitis B antigenaemia is associated with Gianotti-Crosti syndrome only in some cases. Recent reports indicate that a variety of infectious agents are associated with Gianotti-Crosti syndrome. This is a report of 2 1/2-year-old girl with Gianotti-Crosti syndrome and concurrent primary Epstein-Barr virus infection without evidence of hepatitis B infection.

The first outbreak of camelpox in Syria.

In this study we report the first outbreak of camelpox in two provinces in Syria. Clinical symptoms started with fever, salivation and general exanthema. The main features were facial and legs oedema, pustules on the mucosa of the lips and a high rate of abortion. Lesions may also occur on the whole body including scrotum and udder. Specimens were investigated by electron microscopy, virus isolation in cell culture and embryonated eggs and by immunohistochemistry. The causative agent was identified as camelpox virus by polymerase chain reaction and sequencing of the hemagglutinin gene.
