Mask R-CNN based multiclass segmentation model for endotracheal intubation using video laryngoscope.

OBJECTIVE: Endotracheal intubation (ETI) is critical to secure the airway in emergent situations. Although artificial intelligence algorithms are frequently used to analyze medical images, their application to evaluating intraoral structures based on images captured during emergent ETI remains limited. The aim of this study is to develop an artificial intelligence model for segmenting structures in the oral cavity using video laryngoscope (VL) images. METHODS: From 54 VL videos, clinicians manually labeled images that include motion blur, foggy vision, blood, mucus, and vomitus. Anatomical st

Euclidean distances of laryngopharyngeal structures obtained from CT data for preclinical development of laryngoscopic devices.

PURPOSE: This study aims to determine Euclidean distances between landmark structures in the larynx and pharynx to optimize endoscope shaft … ferences could be observed for distances (1)-(4). The results indicate that the length of transoral laryngoscopes should not be below 110 mm and that a minimum angular field of view of α = 17° is required to fully observe the laryngeal inlet.

Ratio of height-to-thyromental distance and ratio of height-to-sternomental distance as predictors of laryngoscopic grade in children.

BACKGROUND AND AIMS: Failure to secure the airway is an important cause of morbidity and mortality in children. Children are often uncooperative for routine examination and pose problems for obtaining external measurements. We aimed to evaluate ratio of height-to-thyromental distance (RHTMD) and ratio of height-to-sternomental distance (RHSMD) as predictors of laryngoscopic grade in children aged 1-12 years. MATERIAL AND METHODS: This study was an observational study conducted in children aged between 1 and 12 years scheduled for elective surgery under general anesthesia. Children unable to s

Submucosal Abscess of the Esophagus Caused by Piriform Sinus Fistula Treated with Transoral Video Laryngoscopic Surgery.

Piriform sinus fistula (PSF) is a rare branchial anomaly that causes repetitive acute suppurative thyroiditis or deep neck abscess. The definitive treatment of PSF is open neck surgery. However, such surgery has a cosmetic problem and a high risk of recurrence. Furthermore, identifying the fistula is difficult due to previous repetitive infections. We report a case of esophageal submucosal abscess caused by PSF treated with endoscopic mucosal incision. The patient underwent transoral video laryngoscopic surgery (TOVS), and endoscopy as well as fluoroscopy revealed complete closure of PSF witho

[Use of a New Video-laryngoscope (McGRATH® X-blade™) in Patients with Difficult Airways].

We describe the use of a new video-laryngoscope (McGRATH® X-blade™, X-blade) in patients with difficult airways. We report four cases of difficult and failed tracheal intubation using a conventional Machintosh laryngoscope and McGRATH® MAC in which tracheal intubation was accomplished swiftly and easily using a newly designed videolaryngoscope, the X-blade. The whole glottis was easily visible (Cormack-Lehane grade 1) with the X-blade. X-blade also provided a good view of glottic opening in a patient with obesity. We believe that X-blade is useful in patients with difficult airways.

Detection of chronic laryngitis due to laryngopharyngeal reflux using color and texture analysis of laryngoscopic images.

OBJECTIVE: To determine if pattern recognition of hue and textural parameters can be used to identify laryngopharyngeal reflux (LPR). METHODS: Laryngoscopic images from 20 subjects with LPR and 42 control subjects without LPR were obtained. LPR status was determined using the reflux finding score. Color and texture features were quantified using hue calculation and two-dimensional Gabor filtering. Five regions were analyzed: true vocal folds, false vocal folds, epiglottis, interarytenoid space, and arytenoid mucosae. A multilayer perceptron artificial neural network with varying numbers of hi

Use of the Venner™ A.P. Advance™ video laryngoscope for biopsy examination of the base of the tongue.

Biopsy specimens taken from the base of the tongue can be non-diagnostic, because of errors in sampling and tissue samples that are too small or superficial. The Venner™ A.P. Advance™ video laryngoscope (Intavent Direct, Maidenhead, UK) is an anaesthetic laryngoscope designed for use in patients with a difficult airway. We present a pilot study that compares video laryngoscope-assisted biopsy examination of the base of the tongue with the standard technique using a surgical laryngoscope. The video laryngoscope gives a wide view of the base of the tongue and the ability to use sinus biopsy forc

Anesthesia management in pediatric patients with laryngeal papillomatosis undergoing suspension laryngoscopic surgery and a review of the literature.

OBJECTIVE: The excision of laryngeal papillomas poses a great challenge for both the anesthesiologist and the surgeon. The narrowness of the airways and the great variability of the pathological lesions necessitate close collaboration between the surgical and anesthesia teams to provide optimal operating conditions and ensure adequate ventilation and oxygenation. Our aim was to explore perioperative anesthesia management in pediatric patients during the excision of laryngeal papillomas with a suspension laryngoscope. METHODS: Fifty-eight pediatric patients suffering from laryngeal papillomas

[Anesthetic management for laryngoscopic evaluation of upper airway stenosis using our modified endoscopic mask].

Young children are at increased risk for hypoxaemia and hypercapnea during flexible laryngoscopy due to the small size and increased collapsibility of their airways. To evaluate upper airway stenosis of a 5-year-old girl presenting with retractive inspiratory breathing, we developed a modified endoscopy mask consisting of a regular face mask where the original port for endoscope was replaced by a disposable rubber dam. This setting allows a leak-free passage of both nasal and oral endoscopy and enables emergent endoscopic intubation through a small hole of a rubber dam. Using this airtightly s

The ProSeal laryngeal mask airway is an effective alternative to laryngoscope-guided tracheal intubation for gynaecological laparoscopy.

We tested the hypothesis that the ProSeal laryngeal mask airway is superior to laryngoscope-guided tracheal intubation for gynaecological laparoscopy. One-hundred and eighty consecutive patients (ASA grade 1-2, aged 18-80 y) were divided into two equal-sized groups for airway management with the ProSeal laryngeal mask airway or tracheal tube. Induction was with fentanyl/propofol, maintenance with sevoflurane and muscle relaxation with atracurium. The following primary variables were tested: time to achieve an effective airway, ventilatory capability, peak airway pressure before and after pneum
