Presentation of two cases of Leydig cell tumor in adult patients who presented with gynecomastia, and slightly abnormal hormonal profiles. One patient had a non palpable tumor. The pathophysiology of this entity is commented, as well as the importance of the different modalities in its diagnosis.

A 22-year-old phenotypic woman with an XY karyotype underwent laparoscopic gonadectomy after a comprehensive evaluation for primary amenorrhea. Pathology revealed primitive testicular support elements with absent Leydig cells bilaterally. This case illustrates the fact that, with a thorough preoperative workup, patients expressing various forms of male pseudohermaphroditism can now be treated appropriately by laparoscopy instead of laparotomy.

The nature of a non-mosaic marker Y chromosome observed in a pseudohermaphrodite patient with Leydig cell agenesis was investigated by high-resolution chromosome analysis and molecular probes from the Y chromosome. Cytogenetically, the marker chromosome appeared to be an isodicentric, with breakage in Yq11.21. Double copies of all Yp-specific loci tested, including SRY, were present. A 22-year-old man with hypogonadism, Leydig cell hypoplasia, high plasma LH, and normal plasma FSH was given human chorionic gonadotropin (hCG) and human luteinizing hormone (hLH) in order to assess testicular testosterone responses to exogenously administered gonadotropins. Clinical, morphological and endocrine characteristics of 2 patients with Leydig cell tumours (1 benign unilateral and 1 malignant bilateral) are reported.
