COVID-19 respiratory support outside the ICU's doors. An observational study for a new operative strategy.

BACKGROUND AND AIM: During the first wave of the Severe Acute Respiratory Syndrome CoronaVirus 2 (SARS-CoV-2) pandemic, we faced a massive clinical and organizational challenge having to manage critically ill patients outside the Intensive Care Unit (ICU). This was due to the significant imbalance between ICU bed availability and the number of patients presenting Acute Hypoxemic Respiratory Failure caused by SARS-CoV-2-related interstitial pneumonia. We therefore needed to perform Non-Invasive Ventilation (NIV) in non-intensive wards to assist these patients and relieve pressure on the ICUs an

The Embedded Model for Bridging Essential Delivery of Care in Assisted Living Facilities-EMBED-ALF.

Older adults residing in assisted living facilities (ALFs) often face fragmented medical care, especially when managing complex chronic conditions, cognitive impairment, or end-of-life needs. This article describes an interprofessional provider-led, integrated care model embedded within an ALF in a major metropolitan area that includes memory care and home hospice. The model integrates geriatrics and palliative medicine-trained providers into routine care, supported by structured communication, tele-emergency services, and collaboration with hospice and hospital teams. While formal outcomes ar

Ranking of Business Process Simulation Software Tools with DEX/QQ Hierarchical Decision Model.

The omnipresent need for optimisation requires constant improvements of companies' business processes (BPs). Minimising the risk of inappropriate BP being implemented is usually performed by simulating the newly developed BP under various initial conditions and "what-if" scenarios. An effectual business process simulations software (BPSS) is a prerequisite for accurate analysis of an BP. Characterisation of an BPSS tool is a challenging task due to the complex selection criteria that includes quality of visual aspects, simulation capabilities, statistical facilities, quality reporting etc. Und

Two models for delivery of women's continence care: the step-wise continence team versus the traditional medical model.

BACKGROUND: The current pyramidal system of health care delivery, in which subspecialized physicians with the most complete knowledge and expertise are difficult to access, is inefficient and ineffective at delivering the best care to the majority of patients. Urinary incontinence care exemplifies this problem. Patients can wait up to one year to see a urogynaecologist, regardless of the complexity of their problem. Many women who do not require subspecialized care could have their incontinence managed in a more timely fashion if they had access to education and conservative interventions via

Identifying Work-Related Factors Leading to Awkward Postures and Musculoskeletal Disorders in Crane Operators: Insights From a Grounded Theory Study.

BACKGROUND: Musculoskeletal disorders (MSDs) remain a leading cause of occupational disability worldwide. Overhead crane operators are frequently exposed to awkward postures that increase the risk of MSDs. While physical conditions are often blamed, the broader organizational and ergonomic context remains underexplored. METHODS: This study employed a grounded theory approach, conducting 52 semi-structured interviews with crane operators and safety personnel in a large Iranian steel company. Transcripts were analyzed using open, axial, and selective coding via MAXQDA software. RESULTS: Eight

Anxiety and the severity of Tension-Type Headache mediate the relation between headache presenteeism and workers' productivity.

The primary objective of this study was to explore the mechanisms and conditions whereby Tension-Type Headache (TTH) presenteeism relates to health-related loss of productivity as a result of both reduced physical and mental health. To this end, Structural Equation Modeling (SEM) was used to conduct a secondary data analysis of a randomized clinical trial involving 78 Tension-type Headache (TTH) patients. The results showed that TTH presenteeism did not directly relate to health-related loss of productivity, either due to physical, or mental health problems. However, through anxiety-state, TTH

The evolution of the Fenway Community Health model.

Fenway Community Health was founded by community activists in 1971 in the Fenway neighborhood of Boston, Mass, and within a decade had rapidly expanded its medical services for gay men in response to the AIDS epidemic. Increased expertise and cultural competence in lesbian, gay, bisexual, and transgender (LGBT) care led to expansion of medical services to address broader community concerns, ranging from substance use to parenting issues to domestic and homophobic violence, as well as specialized programs for lesbians, bisexuals, and transgendered individuals. Fenway began as a grassroots neigh

A clearing in the crowd: innovations in emergency services.

Emergency department overcrowding may not be so much a result of inappropriate use of the emergency department as it is a problem of the unavailability and inaccessibility of primary care services in the community. The prevailing theory that people use the emergency department because they cannot afford care elsewhere does not hold true. The vast majority of patients surveyed as part of the Emergency Services Initiative had some type of coverage, primarily Medicare or Medicaid. Without additional primary care capacity to accommodate walk-in patients and to make appointments available within on

Piecewise latent growth modeling as the methodological standard for evaluation research.

Evaluation research across applied domains-clinical intervention, psychotherapy, curriculum reform, organizational change-has long relied on the pre-post difference score as its primary evidence of change. This paper argues that this reliance reflects not merely a methodological habit but a systematic ontological error: the implicit treatment of change as a discrete event rather than a continuous, phase-structured process. We develop a theoretical and methodological case for piecewise latent growth modeling (PLGM) as the appropriate evaluative standard across these domains. Drawing on classica

Physical and emotional health among nurses in protracted crisis settings in Lebanon and Jordan: A cross-sectional study.

OBJECTIVES: Nurses working in crisis-affected and refugee-hosting settings face demanding conditions that may compromise their physical and emotional health. Evidence on the prevalence and determinants of these outcomes in such contexts remains limited. This study assessed nurses' physical and emotional health in Lebanon and Jordan and identified key associated factors using regression and random forest models. METHODS: A cross-sectional survey was conducted among hospital nurses providing care to Syrian refugees in Lebanon (n = 976; response rate 52%) and Jordan (n = 2,012; response rate 80.

Exploration and exploitation in crisis environment: Implications for level and variability of firm performance.

Exploration and exploitation are two generic strategies of firms' adaptation to their environments. However, the effectiveness and reliability of these approaches are not fully understood when the business environment is undergoing a major crisis. Building on organizational adaptation, strategic fit, and organizational decline streams of literature, we develop a framework that examines exploration and exploitation in crisis contexts. We argue that the severity of crisis a firm is exposed to acts as a positive contingency for the impact of exploration on firm performance level and variability,

Occupational burnout during war: The role of stress, disruptions in routine, sleep, work-family conflict, and organizational support as a moderator.

BACKGROUND: Occupational burnout, resulting from long-term exposure to work-related stressors, is a significant risk factor for both physical and mental health of employees. Most research on burnout focuses on routine situations, with less attention given to its causes and manifestations during prolonged national crises such as war. According to the Conservation of Resources theory, wartime conditions are associated with a loss of resources, leading to accelerated burnout. This study aimed to examine burnout among employees during the war in Israel that broke out in October 2023, placing the p

Exploring the potential of a multi-level approach to improve capability for continuous organizational improvement and learning in a Swedish healthcare region.

BACKGROUND: Eldercare and care of people with functional impairments is organized by the municipalities in Sweden. Improving care in these areas is complex, with multiple stakeholders and organizations. Appropriate strategies to develop capability for continuing organizational improvement and learning (COIL) are needed. The purpose of our study was to develop and pilot-test a flexible, multilevel approach for COIL capability building and to identify what it takes to achieve changes in key actors' approaches to COIL. The approach, named "Sustainable Improvement and Development through Strategic

Perspective: Autonomic care systems for hospitalized patients.

With advancements of medical technology and improved diagnostic and treatment options, children with severe birth defects who would otherwise have no chance of surviving post birth survive to go home every day. The average lifespan in the United States has increased substantially over the last century. These successes and many other medical breakthroughs in managing complex illnesses, particularly in frail, elderly patients, have resulted in an increasing percentage of patients with comorbidities. This, coupled with a policy change by Medicare (i.e., Medicare will no longer reimburse hospitals

Organizational characteristics of solid-organ donor hospitals and nondonor hospitals.

CONTEXT: Efforts to increase organ donation include serious attempts in hospital settings, where unrealized donation potential exists. Research on hospital donation must include understanding organizational as well as patient-specific influences on the donation process. OBJECTIVE: To identify organizational characteristics that distinguish hospitals producing organ donations from those that do not, and to estimate the number of nondonor hospitals with donor potential. DESIGN: Data from the American Hospital Association's 1992 annual survey of hospitals were matched to Organ Procurement and T

A multi-agent RAG system for generating SCORM courses from enterprise documents.

Corporate onboarding requires the effective transfer of complex organizational knowledge embedded in internal policies and procedural documents; however, existing artificial intelligence (AI)-driven course generation systems primarily target academic or public knowledge domains. This gap limits the scalability and consistency of enterprise training, particularly in regulated environments where factual accuracy is critical. In this study, we present a multi-agent pipeline that automatically generates Sharable Content Object Reference Model (SCORM) 1.2-compliant e-learning courses from heterogen

Labouring Together: Clinicians' experiences of working together to get the best outcomes in maternity care.

Interprofessional collaboration (IPC) is crucial for the safe provision of maternity care. However, IPC is poorly understood in the maternity care context, and the role of the childbearing woman within this collaboration remains unclear. The Labouring Together study used a mixed method, multi-site case study design to explore IPC and decision-making with women from the perspectives of maternity health care professionals (HCP). Case studies included a range of maternity models of care in metropolitan and regional settings in Australia. Cross-sectional surveys were used to investigate organizati
