BACKGROUND: Cirrhosis is the end stage of chronic liver disease. Cirrhosis causes portal hypertension, which, in turn, can lead to acute on chronic liver failure (ACLF), which is defined as acute decompensation combined with failure of the liver, coagulation system, kidneys, lungs, and/or circulatory system, or hepatic encephalopathy.
METHODS: This review is based on a selective literature search for international publications in the NCBI database using the keywords "liver cirrhosis" and "ACLF." Valid guidelines (up to June 2024) were also included.
RESULTS: Cirrhosis is present in approxima

Acute decompensated heart failure (ADHF) is a major health challenge globally, with high morbidity and mortality. Data from North African populations remain limited. This study aims to describe the clinical profile, precipitating factors, management, and short-term outcomes of patients admitted with ADHF to a tertiary care center in Morocco. We conducted a retrospective study of 138 Moroccan patients admitted with ADHF at Ibn Rochd University Hospital, Casablanca, between 2017 and 2019. Clinical, paraclinical, therapeutic features, and outcomes were analyzed. The mean patient age was 60.45 ± 1

Foxp3 specifies the Treg cell lineage and is indispensable for immune tolerance. Accordingly, rare Foxp3 mutations cause lethal autoimmunity. The mechanisms precipitating more prevalent human autoimmune diseases are poorly understood, but involve a combination of genetic and environmental factors. Many autoimmune diseases associate with a partial Treg-cell dysfunction, yet mouse models reflecting such complex pathophysiological processes are rare. Around 95% of Foxp3(+) Treg cells can be specifically depleted in bacterial artifical chromosome (BAC)-transgenic Depletion of REGulatory T cells (D

BACKGROUND/AIMS: Different etiologies for transient global amnesia (TGA) have been proposed, such as venous flow abnormalities, spreading depression and migraine-related mechanisms like focal ischemia. Jugular vein valve incompetence (JVI) is commonly described in TGA patients, but its contributing role in TGA pathogenesis is still unknown. We investigated the possible relationship between JVI and pathogenetic mechanisms in TGA.
METHODS: 243 TGA patients underwent clinical and neurological assessment. Demographic characteristics, TGA-precipitating factors and vascular comorbidities were caref

Hepatic encephalopathy (HE) in liver cirrhosis is a clinical manifestation of a low-grade cerebral oedema, which is exacerbated in response to ammonia and other precipitating factors. This low-grade cerebral oedema is accompanied by an increased production of reactive oxygen and nitrogen oxide species (ROS/RNOS), which trigger multiple protein and RNA modifications, thereby affecting brain function. The action of ammonia, inflammatory cytokines, benzodiazepines and hyponatraemia integrates at the level of astrocyte swelling and oxidative stress. This explains why heterogenous clinical conditio

PURPOSE: To describe the anesthetic and obstetrical management of a pregnant patient with co-existing Familial Hypokalemic Periodic Paralysis (FHPP) and Wolff-Parkinson-White syndrome (WPW).
CLINICAL FEATURES: A 29 yr-old primigravida with FHPP and WPW presented to the antenatal clinic at 18 wk gestation, for consideration of her anesthetic and obstetrical management during labour and delivery. A plan was constructed to avoid the known precipitating factors of FHPP including carbohydrate loading, cold, mental stress and exercise, which could lead to acute attacks of weakness. Frequency of painful episodes in sickle-cell disease is considered to be related to clinical severity and possibly to other aspects of the disease. Measurements of frequency often include only hospital-related or more severe, longer-lasting episodes. Since painful episodes, however, may regularly occur in nonhospital settings or be shorter-lasting with possible different pathologic effects, we measured all painful episodes in 10 adults with sickle-cell disease for 1.0-3.8 years, using a daily questionnaire. The results were related to other indices of disease severity and to possible precipita

The details of onset, perceived precipitating factors, associated symptoms, and pain distribution in the painful crisis of homozygous sickle cell (SS) disease have been prospectively recorded in 183 painful crises in 118 patients admitted to a day-care centre in Kingston, Jamaica. Painful crises developed most frequently between 3 p.m. and midnight, most commonly affected patients aged 15-29 years, affected the sexes equally, and were not obviously influenced by menstrual cycle. Of the perceived precipitating factors, skin cooling occurred in 34%, emotional stress in 10%, physical exertion in

The records of 22 patients, aged 61 to 85 years, with histologically proven cholesterol embolism (CE) were reviewed. All had one or more risk factors. Twenty-one patients had preexisting symptomatic or recognized atherosclerotic disease. Twenty patients had one or more identifiable precipitating factors, including warfarin administration (4), angiography (15), angioplasty (2), intra-aortic balloon pump placement (2), vascular surgery (11), aortitis (2), and cardiopulmonary resuscitation (1). These preceded the onset of symptoms by as many as 3 months, but usually by less than 3 weeks. Eight pa

An extract of bovine kidney obtained by trypsin digestion was used in double diffusion gel precipitation tests. Positive results were obtained with seven of twenty-seven infectious mononucleosis sera, five of twenty-five sera of patients with enterovirus diseases, and with only two of sixty normal human sera. The precipitating factor was unrelated to heterophile antibodies detectable by sheep cell haemagglutination and ox cell haemolysis. On the other hand, the precipitating factor appeared to be closely related to the serum factor previously demonstrated by mixed agglutination reactions with

Vitiligo is an acquired depigmentary disorder of the skin that results from the selective destruction of melanocytes. The etiology of vitiligo is poorly understood. There appears to be a genetic predisposition, but additional factors are probably involved. The purpose of this article is to outline the factors that might play a role in the development of vitiligo. These include trauma such as vaccination, radiotherapy, and sun exposure, malignancies and treatment of malignancies like lymphoma or melanoma, bone marrow transplantation, interferon, interleukin, and other drugs, psychological facto

OBJECTIVES: We systematically reviewed the published studies on the relationship between dengue fever and meteorological factors and applied a meta-analysis to explore the effects of ambient temperature and precipitation on dengue fever.
METHODS: We completed the literature search by the end of September 1st, 2019 using databases including Science Direct, PubMed, Web of Science, and Google Scholar. We extracted relative risks (RRs) in selected studies and converted all effect estimates to the RRs per 1 °C increase in temperature and 10 mm increase in precipitation, and combined all standardiz

Chylopericardium is a clinical condition due to the accumulation of triglyceride-rich chylous fluid within the pericardial space. It may be primary (idiopathic), without any apparent cause or secondary to some precipitating factors, such as cardiac surgery, malignancy, trauma, infection and radiation. Primary chylopericardium is rare and a diagnosis of exclusion, so far less than 150 cases have been reported in the literature. The authors present a case of recurrent primary chylopericardium diagnosed by lymphoscintigraphy.

Priapism is a urologic emergency. All patients should receive prompt urologic consultation. Management is based on prompt recognition, differentiation between low- and high-flow priapism, reversal of any potential precipitating factors, and the use of corporal aspiration/irrigation combined with intracavernosal alpha-agonist injection therapy. It cannot be over-emphasized that severely prolonged erections are associated with the development of irreversible problems with erectile function and, therefore, immediate and aggressive management is mandatory.

Exclusion diets may have a practical place in determining the precipitating dietary factors in certain clinical conditions. We present an exclusion diet which is based on the exclusion of food commonly known to cause food allergies, and the exclusion of food which contains salicylates, benzoates, tartrazine, yeast, and penicillin. This provided a basis for challenge with these additives and natural chemicals. Preliminary information in urticaria suggests a use for this diet in some allergic conditions.

The presentation of symptoms, laboratory findings and possible precipitating factors are reviewed in an 18-year-old patient with the onset of typical paroxysmal nocturnal hemoglobinuria. The opportunity was taken to study the physiology in his normal identical twin sibling who showed no hematological abnormalities on exhaustive studies, including photoscanning of the erythrocytes. These observations strengthen the evidence that PNH is an acquired disease with no basic measurable abnormalities of a hereditary or genetic nature.

BACKGROUND: The incidence and mortality from necrotizing fasciitis (NF) are increasing in New Zealand (NZ). Triggered by a media report that traditional Samoan tattooing was causing NF, we conducted a chart review to investigate the role of this and other predisposing and precipitating factors and to document NF microbiology, complications and interventions in NZ.
METHODS: We conducted a retrospective review of 299 hospital charts of patients discharged with NF diagnosis codes in eight hospitals in NZ between 2000 and 2006. We documented and compared by ethnicity the prevalence of predisposin

PURPOSE: Suicide among preteens remains understudied despite increases in suicide rates and medical encounters (e.g., emergency department visits) for suicidal ideation and attempts during the past decade. This study explores differences in individual characteristics and precipitating circumstances of suicide between preteens and teens to inform suicide prevention strategies.
METHODS: Data from the National Violent Death Reporting System were analyzed to compare characteristics and circumstances of suicide decedents aged 8-12 years (preteens) and 13-17 years (teens) between 2012 and 2021. Age

BACKGROUND: Paroxysmal respiratory failure and death occurred in two young adult females with pelvic infections. Autopsy revealed an amorphous material containing calcium obstructing the pulmonary microvasculature of each patient. Both patients received an identical total nutrient admixture (TNA) solution before their deaths.
METHODS: Infusion of TNA into an animal model was undertaken in an effort to reproduce the clinical effect. Laboratory investigation was also performed to isolate a precipitate and identify the factors contributing to precipitation.
RESULTS: A nonvisible precipitate con
