Advances in extracorporeal ventilation.

Mechanical ventilation remains the signature tool of critical care; however, within the past decade, a growing body of evidence suggests that positive pressure ventilation in acute respiratory failure is a double-edged sword that is associated with life-threatening complications such as nosocomial pneumonia and low cardiac performance. Essentially, solutions are required to provide adequate gas exchange and stable acid-base status while optimizing and maximizing pulmonary as well as remote organ protection. Recently, the first commercially available extracorporeal membrane ventilator was appro

Risk factors for adverse events in children with colds emerging from anesthesia: a logistic regression.

BACKGROUND: Recent upper respiratory infection (URI) in children increases respiratory adverse events following anesthesia for elective surgery. The increased risk continues weeks after resolution of acute URI symptoms. Few systematic analyses have explored specific risk factors. This logistic regression explores the relationship between preoperative URI symptoms and adverse events during emergence from anesthesia. METHODS: Data were combined from control groups of several prospective observational and interventional studies in elective pediatric anesthesia in a tertiary care pediatric hospit

Non-invasive ventilation: current status.

Mechanical ventilation is being increasingly used in many disciplines by specialists who manage critically ill patients. Non-invasive ventilation (NIV) refers to the method of providing assisted ventilation without endotracheal intubation. This technique requires a positive-pressure ventilator, connected by a tubing to an interface device that directs intermittent positive pressure through the upper airway and actively assists ventilation. The use of NIV is increasing for patients with acute and chronic respiratory failure, because of its convenience, lower cost and minimal complications. The

Decontamination of surgical face masks and N95 respirators by dry heat pasteurization for one hour at 70°C.

BACKGROUND: The need for protective masks greatly exceeds their global supply during the current COVID-19 pandemic. METHODS: We optimized the temperature used in the dry heat pasteurization method to destroy pathogens and decontaminate masks while retaining their filtering capacity. RESULTS: The current study showed that dry heat at both 60°C and 70°C for 1 hour could successfully kill 6 species of respiratory bacteria and one fungi species, and inactivate the H1N1 indicator virus. After being heated at 70°C for 1, 2, and 3 hours, the N95 respirators and surgical face masks showed no changes

Evaluation of a modified scavenging system to reduce occupational exposure to nitrous oxide in labor and delivery rooms.

We developed a new scavenging mask for the administration of nitrous oxide to mothers-to-be during labor in order to minimize leakage of the patient's exhaled breath into the room and, therefore, decreasing staff exposure to nitrous oxide. The scavenging system was designed with an inner mask for gas delivery, an outer mask to capture exhaled breath, and a continuous exhaust airflow based on breathing flow rates. In this study we field tested the scavenging system by measuring personal exposures of nurses (N = 30) and room air concentrations (N = 33), while patients self-administered nitrous o

Iraqi missile attacks on Israel. The association of mortality with a life-threatening stressor.

OBJECTIVE: The imminent deadline for the 1991 Persian Gulf War and, subsequently, the 18 missile attacks by Iraq on Israel represented an unusual, short-term, life-threatening stressor for an entire nation. We studied mortality in Israel in January and February 1991 to determine whether excess deaths were precipitated on days of missile attacks. DESIGN: A time-series mortality study. SETTING: The state of Israel. PARTICIPANTS: All Israelis aged 25 years and older. MAIN OUTCOME MEASURE: Daily mortality by sex, age, region, underlying cause, and place of death. RESULTS: On January 18, 1991,

The changing face of healthcare worker perceptions on powered air-purifying respirators during the SARS outbreak.

OBJECTIVES: Before the advent of severe acute respiratory syndrome (SARS), use of the powered air-purifying respirator (PAPR) in the setting of pulmonary tuberculosis has been controversial. Data regarding health care worker (HCW) perceptions and problems encountered with the use of the PAPRs were lacking. METHODOLOGY: A questionnaire-based survey was conducted of HCWs who had used the PAPR in clinical practice during the SARS outbreak, when use of the PAPR was mandatory and widespread. Evaluations of the question of whether HCWs were receptive to the use of the PAPR and their perceptions of

Blast injuries to the lung: epidemiology and management.

Lung injury is frequently a component of the polytrauma sustained by military personnel surviving blast on the battlefield. This article describes a case series of the military casualties admitted to University Hospital Birmingham's critical care services (role 4 facility), during the period 1 July 2008 to 15 January 2010. Of the 135 casualties admitted, 107 (79.2%) were injured by explosive devices. Plain chest films taken soon after arrival in the role 4 facility were reviewed in 96 of the 107 patients. In 55 (57.3%) films a tracheal tube was present. One or more radiological abnormalities w

Distribution of Droplets and Immune Responses After Aerosol and Intra-Nasal Delivery of Influenza Virus to the Respiratory Tract of Pigs.

Recent evidence indicates that local immune responses and tissue resident memory T cells (TRM) are critical for protection against respiratory infections but there is little information on the contributions of upper and lower respiratory tract (URT and LRT) immunity. To provide a rational basis for designing methods for optimal delivery of vaccines to the respiratory tract in a large animal model, we investigated the distribution of droplets generated by a mucosal atomization device (MAD) and two vibrating mesh nebulizers (VMNs) and the immune responses induced by delivery of influenza virus b

Oropharyngeal dysphagia telerehabilitation in the Intensive Care Unit for COVID-19: a case report.

The face-to-face assessment of and training for dysphagia are considered aerosol-generating procedures, and thus are contraindicated for patients who are positive or suspected of having severe acute respiratory syndrome coronavirus 2 infection. Considering the extremely infectious nature of the virus, transmission to other individuals during rehabilitation is possible. Some patients in the intensive care unit and those who are on endotracheal intubation and mechanical ventilation often have dysphagia. Therefore, assessment and training for oropharyngeal dysphagia are provided by rehabilitation

Comfortable SCBA Weights from Biomechanical Models for Firefighting Tasks.

Firefighters are required to use a self-contained breathing apparatus (SCBA) for respiratory protection when engaged in a variety of firefighting duties. While the SCBA provides crucial respiratory support and protection, it is also cumbersome and heavy. Questions have arisen to address the current weight limitation listed in 42 CFR Part 84, which mandates the maximum weight of 16kg (35.2 lbs.) for an SCBA system. The objective of the overarching study is to inform a maximum comfortable SCBA weight by developing static and dynamic biomechanical models and collecting physiological burden and po

[Chest Wall Injury].

A traumatic chest wall injury is a condition that is often encountered in medical practice, but the underlying pathophysiology varies widely depending on the circumstances of the injury and the patient's background. The chest wall, which protects vital organs and provides respiratory movement, can be a life-threatening emergency when injured, thus it is important to respond promptly and appropriately. Because a penetrating chest wall trauma is often associated with intra-thoracic organ damage, it is important to consider an emergency thoracotomy in cases involving massive hemorrhage or air lea

Tracheostomy in mechanical ventilation.

Airway access for mechanical ventilation (MV) can be provided either by orotracheal intubation (OTI) or tracheostomy tube. During episodes of acute respiratory failure, patients are commonly ventilated through an orotracheal tube that represents an easy and rapid initial placement of the airway device. OTI avoids acute surgical complications such as bleeding, nerve and posterior tracheal wall injury, and late complications such as wound infection and tracheal lumen stenosis that may emerge due to tracheostomy tube placement. Tracheostomy is often considered when MV is expected to be applied fo

Fit testing of N95 respirators: A pilot study on respirator design and demographic factors.

N95 filtering facepiece respirators (FFRs) approved by the National Institute for Occupational Safety and Health are widely used by healthcare workers to prevent the inhalation of hazardous airborne contaminants. Various nose-clip shapes and respirator styles in N95 FFRs are designed to eliminate gaps between the respirator and the wearer's facial skin, ensuring adequate protection for the wearer. However, variations in facial dimensions across sexes and races may affect the fit and, consequently, the performance of these respirators. The primary objective of this study was to assess the effec
