Ethical Challenges in Managing a Displaced Neer III Humeral Head Fracture in an Undocumented Immigrant With Early Alzheimer's Disease: A Case Report.

This report describes an 82-year-old undocumented Peruvian female immigrant presenting to the ED with right elbow pain and limited shoulder … ndocumented status and lack of health insurance, she was required to pay the full cost of a reverse shoulder prosthesis (€24,000), which she could not afford. Her Alzheimer's-related dependency and absence of family support in Peru prevented seeking treatment abroad, resulting in discharge with a sling. Comprehensive blood tests ruled out metabolic contributors to he …

Is revision to anatomic shoulder arthroplasty still an option? A systematic review.

BACKGROUND: With the historical complications when using total shoulder arthroplasty (TSA) to revise failed arthroplasties, and the success of the reverse prosthesis in the revision setting, the question arises whether revision to TSA is still a reasonable option? This systematic review examines revision to TSA and the factors associated with outcomes. METHODS: A systematic review was performed for studies of TSA used to revise a failed hemiarthroplasty or TSA. The primary outcome was implant failure leading to a repeat revision arthroplasty. Secondary outcomes included visual analog scale (V

The effect of vitamin E-enhanced cross-linked polyethylene on wear in shoulder arthroplasty-a wear simulator study.

BACKGROUND: Wear of the polyethylene glenoid component and subsequent particle-induced osteolysis remains one of the most important modes of failure of total shoulder arthroplasty. Vitamin E is added to polyethylene to act as an antioxidant to stabilize free radicals that exist as a byproduct of irradiation used to induce cross-linking. This study was performed to assess the in vitro performance of vitamin E-enhanced polyethylene compared with conventional polyethylene in a shoulder simulator model. METHODS: Vitamin E-enhanced, highly cross-linked glenoid components were compared with convent

Effects of different humeral stem lengths on stem alignment and proximal stress shielding in reverse total shoulder arthroplasty.

BACKGROUND: Few comparative studies on the correlation between stem length, stem alignment, and/or stress shielding have been conducted in reverse total shoulder arthroplasty (rTSA). This study aimed to investigate the effects of different humeral stem lengths on stem alignment and proximal stress shielding after rTSA. METHODS: A total of 320 patients who underwent primary rTSA from October 2010 to May 2020 with at least 2 years of follow-up (mean follow-up: 32.6 months) were retrospectively reviewed. The participants were classified into 3 groups according to the humeral stem length of diffe

Analysis of three different reverse shoulder arthroplasty designs for cuff tear arthropathy - the combination of lateralization and distalization provides best mobility.

BACKGROUND: The two major reverse shoulder arthroplasty (RSA) designs are the Grammont design and the lateralized design. Even if the lateralized design is biomechanically favored, the classic Grammont prosthesis continues to be used. Functional and subjective patient scores as well as implant survival described in the literature so far are comparable to the lateralized design. A pure comparison of how the RSA design influences outcome in patients has not yet been determined. The aim of this study was a comparison focused on patients with cuff tear arthropathy (CTA). METHODS: We analyzed regi

Humeral stem alignment of curved short stem in reverse shoulder arthroplasty: varus/valgus alignment is not a determinant for clinical outcomes.

BACKGROUND: This study aimed to investigate the influence of curved short stem alignment within the proximal humerus on the clinical outcomes of reverse shoulder arthroplasty. We hypothesized that the varus/valgus alignment would yield clinical outcomes comparable to those of a neutral alignment. METHODS: We retrospectively reviewed 167 patients who underwent reverse shoulder arthroplasty using the Aequalis Ascend Flex implant between January 2017 and December 2021. The study categorized the patients into 3 groups based on the angle difference between the humeral axis and the stem axis: neutr

An unusual case of clavicular prosthesis migration causing traumatic injury to the ascending aorta.

Traumatic aortic injuries can be a lethal event. Almost 88% of patients with traumatic aortic injuries die within the first hour and only 2% survive long enough to develop a chronic aneurysm. Injury to the ascending aorta, whether acute or chronic, are typically managed with surgery, and those in the descending aorta, are managed conservatively or in some cases with stents. We present a rare case of a 53-year old gentleman with intra-aortic migration of a left clavicular prosthesis used for restoration of the left shoulder girdle.

Amputee, clinician, and regulator perspectives on current and prospective upper extremity prosthetic technologies.

Existing prosthetic technologies for people with upper limb amputation are being adopted at moderate rates. Once fitted for these devices, many upper limb amputees report not using them regularly or at all. The primary aim of this study was to solicit feedback about prosthetic technology and important device design criteria from amputees, clinicians, and device regulators. We compare these perspectives to identify common or divergent priorities. Twenty-one adults with upper limb loss, 35 clinicians, and 3 regulators completed a survey on existing prosthetic technologies and a conceptual sensor

Granulomatous Reaction on a Double-Level Cervical Total Disc Arthroplasty.

BACKGROUND: Cervical total disc arthroplasty (TDA), or cervical artificial disc replacement, is an alternative technique to anterior cervical discectomy and fusion for treatment of symptomatic degenerative cervical spine disease. The main goal of TDA is to maintain cervical motion and lower the risk of deterioration of adjacent levels. Granuloma formation on a cervical TDA is exceptional. CASE DESCRIPTION: A 48-year-old woman with left cervicobrachialgia underwent a double-level TDA (M6-C Artificial Cervical Disc) on C5-C6 and C6-C7 at another hospital in 2010. Two years later, she reported a

Adaptive glenoid bone remodeling simulation.

Glenoid prosthesis loosening is the most common cause for revision total shoulder arthroplasty. Stress-induced bone remodeling may compromise long-term prosthesis fixation and significantly contribute to loosening. Realistic, robust analysis of bone-prosthesis constructs need to look beyond initial post-implantation mechanics provided by static finite element (FE) simulation. Adaptive bone remodeling simulations based on Wolff's law are needed for evaluating long-term glenoid prostheses fixation. The purpose of this study was to take a first step towards this goal and create and validate two-d

Clinical and radiological results of the SIMPLICITI* stemless shoulder arthroplasty: a twenty five patients retrospective cohort.

PURPOSE: For preserving the humeral bone stock, some surgeons proposed a stemless humeral prosthetic component. This study reports the functional and radiologic results of the stemless anatomic prosthesis Simpliciti*(Tornier, Wright, Stryker), with the hypothesis that it can achieve a good metaphyseal fixation. METHODS: 28 patients underwent 30 shoulder replacements with the Simpliciti* humeral prosthesis followed for an average of three years (2 months to 8 years). The clinical outcome used the Constant-Murley score and the Bankes resistance and force evaluation. The radiologic assessment lo

Scapulothoracic dissociation secondary to major shoulder trauma.

Three skeletally immature patients with scapulothoracic dissociation were reviewed. A 5-year-old child's arm, caught in a conveyor belt, led to complete upper extremity amputation. Multiple fractures, muscular damage, and cutaneous and subcutaneous tissue disruption (degloving) were present throughout the avulsed extremity. Replantation was considered, but not carried out because of these extensive injuries. He was treated with a myoelectric prosthesis. Two older boys had scapulothoracic dissociation (one open, one closed) associated with clavicular diaphyseal fractures following blunt trauma.

Mid-term clinical and radiographic outcomes at minimum five-year follow-up after stemless reverse shoulder arthroplasty.

BACKGROUND: Stemless reverse total shoulder arthroplasty (rTSA) was developed to preserve humeral bone stock and facilitate revision surgery. While short-term outcomes are promising, mid-term data beyond five years remain limited. The purpose of this study was to evaluate clinical and radiographic outcomes at a minimum follow-up of five years after implantation of a stemless reverse shoulder prosthesis. We hypothesized that metaphyseal fixation would demonstrate sustained clinical function and stable radiographic integration at mid-term follow-up. METHODS: This retrospective case series inclu…

Analysis of polyethylene wear of reverse shoulder components: A validated technique and initial clinical results.

One of the most prevalent phenomena associated with reverse total shoulder arthroplasty (rTSA) is scapular notching. Current methods examine only the damage to the scapula and no methods are available for quantifying the total wear volume of the polyethylene humeral bearing. Quantifying the polyethylene material loss may provide insight into the mechanism for scapular notching and into the particle dose delivered to the patient. A coordinate measurement machine (CMM) and custom computer algorithms were employed to quantify the volumetric wear of polyethylene humeral bearings. This technique wa

Robert H. Cofield, MD, Award for Best Oral Presentation 2023: Up to 8 mm of glenoid-sided lateralization does not increase the risk of acromial or scapular spine stress fracture following reverse shoulder arthroplasty with a 135° inlay humeral component.

BACKGROUND: Glenoid-sided lateralization in reverse shoulder arthroplasty (RSA) decreases bony impingement and improves rotational range of motion, but has been theorized to increase the risk of acromial or scapular spine fractures (ASFs). The purpose of this study was to assess if glenoid-sided lateralization even up to 8 mm increases the risk for stress fracture following RSA with a 135° inlay humeral component. METHODS: A retrospective review was performed from a multicenter prospectively collected database on patients who underwent primary RSA from 2015 to 2021. All RSAs were performed wi

The effect of glenohumeral radial mismatch on different augmented total shoulder arthroplasty glenoid designs: a finite element analysis.

BACKGROUND: Augmented glenoid implants to correct bone loss can possibly reconcile current prosthetic failures and improve long-term performance for total shoulder arthroplasty. Biomechanical implant studies have suggested benefits from augmented glenoid components, but limited evidence exists on optimal design. METHODS: An integrated kinematic finite element analysis (FEA) model was used to evaluate optimal augmented glenoid design based on biomechanical performance in translation in the anteroposterior plane similar to clinical loading and failure mechanisms with osteoarthritis.
