The effect of spironolactone on ventricular tachyarrhythmias in patients with implantable cardioverter-defibrillators.

BACKGROUND: Previous studies have suggested that aldosterone blockade can reduce the incidence of ventricular tachycardia (VT) or ventricular fibrillation (VF) in patients with heart failure. The SPIronolactone to Reduce ICD Therapy (SPIRIT) trial was designed to test the hypothesis that spironolactone reduces the incidence of VT/VF in patients with implantable cardioverter-defibrillators (ICDs) who are at moderately high risk for recurrent VT/VF. METHODS AND RESULTS: Ninety patients who had ICDs who were at moderately high risk for recurrent VT/VF and who were not candidates for spironolacto…

Effect of spironolactone on patients with atrial fibrillation and structural heart disease.

BACKGROUND: Several studies have shown that the modulation of fibrotic scar in cardiac diseases has beneficial effects on cardiac arrhythmias. In addition, recent reports suggest a potential role of mineralocorticoid receptor upregulation in atrial fibrillation (AF). The role of spironolactone, a mineralocorticoid receptor blocker and a potent antifibrotic agent, in AF is as yet unexplored. The aim of this study was to determine if spironolactone, a mineralocorticoid receptor blocker with potent antifibrotic properties, has beneficial effects on AF. HYPOTHESIS: Spironolactone therapy in patie…

Impact of spironolactone on endothelial function in patients with single ventricle heart.

BACKGROUND: Mid-term survivors of the Fontan procedure are at risk for progressive heart failure, and endothelial dysfunction is thought to contribute to this process. Aldosterone antagonism has been shown to improve survival in adults with heart failure and the effects are mediated in part by changes in endothelial function. In the present study, we sought to determine if a short course of spironolactone improves endothelial function and alters serum cytokine profiles in adolescents and adults with single ventricle heart. METHODS: Subjects had baseline assessment of flow-mediated dilation an…

Low-dose spironolactone in the management of resistant hypertension: a surveillance study.

METHODS: We have conducted an open observational study of the use of spironolactone 25-50 mg in the management of patients with resistant hypertension. This drug was recommended in 133 patients who were already receiving an angiotensin-blocking drug in addition to other therapies. RESULTS: Of these, three defaulted from follow-up and 11 could not tolerate spironolactone. We therefore have outcome data on 119 patients. The addition of spironolactone (median dose 25 mg) was associated with a mean (SD) fall in systolic blood pressure of 21.7 mmHg (24.0; P < 0.001) and diastolic blood pressure of…

Spironolactone effects on myocardium changes induced by thyroid hormone in rats.

BACKGROUND: To study the possible role of aldosterone on thyroid hormone-induced myocardium hypertrophy, using spironolactone. OBJECTIVE: To evaluate morphological changes in the myocardium induced by thyroid hormone and the possible effects of spironolactone use on these alterations. METHODS: Forty Wistar rats were studied. The animals were allocated to four groups and were given: the vehicle used for dilution of the thyroid hormone (C); sodium levothyroxin at 50 microg/rat/day (H); spironolactone, 0.3 mg/kg/day (S); or thyroid hormone plus spironolactone (HS), at the same doses mentioned a…

Spironolactone with ACE inhibitor is effective in gross hematuria caused by nephroptosis.

Gross hematuria caused by nephroptosis is difficult to treat and sometimes requires surgical treatment. We experienced a case of a 64-year-old woman with gross hematuria due to nephroptosis and glomerular thin basement membrane that showed remission of hematuria with the blockade of the renin-angiotensin-aldosterone system with an angiotensin-converting enzyme inhibitor and spironolactone.

Use and side-effect profile of spironolactone in a private cardiologist's practice.

BACKGROUND: The beneficial effects of spironolactone on the treatment of cardiovascular diseases are well known, but translating these benefits into private practice can be difficult because of the drug's side-effect profile. HYPOTHESIS: When patients are monitored over the long term, spironolactone can be used safely with an acceptable side-effect profile. METHODS: We retrospectively studied 762 patients taking spironolactone over a 7-year period in a cardiologist's referral-based practice and monitored them for side effects from the medication. RESULTS: Data were available on 762 patients…

Differences in the determinants of eplerenone, spironolactone and aldosterone binding to the mineralocorticoid receptor.

The importance of mineralocorticoid receptor (MR) antagonists in the treatment of cardiovascular disease has been emphasised by two recent clinical trials, one using spironolactone and the other using a new selective MR antagonist, namely eplerenone. Eplerenone has a very low affinity for the glucocorticoid receptor (GR). Determinants of binding specificity of eplerenone to the MR were investigated using chimeras created between the ligand-binding domains (LBD) of the MR and the GR. These chimeras had been used previously to investigate aldosterone and spironolactone binding specificity to the…

Spironolactone in left-sided heart failure: how does it fit in?

The familiar diuretic spironolactone has taken on new life as a treatment for left-sided congestive heart failure. Spironolactone has been shown to decrease mortality in such patients who are New York Heart Association class IV. It can be used in addition to agents such as angiotensin-converting enzyme inhibitors and beta blockers, which also decrease mortality, and diuretics and digoxin, which are useful in treating symptoms. Spironolactone is safe, easy to use and reasonably priced. More research is necessary to determine the order and combinations of these medications in slowing the progres…

Effect of spironolactone on dimethyl mercury toxicity. A possible molecular mechanism.

The protective activity of spironolactone (CAS 52-01-7) against dimethyl mercury intoxication was studied. Dimethyl mercury increased serum glutamate pyruvate transaminase (SGPT), serum bilirubin, blood urea nitrogen (BUN), and caused impairment of the drug metabolic activity of rat liver in vivo and in vitro. It also caused a severe neuropathy to these animals. Administration of spironolactone caused a reduction of dimethyl mercury toxicity. It decreased the values of SGPT, bilirubin and BUN, and restored the impaired drug metabolism caused by dimethyl mercury. The neuropathy produced after a…

Administration time-dependent change in the effect of spironolactone in aged rats.

We have previously demonstrated that the natriuretic effect of spironolactone, a competitive antagonist of mineralocorticoid, varies with its administration time in young rats. The present study was undertaken to examine the influence of aging on chronopharmacological profiles of the agent. Spironolactone (10 and 50 mg/kg) was given orally at 12 am or 12 pm in young (12 weeks old) and aged (28 months old) Wistar rats. The 8-hour urine was collected after each administration, and the urinary sodium excretion was determined. The urinary sodium excretion increased dose-dependently following spiro…

Lack of effect of spironolactone on hair shaft diameter in hirsute females.

Because of its anti-androgenic activity spironolactone 50-200 mg daily is advocated widely for the management of female hirsutism but this practice lacks adequately controlled experimental support. In a double-blind placebo controlled study of the efficacy of spironolactone 100 mg daily in idiopathic hirsutism we were unable to demonstrate objective benefit from spironolactone treatment. The mean effect of spironolactone given over a 9 month period was to increase hair shaft diameter by +15% with 95% CI (-0.4% to +29%). In addition there were no changes in circulating serum androgen concentrat…

[Electrophysiologic effects of a spironolactone-altizide combination on the isolated rat heart].

The association of spironolactone and altizide is effective in the treatment of some forms of resistant oedemas. Our aim was to determine whether the renal effects of this association were associated with abnormal cardiac cellular electrophysiological changes. Isolated rat hearts perfused by the retrograde aortic approach and paced at 200 beats per minute were studied. Cardiac cellular activation was measured by floating microelectrodes in the subepicardial layers of the left ventricle. The following parameters were analysed: the duration of the action potential at 25% and 75% of repolarisatio…
