Non-intubated vs. intubated video-assisted thoracoscopic surgery for the treatment of thoracic diseases: a systematic review and meta-analysis of propensity score-matched cohorts.

BACKGROUND: Video-assisted thoracic surgery (VATS) is commonly conducted under general anesthesia with tracheal intubation, which can lead to intubation-related complications. Consequently, non-intubated video-assisted thoracoscopic surgery (NIVATS) has gained increasing attention. However, debates persist regarding its safety and efficacy. This study aims to compare the safety and efficacy of NIVATS vs. intubated VATS (IVATS) in thoracic surgery. METHODS: Relevant literature published up to October 2024 was collected from the Cochrane, PubMed, and Embase databases based on predefined inclusi

Performance of wider parenchymal lung resection than preoperatively planned in patients with low preoperative lung function performance undergoing video-assisted thoracic surgery major lung resection.

OBJECTIVES: Pulmonary assessment before major lung resections is used to determine patient's operability. In marginal cases, extensive pulmonary assessment is additionally important under the fear of a more radical parenchymal removal. This study investigates the outcome of wider lung parenchymal resections in patients with low lung functional status undergoing video-assisted thoracic surgery (VATS) major lung resection. METHODS: The medical records of patients who underwent VATS major lung resection for cancer, over a period of 5 years (August 2009-August 2014), were retrospectively reviewed

Effect of Intravenous Lidocaine on Serum Interleukin-17 After Video-Assisted Thoracic Surgery for Non-Small-Cell Lung Cancer: A Randomized, Double-Blind, Placebo-Controlled Trial.

PURPOSE: Surgical stress promotes tumor metastasis. Interleukin (IL)-17 plays a pivotal role in cancer progression, and high IL-17 expression predicts poor prognosis of non-small-cell lung cancer (NSCLC). Lidocaine may exert tumor-inhibiting effects. We hypothesize that intravenous lidocaine attenuates surgical stress and reduces serum IL-17 levels during video-assisted thoracic surgery (VATS) for NSCLC. METHODS: This randomized, double-blind, placebo-controlled trial included 60 early-stage NSCLC patients undergoing VATS, into a lidocaine group (n = 30; intravenous lidocaine bolus 1.0 mg/kg,
