Hemodynamic effects of high-frequency oscillatory ventilation in preterm neonates with respiratory distress syndrome.

BACKGROUND: High-frequency oscillatory ventilation (HFOV) can have negative hemodynamic effects in neonates. We aimed to assess systemic, cerebral, and cardiac hemodynamic changes in preterm neonates with respiratory distress syndrome (RDS) on HFOV. METHODS: This observational study was conducted from June 2017 until May 2019 on 100 preterm neonates with RDS that needed switching from conventional mechanical ventilation to HFOV. Initial and Follow up capillary blood gas, echocardiographic examination, cranial ultrasound, and Doppler study of cerebral, celiac, superior mesenteric, and renal ar

Mechanisms used to restore ventilation after partial upper airway collapse during sleep in humans.

BACKGROUND: Most patients with obstructive sleep apnoea (OSA) can restore airflow after an obstructive respiratory event without arousal at least some of the time. The mechanisms that enable this ventilatory recovery are unclear but probably include increased upper airway dilator muscle activity and/or changes in respiratory timing. The aims of this study were to compare the ability to recover ventilation and the mechanisms of compensation following a sudden reduction of continuous positive airway pressure (CPAP) in subjects with and without OSA. METHODS: Ten obese patients with OSA (mean (SD

Nebulized Antibiotics for Healthcare- and Ventilator-Associated Pneumonia.

Global emergence of multidrug-resistant and extensive drug-resistant gram-negative bacteria has increased the risk of treatment failure, especially for healthcare- or ventilator-associated pneumonia (HAP/VAP). Nebulization of antibiotics, by providing high intrapulmonary antibiotic concentrations, represents a promising approach to optimize the treatment of HAP/VAP due to multidrug-resistant and extensive drug-resistant gram-negative bacteria, while limiting systemic antibiotic exposure. Aminoglycosides and colistin methanesulfonate are the most common nebulized antibiotics. Although optimal n

High-frequency oscillatory ventilation versus synchronized intermittent mandatory ventilation plus pressure support in preterm infants with severe respiratory distress syndrome.

BACKGROUND: Mechanical ventilation and surfactants are the standard treatment of preterm respiratory distress syndrome (RDS). The effects of the primary ventilation model on bronchopulmonary dysplasia (BPD) and long-term neurodevelopment outcomes are controversial. The purpose of this study was to compare the efficacy and safety of high-frequency oscillatory ventilation (HFOV) and synchronized intermittent mandatory ventilation plus pressure support ventilation (SIMV-PSV) in preterm infants with severe RDS. METHODS: A total of 366 eligible preterm infants were randomly assigned to treatment w

Negative-pressure pulmonary oedema in a patient undergoing shoulder arthroscopy.

An 18-year-old ASA-I patient who underwent elective left shoulder arthroscopy developed severe airway obstruction post-extubation due to fluid extravasation from the shoulder joint into the neck and airway tissue. Re-intubation for relief of obstruction resulted in negative-pressure pulmonary oedema. The patient was electively ventilated in the intensive care unit and recovered uneventfully. A high index of suspicion along with monitoring of neck circumference can prevent this kind of complication.

Anesthetic Management of Thoracoscopic Pulmonary Cystectomy in a Patient With Fontan Circulation and Disturbed Lung Inflation During Leakage Testing: A Case Report.

Congenital heart disease may require multiple cardiac surgeries during childhood. Subsequent non-cardiac surgeries increase the perioperative bleeding and hypoxia because of changes in circulation. An 18-year-old male patient with a history of multiple cardiac interventions, including Fontan surgery, underwent a thoracoscopic right lung suture and coverage for recurrent right spontaneous pneumothorax under general anesthesia with one lung ventilation (OLV). The superior dorsal and inferior lobes, which were inflatable before surgery, failed to expand during leakage testing. The trachea's condi

Prospective evaluation of epidural versus intrapleural catheters for analgesia in chest wall trauma.

Severe blunt chest trauma can produce multiple rib fractures, flail segments, and pulmonary contusions. All of these injuries produce pain and diminished pulmonary function. The effectiveness of intrapleural and epidural administration of bupivacaine was prospectively evaluated in 19 patients with severe chest trauma. Pain relief and pulmonary function were evaluated for 72 hours after catheter placement. Epidural administration of bupivacaine significantly reduced pain at rest and with motion compared with the intrapleural route (p < 0.05). Parenteral narcotic use was also significantly less

Use of sole-agent ketamine sedation to facilitate laryngoscopy and foreign body removal in acute supraglottic airway obstruction.

A 78-year-old man presented with stridor and dysphonia after choking at home. He deteriorated, with respiratory distress and an oxygen saturation of 55% despite oxygen via a non-rebreather mask. Flexible nasendoscopy demonstrated a food bolus between the epiglottis and glottis. The team judged apnoea and assisted positive-pressure ventilation before extraction to be hazardous, while deterioration and limited cooperation made an awake technique impracticable. Sole-agent ketamine sedation facilitated videolaryngoscopy with clinically observed spontaneous breathing; the bolus, a slice of carrot,

Agreement Between Automated and Equation-Derived Mechanical Power in Pediatric Mechanical Ventilation.

BACKGROUND: Newer ventilators provide automated real-time mechanical power (MPauto) measurements, but their accuracy in children across ventilation modes remains uncertain. We aimed to evaluate the agreement between the MPauto measured on the Mindray SV-600 ventilator and the equation-derived MP (MPcalc) across two tidal volume (VT) settings, ventilation modes, and predefined age groups in ventilated children. METHOD: In this prospective observational physiology study, consecutive mechanically ventilated children aged 1 month to 18 years were enrolled between August 2024 and May 2025 in a 30-

Hemodynamic effects of high frequency oscillatory ventilation with volume guarantee in a piglet model of respiratory distress syndrome.

Respiratory failure is a common condition faced by critically ill neonates with respiratory distress syndrome (RDS). High frequency oscillatory ventilation (HFOV) is often used for neonates with refractory respiratory failure related to RDS. Volume guarantee (VG) mode has been added to some HFOV ventilators for providing consistent tidal volume. We sought to examine the impact of adding the VG mode during HFOV on systemic and cerebral hemodynamics, which has not been studied to date. A neonatal piglet model of moderate to severe RDS was induced by saline lavage. Piglets (full term, age 1-3 day

Fluid flow and particle transport in mechanically ventilated airways. Part II: particle transport.

The flow mechanisms that play a role on aerosol deposition were identified and presented in a companion paper (Timothy et al. in Med Biol Eng Comput. doi: 10.1007/s11517-015-1407-3 , 2015). In the current paper, the effects of invasive conventional mechanical ventilation waveforms and endotracheal tube (ETT) on the aerosol transport were investigated. In addition to the enhanced deposition seen at the carinas of the airway bifurcations, enhanced deposition was also seen in the right main bronchus due to impaction and turbulent dispersion resulting from the fluid structures created by jet cause

Ventilatory characteristics in mechanically ventilated patients during manual hyperventilation for chest physiotherapy.

We measured the tidal volumes and peak inflation pressures generated during manual hyperventilation for chest physiotherapy in 25 adult ventilated patients. The average tidal volume ranged from 711 to 1511 ml, with a mean (SD) of 1120 (274) ml. There was a negative correlation (p < 0.05) between the average tidal volume and the lung injury, as measured by the Murray lung injury score. The average peak inflation pressure ranged from 37 to 74 cmH2O with a mean (SD) of 51.5 (7.6) cmH2O. There was a positive correlation (p < 0.05) between average peak inflation pressure and the lung injury score.

Temperature instability as an early predictive factor of brain death in paediatric near-drowning victims.

Predicting outcome after near-drowning has been extensively studied. During four years, 42 near drowned children were aggressively treated with positive pressure ventilation, barbiturates and hypothermia. This mode of treatment makes it difficult to clinically assess the child's prognosis. Temperature instability after rewarming is an early negative predictive factor following treatment of near-drowning, and in conjunction with cerebral flow studies avoids the potential commitment to prolonged and unwarranted cardiovascular and respiratory support.

[Pulmonary edema due to acute airway obstruction immediately after tracheal extubation].

A 33-year-old male was scheduled for tonsillectomy and pharyngoplasty due to sleep apnea syndrome. The intubation was uneventful following induction with thiamylal and vecuronium. Anesthesia was maintained with O2-N2O-sevoflurane. No complications were observed during the 90 min operation. After the termination of the anesthesia, a hyperadrenergic state was observed: arterial pressure and heart rate rose to 230/135 mmHg and 135 bpm, respectively. Immediately after extubation, he developed dyspnea with tracheal tag and stridor, and became cyanotic despite the use of a simple oxygen mask and ass
