Pediatric nasal fractures: evaluation and management.

Nasal fractures have been reported as 1 of the 3 most commonly encountered pediatric facial bone fractures. The most common causes of nasal fractures in this age group are auto accidents (40%), sports injuries (25%), intended injuries (15%), and home injuries (10%). Nasal fractures are usually treated with closed reduction (Higuera S, Lee EI, Stal S. Nasal trauma and the deviated nose. Plast Reconstr Surg 2007;120:64S-75S). This results in a significant incidence of posttraumatic deformities, often requiring secondary surgical treatment. For this reason, it is paramount to pay careful attentio

A traumatic pseudoaneurysm of the vertebral artery.

Although uncommon, vertebral artery pseudoaneurysms harbour significant risk of embolic stroke and their presence should be considered in cases of blunt cervical trauma. We illustrate a case of a traumatically ruptured vertebral artery pseudoaneurysm treated with coil embolisation.

The management of complex fractures of the proximal tibia with minimal intra-articular impaction in fragility patients using intramedullary nailing and compression bolts.

BACKGROUND: Intra-articular fractures of the proximal tibia that extend to the meta-diaphyseal part of the bone represent a severe injury, especially if they occur in osteoporotic patients. Current treatment modalities include either internal fixation with traditional or modern plating techniques or external fixation with circular frames or hybrid systems. However, problems and complications related with these techniques are increasing with age and future reconstructive operations, such as arthroplasty, may be jeopardised. METHOD: This is a prospective pilot study about a novel type of osteos

Traumatic avulsion and reconstruction of the midface.

Traumatic loss of midface soft tissue and supporting structures may result in communication between the oral and nasal cavities. Reconstruction requires both oral and nasal lining, as well as supporting structures. The need for multilaminar tissue, as well as the paucity of local tissue, creates a reconstructive challenge. This case report describes the reconstruction of a traumatic defect of the alveolus, hard palate, inferior orbits, and local soft tissues. An intraoperative alginate mold facilitated a three-dimensional understanding of the wound, and allowed translation of an osseomyocutane

Flap Impaling: A Simple and Effective Technique in Soft Tissue Reconstruction of Complex Extremity Wounds.

Composite defects due to trauma present with compound comminuted fractures with soft tissue loss. Stabilization requires multiple Schanz pins across the fracture segments and flap cover for soft tissue reconstruction. These pins hinder flap inset and may require complex planning with multiple flaps. A simple approach is to impale the flaps over these pins to achieve flap inset without disturbing the skeletal fixation. This method of impaling was utilized for free as well as pedicled fasciocutaneous and muscle flaps. A stab incision is made and careful dissection is done to create a passage for

Seismic shifts in the geochemical and microbial composition of a Yellowstone aquifer.

Seismic energy, like that released by earthquakes, can fracture rock and thereby alter subsurface fluid flow paths, release substrates from inclusions, and expose fresh mineral surfaces capable of reacting with water. However, it is unclear how such seismic-induced changes influence microbial communities. Volcanically active areas experience frequent seismic activity and thus represent ideal locations to examine the influence of seismic-induced geochemical change on subsurface microbial communities. Here, we demonstrate that energy released in an earthquake swarm in 2021 correlated with extens

Management of metacarpal shaft fractures: a survey of current UK practice.

INTRODUCTION: Metacarpal shaft fractures (MSF) are common injuries that predominantly affect young, economically active people. However, there is limited evidence to guide their management. The aims of this study were to: evaluate the management of extra-articular MSF of the fingers; assess equipoise for surgical and nonsurgical treatments; and explore factors influencing clinician decision making to inform the design of a randomised controlled trial (RCT) comparing surgical and nonsurgical treatments. METHODS: A cross-sectional, web-based survey was distributed to UK hand surgeons using memb

EXPANDABLE INTRAVERTEBRAL IMPLANTS IN POST-TRAUMATIC VERTEBRAL NECROSIS - NEW CLASSIFICATION SUGGESTION.

The progressive evolution of post-traumatic vertebral necrosis and consequent loss of structural integrity of the vertebral body along with neurological risk, makes it one of the most feared and unpredictable pathologies in spine traumatology. Several studies have addressed the role of vertebroplasty, kyphoplasty, and corpectomy in its treatment; however, it remains a controversial concept without a defined therapeutic algorithm. The recent emergence of expandable intravertebral implants, which allow, by a percutaneous transpedicular application, the capacity for intrasomatic filling and maint

Reconstruction of a Large Posttraumatic Mandibular Defect Using Bone Tissue Engineering With Fresh-Frozen Humeral Allograft Seeded With Autologous Bone Marrow Aspirate and Vascularized With a Radial Forearm Flap.

INTRODUCTION: Currently, vascularized autologous bone transplantation is considered the gold standard for large mandibular continuity defect reconstruction. Donor site morbidity is a major concern. Therefore, bone tissue engineering (BTE) seems to be the ideal solution. Fresh-frozen bone allograft is the closest material to autologous bone. The purpose of this clinical report is to show a new technique of large mandibular continuity defect reconstruction using a fresh-frozen humeral allograft seeded with autologous iliac bone marrow aspirate and vascularized with a radial forearm flap. METHOD

Comparison of Radiographic Outcomes of Surgical Treatment in Patients with Distal Radial Fractures.

BACKGROUND: Unstable and comminuted distal radial fractures require surgical treatment by percutaneous insertion of Kirschner wires, open reduction, and fixation with a non-locking or locking plate or with an external device. Choosing a surgical method that produces "better" outcomes may help select the most efficient treatment method. MATERIAL AND METHODS: The study group included 100 patients after surgical treatment by closed reduction and simple fixation with Kirschner wires and by open reduction and LCP locking plate fixation. Radiographic assessment was based on images obtained before t

Combination of the AO-Magerl and load-sharing classifications for the management of thoracolumbar burst fractures.

The AO-Magerl classification is widely accepted for the appropriate management of patients with thoracolumbar burst fractures; however, it fails to assess the ability of the injured spine to withstand compressive loading and cannot predict instrumentation failure after short-segment posterior fixation. The load-sharing classification depends on the degree of comminution and apposition of bony fragments. We retrospectively classified according to both classifications 100 consecutive patients with 1-level thoracolumbar burst fractures treated nonoperatively or operatively within a 7-year period.

The "Z-effect" phenomenon defined: a laboratory study.

The Z-effect phenomenon is a potential complication of two lag screw intramedullary nail designs used for fixation of intertrochanteric hip fractures, in which the inferior lag screw migrates laterally and the superior lag screw migrates medially during physiologic loading. The current investigation was undertaken in an attempt to reproduce the Z-effect phenomenon in a laboratory setting. Sixteen different simulated femoral head and neck constructs having varying compressive strengths were created using four densities of solid polyurethane foam and instrumented with a two-screw cephalomedullar

Factors influencing the mid-term radiological and functional outcomes of 41 post-fracture bipolar radial head arthroplasty cases at a mean follow-up of 87 months.

INTRODUCTION: When treating comminuted radial head fractures that cannot be adequately fixed, the next option is arthroplasty (radial head replacement). We hypothesized that the radiological and functional outcomes of bipolar mobile cup radial head arthroplasty is not influenced by the length of follow-up but instead by the correct positioning of the implant intraoperatively and by the presence of associated bone or ligament injuries. PATIENTS AND METHODS: Between May 1998 and December 2016, 82 cases of radial head arthroplasty were performed at our hospital. The mean age of patients at the t

Extensive Craniofacial Trauma Caused by an Intraoral Pyrotechnic Explosion in a Patient with Paranoid Schizophrenia.

Background: Intraoral explosions of pyrotechnic devices are exceptionally rare and combine blast overpressure, penetrating fragments, thermal injury, and extensive wound contamination. The immediate threats include airway obstruction, hemorrhage, loss of facial buttresses, and destruction of the occlusal unit. This report describes the acute multidisciplinary management of a self-inflicted intraoral pyrotechnic explosion in a male patient during an exacerbation of paranoid schizophrenia. Case presentation: The explosion caused bilateral comminution of the midface, fractures of the mandibular b

Bone Tissue Engineering With Chitosan, Carbon Nanotubes, and Hydroxyapatite Biomaterials Enriched With Mesenchymal Stem Cells: A Radiographic and Histological Evaluation in a Sheep Model Undergoing Ostectomy (Bone Tissue Engineering in a Sheep Model).

Comminuted fractures associated with tissue loss can adversely affect bone regeneration. Biomaterials enriched with mesenchymal stem cells (MSCs) employed for supporting osteosynthesis and potentiating osteoconduction are necessary to fill these bone defects. Natural compound biomaterials, similar to bone tissue, have been extensively tested in animal models for clinical use. Bone tissue engineering studies have used critical-size defects in ovine tibia monitored by imaging and histological examinations to evaluate the regenerative process. This study aimed to monitor the regenerative process
