Polyacrylamide gel for facial wasting rehabilitation: how many milliliters per session?

BACKGROUND: Facial lipoatrophy is most distressing for HIV patients in pharmacologic treatment. Nonabsorbable fillers are widely used to restore facial features in these patients. We evaluated the safety and aesthetic outcomes of two samples of HIV+ patients affected by facial wasting who received different filling protocols of the nonabsorbable filler Aquamid® to restore facial wasting. METHODS: Thirty-one HIV+ patients affected by facial wasting received injections of the nonabsorbable filler Aquamid for facial wasting rehabilitation. Patients were randomly divided into two groups: A and B.

Histopathological evaluation of viscoelastic POSS administered subcutaneously using a rat model.

INTRODUCTION: Polyhedral oligomeric silsesquioxanes (POSS), have a structure resembling a cage in which the corners are silicon atoms bound with oxygen bridges. POSS cages may be dissolved in a polymer matrix or used as a cross-linker. When added into a polymer matrix, they remain non-toxic, improve mechanical properties, and resist biodegradation, which is important in tissue engineering. Limited studies evaluate long-term tissue responses to POSS gels. METHODS: Adult male Sprague Dawley rat were used as a model to determine the tissue response towards multiple dosing of POSS gel compared to

Granulomatous mastitis and pectoralis major muscle defect following polyacrylamide hydrogel injection: a case report and literature review.

BACKGROUND: Breast augmentation through the injection of polyacrylamide hydrogel (PAAG) was a popular procedure in the past, but it has since been prohibited due to various complications, including masses, migration, infection, inflammation, and even cancer. However, there were rare cases of granulomatous mastitis with pectoralis major muscle defect following PAAG injection for breast augmentation. CASE DESCRIPTION: A 40-year-old female patient presented with a swollen and suppurative mass in her left breast and was insensitive to antibiotics. She was admitted to our department for further tr

Long-Term Outcomes of Peripheral Pulmonary Cement Embolism in Patients with Polymethylmethacrylate Augmentation: A Case Series with a Minimum Follow-Up of Five Years.

BACKGROUND: Pulmonary cement embolism (PCE) is a rare but lethal complication. However, few long-term follow-up studies have investigated PCE after polymethylmethacrylate augmentation. This study aimed to investigate both the clinical and imaging outcomes of patients with PCE during a follow-up period of at least 5 years. METHODS: A total of 1460 patients were initially included in this retrospective study. After exclusion, the clinical and imaging data were analyzed for selected patients, including the augmented level, location and length of the PCE, symptoms, therapy, migration and disinteg

Abdominal aortic aneurysm neck remodeling after Anaconda stent graft implantation.

OBJECTIVE: The aim of this study was to define how the proximal landing zone changes geometrically after endovascular abdominal aortic aneurysm repair (EVAR) with the Anaconda (Vascutek, Inchinnan, United Kingdom) stent graft. METHODS: Among 230 patients who underwent Anaconda stent graft implantation between 2005 and 2014, we included 126 with adequate computed tomography (CT) image quality and follow-up. CT analysis entailed the geometric changes in the main body, proximal rings, and proximal landing zone. The median CT follow-up was 2.0 years (345.8 patients-years). RESULTS: The proximal

Endovascular Transpulmonary Retrieval of a Migrated Amplatzer Vascular Plug Following Balloon-Occluded Retrograde Transvenous Obliteration.

Gastric varices are a common manifestation of portal hypertension and are associated with a high rate of mortality and rebleeding. Balloon-occluded retrograde transvenous obliteration (BRTO) is a commonly used method to sclerose gastric varices and has a high clinical success. Common complications following BRTO include portal or splenic vein thrombosis, systemic sclerosant extravasation, pulmonary emboli, and inferior vena cava thrombosis. This report describes a patient with vascular plug migration into the left pulmonary artery with subsequent endovascular retrieval.

Comparing fourth ventricle shunt survival after placement via stereotactic transtentorial and suboccipital approaches.

OBJECT: Fourth ventricle hydrocephalus, or a "trapped" fourth ventricle, presents a treatment challenge in pediatric neurosurgery. Fourth ventricle hydrocephalus develops most commonly as a result of congenital anomalies, intraventricular hemorrhage, or infection. Standard management of loculated fourth ventricle hydrocephalus consists of fourth ventricle shunt placement via a suboccipital approach. An alternative approach is stereotactic-guided transtentorial fourth ventricle shunt placement via the nondominant superior parietal lobule. In this report, the authors compare shunt survival after

Iatrogenic atrial septal defect and aortoatrial fistula in a patient with endovascular prosthesis in the inferior vena cava.

Percutaneous procedures and endovascular prostheses are becoming increasingly frequent, replacing classic interventions, and new complications are now appearing. We report the case of a liver transplant patient with a stenosis in the anastomosis of the suprahepatic veins to inferior vena cava, treated by self-expanding prosthesis, who developed an aorto-right atrial fistula and an atrial septal defect. Open heart surgery was performed to correct the defects. Transthoracic echocardiogram 1 year later revealed no evidence of residual shunt.

Outcomes with retrievable inferior vena cava filters.

This was a retrospective study of 144 patients with retrievable inferior vena cava (IVC) filters inserted between 2004 and 2008 at a community/teaching hospital. The purpose was to evaluate the incidence of complications and the rate and success of retrieval. Retrieval of IVC filters was attempted in 14 of 144 (10%) patients at an average of 4.6 months. Retrieval was successful in 10 of 14 (71%). Within 6 months of insertion, retrieval was successful in 10 of 12 (83%). Unsuccessful attempts were at 3, 6, 8 and 9 months. Non-bleeding complications of IVC filters occurred in 12 of 144 (8.3%). Ha

From clinical trials to clinical practice: 612 cases treated with the Powerlink stent-graft for endovascular repair of AAA.

AIM: Most endovascular stent-grafts in endovascular aortic aneurysm repair (EVAR) are designed to deploy from the renal artery level down to the iliac level. The Powerlink device (Endologix Inc, Irvine, Calif) can be deployed from the iliac level up to the renal artery level. The aim of this study was to summarize our clinical experience of EVAR using the Powerlink device. METHODS: The fully-supported unibody bifurcated Powerlink stent graft was deployed sitting onto the abdominal aortic bifurcation (termed anatomical fixation), a proximal long cuff with or without Palmaz stent may follow the

Embolization of the gastroduodenal artery before selective internal radiotherapy: a prospectively randomized trial comparing platinum-fibered microcoils with the Amplatzer Vascular Plug II.

The Amplatzer Vascular Plug II (AVP II) is a novel device for transcatheter vessel occlusion, for which only limited comparative data exist. Embolotherapy of the gastroduodenal artery (GDA) is essential before internal radiotherapy (SIRT) in order to prevent radiation-induced peptic ulcerations due to migration of yttrium-90 microspheres. The purpose of this study was to compare the vascular anatomical limitations, procedure time, effectiveness, and safety of embolization of the GDA with coils versus the AVP II. Fifty patients stratified for SIRT were prospectively randomized for embolization

Popliteal artery occlusion as a late complication of liquid acrylate embolization for cerebral vascular malformation.

Occlusion of arteriovenous malformations of the brain (BAVMs) by means of an endovascular approach with liquid acrylate glue is an established treatment modality. The specific hazards of this procedure are related to the central nervous system. In the case of unexpectedly rapid polymerization of the cyanoacrylate glue and adhesion of the delivering microcatheter to the BAVM, severing the catheter at the site of vascular access is considered an acceptable and safe management. We present a unique complication related to this technique that has not been described yet. Fragmentation and migration

[Late complications of percutaneous closure of atrial septal defects with the Sideris occluder].

Between June 1992 and January 1996, 27 patients aged 3.9 to 74 years with an ostium secundum (22 patients) or patent foramen ovale with right-to-left shunts (5 patients) underwent percutaneous closure of their atrial septal defects with the Sideris occluder. After a thromboembolic complication, transesophageal echocardiography was performed routinely after the procedure in 15 patients between 1 month and 2 years, and in 6 patients on the 15th day. Two patients died, on the 2nd day and 21st month, of non-related causes. After an average follow-up of 33 months, 59% of patients had complete occlu

Acetabular cup migration. Prediction of aseptic loosening.

We measured the proximal migration of 265 acetabular cups over seven years and correlated the findings with clinical outcome and acetabular revision for aseptic loosening. Cups which eventually became aseptically loose were shown to migrate more rapidly than successful cups. The average proximal migration at two years postoperatively for four groups of cups showed a monotonic relationship to the acetabular revision rate for aseptic loosening at 6.5 years. We conclude that acetabular cups which develop aseptic loosening as evidenced by pain, revision or screw fracture show increased proximal mi

[Temporary vascular access: from peripheral to central, from temporary to permanent].

Temporary vascular access (TVA) is a basic requirement in clinical nephrology. TVA permits immediate and repeated hemodialysis for all type of ESRD patient. Going from peripheral to central and from temporary to permanent angioaccess are the two main trends observed in TVA in contemporary dialysis. Our experience over the last decade illustrates this TVA changes. On the one hand peripheral arterio-venous shunt has been forsaken in favour of percutaneous implanted central venous catheters. On the other hand, two types of central venous catheters have been used covering up our preferences: cathe
