Comparison of radiologic and arthroscopic diagnoses of distal tibiofibular syndesmosis disruption in acute ankle fracture.

PURPOSE: The purpose of this study was to compare intraoperative stress radiography and ankle arthroscopy in the evaluation of distal tibiofibular syndesmosis disruption in acute ankle fracture. TYPE OF STUDY: Prospective study. METHODS: We treated 53 Weber type B or C ankle fractures without radiographic evidence of frank syndesmosis diastasis. Intraoperative stress radiography and ankle arthroscopy were performed. Syndesmotic screws were inserted in those patients with syndesmosis diastasis. Screws were removed 12 weeks later and second-look ankle arthroscopy was performed at the same time

Palmar and dorsal fixed-angle plates in AO C-type fractures of the distal radius: is there an advantage of palmar plates in the long term?

BACKGROUND: Current surgical approaches to the distal radius include dorsal and palmar plate fixation. While palmar plates have gained widespread popularity, few reports have provided data on long term clinical outcomes in comparison. This paper reports the result of a randomised clinical study comparing dorsal Pi plates and palmar, angle-stable plates for treatment of comminuted, intraarticular fractures of the distal radius over the course of twelve months. METHODS: 42 patients with unilateral, intraarticular fractures of the distal radius were included and randomised to 2 groups, 22 were t

Incarcerated patellar tendon in Hoffa fracture: an unusual cause of irreducible knee dislocation.

Irreducible knee dislocation due to incarcerated soft tissue structures in associated fractures is extremely uncommon. We report a case of irreducible, closed dislocation of the knee secondary to incarcerated patellar tendon in a Hoffa fracture (unicondylar fracture of femur in the coronal plane) with associated communited fracture of patella and disruption of both cruciates. We treated this case initially with open reduction and internal fixation of fracture and repair of the patellar tendon. During the total followup period of eighteen months, the patient had restricted joint range of motion

Priorities in the management of penetrating maxillofacial trauma in the pediatric patient.

Penetrating facial trauma is uncommon in children; a large series published by Cooper et al revealed that only 1% to 2% of the total population of infants and children admitted for trauma during their study period had a diagnosis of penetrating trauma to the head or neck. Little has been published specifically addressing these injuries in the pediatric population. The records of 20 patients treated for penetrating facial injuries at Kosair-Children's Hospital in Louisville, Kentucky from January 1991 through December 1994 were reviewed. The location, mechanism and extent of injury, as well as

Effectiveness over time of the reverse shoulder prosthesis for acute proximal humeral fracture.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has proven to be an effective surgical procedure for irreparable rotator cuff tears, comminuted fractures of the proximal humerus, and shoulder arthroplasty revision surgeries. We know from the literature that the functional results are good in the short term, but not whether these results remain stable in the long term or, on the contrary, show deterioration in functionality. OBJECTIVE: This study aims to analyze the functionality, degree of satisfaction, complications, and prosthesis survival at different cut-off points in patients with

Treatment of inferior pole fracture of the patella with tension-free external immobilization.

BACKGROUND: Inferior pole fracture of the patella (IPFP) has small and comminuted fracture blocks that are hard to immobilize, and early mobilization may lead to loss of fracture reduction and immobilization failure. Therefore, a difficulty of treatment is to achieve rigid immobilization with early functional exercise. Here, a new treatment method of tension-free external immobilization is put forward. METHODS: The clinical data of 11 IPFP patients treated with tension-free external immobilization from May 2016 to June 2019 were retrospectively analyzed. There were six males and five females

Comparison of functional outcome after volar plate fixation with 2.4-mm titanium versus 3.5-mm stainless-steel plate for extra-articular fracture of distal radius.

PURPOSE: Open reduction and locked volar plate and screw fixation is a popular treatment method for extra-articular distal radius fractures with dorsal metaphyseal comminution. In this study, we compared the use of a titanium 2.4-mm precontoured plate with that of a stainless-steel oblique 3.5-mm T-shaped plate to test the null hypothesis that there would be no difference in wrist function or upper extremity-specific health status in the internal fixation of AO-type A3.2 distal radius fractures. METHODS: We retrospectively analyzed 24 patients treated with a 2.4-mm titanium plate and 38 patie

The pattern of the fracture and displacement of the fragments predict the outcome in proximal humeral fractures.

Our aim was to determine the effect of the initial pattern of fracture and the displacement of fragments on the outcome of proximal humeral fractures treated conservatively. We followed 93 consecutive patients prospectively for one year. Final movement and strength were compared with those of the contralateral side. The final American Shoulder and Elbow Society score and the Disabilities of Arm, Shoulder and Hand and Short-Form 36 questionnaires were compared with those provided by the patient on the day of the injury. Radiographs and CT scans with three-dimensional reconstruction were obtaine

Dislocation of the mandibular condyle into the middle cranial fossa: report of a case with 5 year CT follow-up.

A 17-year-old patient suffered a dislocation of the right mandibular condyle into the middle cranial fossa following a motorcycle accident. Reduction was delayed because of serious orthopaedic injuries. The condyle was relocated into the glenoid fossa through a combined neurosurgical and preauricular approach. Five years later the patient presented for removal of wisdom teeth and a CT-scan was done to determine the effects of the trauma and management to the condyle. A fibro-osseous ankylosis had occurred in the right temporomandibular joint. The radiological changes confirm that early managem
