Distinct patterns of ligament and meniscal injuries in multiligamentous knee injuries with and without dislocation: a 15-year retrospective study.

PURPOSE: This study aimed to compare the incidence rates of concomitant injuries, including meniscal and cartilage injuries, between multiligamentous knee injuries (MLKI) with and without dislocation based on our 15-year experience of knee dislocation and MLKI at a level 1 trauma center. METHODS: We retrospectively identified 100 patients (115 knees) with MLKIs and/or dislocations at our trauma center between 2007 and 2021. Magnetic resonance imaging was routinely performed to evaluate the injured structures and extent of injury. The anatomic structures of the knee were categorized into anter

Secondary Meniscectomy Rates and Risk Factors for Failed Repair of Ramp Lesions Performed at the Time of Primary ACL Reconstruction: An Analysis of 1037 Patients From the SANTI Study Group.

BACKGROUND: Studies evaluating secondary meniscectomy rates and risk factors for failure of ramp repair are sparse and limited by small numbers and heterogeneity. PURPOSES/HYPOTHESIS: The purposes were to determine the secondary meniscectomy rate for failure of ramp repair performed using a posteromedial portal suture hook at the time of anterior cruciate ligament reconstruction (ACLR) and to identify risk factors for secondary meniscectomy. It was hypothesized that patients who underwent ACLR combined with a lateral extra-articular procedure (LEAP) would experience significantly lower rates

Magnetic resonance imaging does not reliably detect Kaplan fiber injury in the setting of anterior cruciate ligament tear.

PURPOSE: There has been a continued effort to better understand the role Kaplan fiber injury plays in persistent instability following ACL tears. However, the prevalence of these injuries remains poorly understood. Therefore, the purpose of this study was to define the prevalence of Kaplan fiber injury in the setting of complete anterior cruciate ligament tear using a commonly used grading system for assessing ligament injuries. The inter-rater reliability of this commonly used grading system and the relationship between Kaplan fiber injury and injury to other structures commonly found in conj

Leukocyte-dependent effects of platelet-rich plasma on cartilage loss and thermal hyperalgesia in a mouse model of post-traumatic osteoarthritis.

OBJECTIVE: Platelet-rich plasma (PRP) is an emerging therapeutic strategy for treatment of osteoarthritis (OA); however, there is a lack of preclinical and clinical evidence for its efficacy and its mechanism of action is unclear. In the current study, we utilized leukocyte poor-PRP (LP-PRP) and leukocyte rich-PRP (LR-PRP) to mimic clinical point of care formulations and assessed their potential to alter disease progression in a mouse model of post-traumatic OA. METHOD: Three-month-old wild-type male FVB/N mice received destabilization of the medial meniscus (DMM) surgery to induce OA. To ass…

Large chondral defect not covered by meniscal allograft is associated with inferior graft survivorship after lateral meniscal allograft transplantation.

PURPOSE: This study aimed to evaluate graft survivorship according to the size and location of chondral defects and its effect on clinical outcomes after meniscal allograft transplantation (MAT). It was hypothesized that large chondral defects would be associated with inferior outcomes. METHODS: Patients who underwent lateral MAT with fresh-frozen allografts between 2007 and 2016 were retrospectively reviewed. The inclusion criteria were patients with femoral or tibial chondral defects (International Cartilage Repair Society grade 4) who were followed up more than 2 years with 3.0-T magnetic

Is Knee Arthroscopy Beneficial in Treating Traumatic Meniscal Tears on Patients Older Than 60 Years?

Knee arthroscopic surgery is usually performed on young or middle-aged patients with meniscal tears. However, we have noted an amount of act … icate that patients older than 60 years with mild osteoarthritis and clear traumatic history of the meniscus are a good indication for arthroscopic partial meniscectomy. The level of evidence is level IV (a retrospective case series).

Meniscal extrusion or subchondral damage characterize incident accelerated osteoarthritis: Data from the Osteoarthritis Initiative.

Knee osteoarthritis (KOA) is typically a slowly progressive disorder; however, a subset of knees progress with dramatic rapidity. We aimed t … KOA are characterized by structural damage that compromises subchondral bone or the function of the meniscus. Recognizing meniscal extrusion and/or change in shape, lateral meniscal tear, or acute subchondral damage may be vital for identifying individuals at risk for accelerated KOA.

MRI versus ultrasonography to assess meniscal abnormalities in acute knees.

While magnetic resonance imaging (MRI) is often considered the "gold standard" diagnostic imaging modality for detection of meniscal abnormalities, it is associated with misdiagnosis in as high as 47% of cases, is costly, and is not readily available to a large number of patients. Ultrasonographic examination of the knee has been reported to be an effective diagnostic tool for this purpose with the potential to overcome many of the shortcomings of MRI. The purpose of this study is to determine the clinical usefulness of ultrasonography for diagnosis of meniscal pathology in patients with acute

Knee injuries and the use of prophylactic knee bracing in off-road motorcycling: results of a large-scale epidemiological study.

BACKGROUND: The effectiveness of prophylactic knee bracing in preventing knee injuries during sports has been evaluated; however, because of the variability in study conclusions, the topic remains controversial. Despite a paucity of data, the authors believe that prophylactic knee bracing is frequently used in off-road motorcycling. HYPOTHESIS: No statistically significant difference exists in the frequency and types of knee injuries incurred between braced and nonbraced riders using commercially available knee braces in off-road motorcycling. STUDY DESIGN: Descriptive epidemiology study. M

[Plain radiography of the knee: the articular surfaces].

Lateral knee radiographs allow recognition of both medial and lateral femoral and tibial surfaces, groove and anterior borders of the trochlea and lateral facet and ridge of the patella. Analysis of these lines allows detection of focal contour abnormalities, femoral trochlear dysplasia and patellar tilt. Qualitative radiological analysis of the osseous surfaces detects the particular aspect of abraded subchondral bone ("drawn with chalk"), preventing the trap of false joint spaces on non weight-bearing views. Occasionally, very subtle bone abnormalities can be recognized in cases of cartilagi

Relationship of meniscal damage, meniscal extrusion, malalignment, and joint laxity to subsequent cartilage loss in osteoarthritic knees.

OBJECTIVE: Progressive knee osteoarthritis (OA) is believed to result from local factors acting in a systemic environment. Previous studies have not examined these factors concomitantly or compared quantitative and qualitative cartilage loss outcomes. The aim of this study was to test whether meniscal damage, meniscal extrusion, malalignment, and laxity each predicted tibiofemoral cartilage loss after controlling for the other factors. METHODS: Laxity and alignment were measured at baseline in individuals with knee OA. Magnetic resonance imaging included spin-echo coronal and sagittal imaging

Lack of posteromedial tibiofemoral congruence at full flexion as a causative factor in isolated medial meniscal tears.

The aim of the current study was to investigate posteromedial tibiofemoral congruence at full flexion of the knee in control knee joints and those affected by an isolated medial meniscal tear, to examine whether lack of such congruence was a causative factor in isolated medial meniscal tears. In this study, 1677 knee joints in 875 subjects were evaluated. The joints were classified as a control group (1345 joints), an isolated medial meniscal tear group (224 joints), and a contralateral isolated medial meniscal tear group (108 joints). Posteromedial tibiofemoral congruence was examined on a la

Juxta-articular bone marrow signal changes on magnetic resonance imaging following arthroscopic meniscectomy.

PURPOSE: Postmeniscectomy osteonecrosis of the knee has been reported in the past decade but the etiology remains unclear. Some investigators have indicated that bone marrow signal changes evident on magnetic resonance imaging (MRI) could be early warning signs of osteonecrosis. The purpose of this study was to determine the incidence rate, location, and magnitude of such changes in bone marrow of the knee after arthroscopic meniscectomy, using MRI. TYPE OF STUDY: Cohort analytic study. METHODS: Ninety-three patients with no bone marrow signal abnormalities on preoperative MRI were examined
