OBJECTIVE: To assesses the frequency and degree of pituitary hyperplasia in patients with primary hypothyroidism, the association of pituitary enlargement with disease severity, and the response to treatment.
METHODS: Between April 2002 and August 2004 at the National Center for Diabetes, Endocrinology and Genetics in Amman, Jordan, 53 patients (49 female and 4 male subjects) with primary hypothyroidism and serum thyrotropin (thyroid-stimulating hormone or TSH) levels of > or = 50 microIU/mL were encountered. Initial and follow-up investigations included thyroid function tests, serum prolacti

OBJECTIVE: To report 3 cases of bisphosphonate-induced hypocalcemia and review the relevant literature.
METHODS: We present the clinical and laboratory findings in 3 cases of bisphosphonate-induced hypocalcemia, and discuss the pathophysiologic mechanisms and the pertinent literature.
RESULTS: In our first patient (case 1), symptomatic hypocalcemia developed after intravenous administration of pamidronate for management of multiple myeloma. He had vitamin D insufficiency and impaired renal function at the time of pamidronate therapy. Our second patient (case 2) presented with symptomatic hyp

OBJECTIVE: To highlight a strategy for potential detection of mesenchymal tumors in oncogenic malacia, as illustrated by 3 cases.
METHODS: Three case reports are presented in which successful localization of the offending neoplasm was accomplished by using whole-body Tc 99m sestamibi scanning. Alternative localization techniques are also reviewed.
RESULTS: Oncogenic osteomalacia occurs infrequently and is caused by neoplasms that secrete phosphatonins, substances that interfere with proximal tubular resorption of phosphorus and can result in phosphaturia, hypophosphatemia, reduced 1,25-dihyd

OBJECTIVE: To determine reasons for hospitalization among adult patients with diabetes.
METHODS: A cross-sectional analysis was conducted of hospital discharges in the state of Georgia for the years 1998 through 2001 that contained either a primary or a coexisting diagnosis of diabetes. With use of the Clinical Classification Software of the Agency for Healthcare Research and Quality, the principal diagnoses among diabetes-related hospital discharges were organized into diagnostic categories.
RESULTS: Diabetes was listed as a diagnosis in 14% of all Georgia hospital discharges of adult patie

OBJECTIVE: To report the occurrence of hypocalcemia in a patient with metastatic prostate cancer, discuss its pathogenesis, and review the related medical literature.
METHODS: An 82-year-old man with a known history of prostate cancer was found to have a serum calcium level of 5.4 mg/dL during an admission to the hospital for small bowel obstruction. A thorough review of his medical history revealed a temporal relationship between the diagnosis of malignant disease and progressive hypocalcemia. A complete evaluation was performed, including laboratory and imaging studies, to ascertain the cau

OBJECTIVE: To describe the clinical course of a patient with atypical cystic parathyroid adenoma manifesting as hypercalcemic parathyroid crisis.
METHODS: We present a case report and review the relevant literature on parathyroid cysts and atypical cystic parathyroid adenomas.
RESULTS: A 76-year-old woman was evaluated for weakness, weight loss, urinary incontinence, and memory disturbances. She had severe hypercalcemia (calcium level, 18.3 mg/dL) in conjunction with an elevated parathyroid hormone level of 1,472 pg/mL. She received aggressive hydration, pamidronate, and calcitonin. METHODS: We present the clinical and laboratory findings in a patient with anaplastic thyroid carcinoma and thyrotoxicosis. We also review the literature for previous cases of anaplastic thyroid carcinoma with thyrotoxicosis.
RESULTS: A 74-year-old man presented with left-sided neck pain and a rapidly enlarging neck mass. Initial thyroid function tests revealed the following: thyrotropin (thyroid-stimulating hormone or TSH) 0.03 microIU/mL, free thyroxine

OBJECTIVE: To report several cases of hyperthyroidism in patients presenting with the unusual symptom of sleepwalking and to discuss the possible pathophysiologic basis for this novel association.
METHODS: After encountering and reporting the first case of new-onset somnambulism in a patient presenting with thyrotoxicosis at our institution, we routinely inquired about the sleep history of patients with thyrotoxicosis, questioning both the patients and family members when applicable. Those patients who actually had sleepwalking episodes coinciding with the onset of thyrotoxicosis underwent cl

OBJECTIVE: To present a series of cases of parathyroid incidentalomas and review the related medical literature.
METHODS: The medical records of all patients of one surgeon who underwent unplanned removal of enlarged parathyroid glands found either during a surgical procedure performed for indications other than hyperparathyroidism or by ultrasonography of the thyroid gland between September 1989 and December 2003 were reviewed retrospectively. Clinical and laboratory data-- especially serum calcium and parathyroid hormone (PTH) levels--as well as information on postoperative parathyroid func

OBJECTIVE: To report a case of extensive calcinosis cutis presumably caused by silicone injections, which was complicated by hypercalcemia.
METHODS: A long-standing case of calcinosis cutis with subsequent development of hypercalcemia is presented, and possible mechanisms for the underlying metabolic dysfunction are discussed.
RESULTS: A 78-year-old woman presented with severe hypercalcemia and was found to have extensive subcutaneous calcifications on the anterior chest wall and abdomen, likely related to previous silicone injections. The hypercalcemia was treated with intravenously adminis

METHODS: We report the clinical and laboratory data in 3 patients who presented with severe hypercalcemia (corrected serum calcium > or = 14 mg/dL) and review the pertinent literature on milk-alkali syndrome.
RESULTS: The 3 patients had acute renal insufficiency, relative metabolic alkalosis, and low parathyroid hormone (PTH), PTH-related peptide, and 1,25-dihydroxyvitamin D concentrations. No malignant lesion was found. Treatment include

OBJECTIVE: To assess the short-term effects of sibu-tramine on blood pressure (BP) and heart rate (HR) in normotensive and controlled hypertensive obese patients by using ambulatory blood pressure monitoring (ABPM).
METHODS: Before and 5 to 7 days after the initiation of sibutramine treatment (10 mg or 15 mg daily), when steady-state concentrations of sibutramine had been reached, ABPM was performed in 81 obese patients. Of this overall study group, 47 patients (26 with normoten-sion and 21 with controlled hypertension) received sibu-tramine in a dosage of 10 mg/day, and 34 patients (18 with
